[A Case of Advanced Esophagogastric Junction Adenocarcinoma with Enlarged Lymph Node Metastases of the Middle Mediastinum and Intraperitoneal That Exhibited pCR by Preoperative SOX Therapy].

A 79-year-old man was diagnosed with esophagogastric junction adenocarcinoma, cT3N3M0, cStage Ⅲ, including enlarged lymph node metastases(Bulky N)in the middle mediastinum and intraperitoneal. A total of 2 cycles of S-1 plus oxaliplatin(SOX)was administered. After neoadjuvant chemotherapy, the primary tumor and enlarged lymph nodes had greatly decreased in size. Subsequently, thoracoscopic subtotal esophagectomy and reconstruction with a gastric tube were performed. Histopathological examinations showed no residual cancer cells in the primary lesion and dissected lymph nodes (pathological comp

Plasma Cell Gingivitis Mimicking Granulomatous Disease: A Diagnostic Dilemma.

INTRODUCTION: Plasma cell gingivitis is an uncommon benign inflammatory condition of the gingiva which is considered to arise as a hypersensitivity reaction to certain allergens. The distinguishing feature of this condition is massive infiltration of plasma cells into the subepithelial connective tissue. This case report discusses a case of plasma cell gingivitis which produced a diagnostic dilemma due to its unusual histopathologic appearance. CASE PRESENTATION: A 13-year-old female patient presented with diffuse gingival enlargement in relationship to upper anterior teeth. The case was mana

Does the epithelium play a central role in the immune function of rhinopharyngeal tonsils? An immunocytochemical and ultrastructural study.

Enlarged adenoids from 10 children with chronic rhinitis and otitis media with effusion have been studied immunocytochemically and ultrastructurally, to better define the possible role of the epithelium and the dendritic accessory cells in the immune activation of lymphoid cells, and provide further insight into the pathogenesis of the disease. The presence within the columnar epithelium of lymphocytes positive for CD8 antigen, and which electron microscopically have been found frequently apposed to degenerating epithelial cells suggests that the latter cells are targets for cytotoxic activity

Pediatric IgG4-related dacryoadenitis and sialadenitis (Mikulicz's disease) with acquired hemophilia A: A case report and review of literature.

We report a case of IgG4-related dacryoadenitis and sialadenitis (Mikulicz's disease) with acquired hemophilia A (AHA) in a pediatric patient. An 11-year-old female presented with intermittent swelling of the bilateral upper eyelids and neck areas. Clinical examination revealed bilateral swollen upper eyelids with palpable mass lesions in the lacrimal fossa. The submandibular and sublingual glands were enlarged, tender, and movable. Neck ultrasound showed bilateral gland enlargement with irregular hypoechoic parenchyma. Maxillofacial magnetic resonance imaging (MRI) revealed bilateral lacrimal

Fibrolipomatous hamartoma of nerve. A clinicopathologic analysis of 26 cases.

Nineteen cases of fibrolipomatous hamartoma of nerve without macrodactyly and seven cases with macrodactyly are discussed. Twenty-five involved the hand, wrist, palm, and finger, and one case involved the foot. Nineteen patients had isolated fibrofatty enlargement of nerve, while seven had macrodactyly in addition to the peripheral nerve changes. Involved nerves included the median nerve, ulnar nerve, an unidentified nerve near the elbow, and a nerve on the dorsum of the foot. Four of nine patients with neurologic symptoms of pain or paresthesias had physical findings compatible with compressi

Histopathological and ultrastructural changes of lichen sclerosus et atrophicus on the vulva.

Studies of 82 skin specimens from 81 cases of lichen sclerosus et atrophicus (LSA) are reported. Typical LSA pathological changes showed atrophy of the epidermis, hydropic degeneration at the basal layer, and homogenization of the collagen fibers in the superficial dermis with inflammatory cell infiltration underneath. Ultrastructural changes showed intercellular spaces enlarged, particularly near basal cells. Some mitochondria were swollen and rough endoplasmic reticula were enlarged. Some basement membranes were thinner, collagen fibers of varying diameters were arranged in irregular shapes

Lymphangioma Circumscriptum - A Rare Cause of Gingival Enlargement: A Case Report and Review of Literature.

BACKGROUND: Lymphangiomas or microcystic lymphatic malformations (MLM) are hamartomatous formations that occur due to the proliferation of lymphatic vessels. They commonly manifest in the head and neck region with only a few cases reported within the oral cavity. Lymphangioma circumscriptum in the gingiva is a rare condition which presents as asymptomatic pebbly gingival enlargement. They are characterized by lesions that are microscopic thin-walled cysts. Histopathologically, they show multiple dilated lymphatic channels which are lined by endothelial cells. The lumen is filled with lymphatic

Diagnostic exercise: epithelioid hemangiosarcoma mimicking metastatic prostatic neoplasia in a dog.

A 10-year-old intact male Labrador Retriever dog was euthanized because of rapid deterioration after suffering from mild chronic cough, hematuria, acute blindness, ataxia, and lethargy. Clinical examination revealed blepharospasm and hyphema, with clear discharge from the right eye; a firm mass in an enlarged right testicle; a mass in the irregularly enlarged prostate; and nodules in the left kidney, liver, and spleen detected by abdominal sonography. Cytologic evaluation of fine needle aspirates from the prostate, testis, and kidney comprised large, clustered or individualized, anaplastic cel

Stem Cells in Myometrial Physiology.

Normal adult tissues contain a small subset of tissue-specific stem cells. These stem cells are capable of self-renewing as well as generating daughter cells that are destined to completely differentiate and thereby support tissue remodeling and repair. The human uterus is an example of how such cell populations support the function of an organ. The uterus primarily consists of endometrium and myometrium, and these tissues rapidly enlarge to support the developing fetus during pregnancy. Uterine enlargement and remodeling can occur more than a dozen times during a woman's reproductive life. Th

Ballooned hepatocytes in steatohepatitis: the value of keratin immunohistochemistry for diagnosis.

BACKGROUND/AIMS: Hepatocyte "ballooning" is an often used but ill defined term in liver pathology to designate a special form of liver cell degeneration associated with cell swelling and enlargement found particularly in steatohepatitis. Alterations of the intermediate filament cytoskeleton of the hepatocyte may contribute to the pathogenesis of this microscopic change. Ballooning degeneration is considered a hallmark of steatohepatitis, but enlarged hepatocytes may also be observed in a variety of other acute and chronic liver diseases. METHODS: The intermediate filament cytoskeleton was inv

Metaplastic Squamous Cell Carcinoma of Breast Demonstrated on 18F-FDG PET/CT.

A 32-year-old woman presented with 3 months' history of rapid enlargement of the left breast and superficial ulceration. Staging F-FDG PET/CT performed after admission demonstrated a hypermetabolic mass in the left breast, with smaller hypermetabolic areas in the left axillary and internal mammary lymph nodes, as well as in the sternum. Postoperatively, the mass was diagnosed as metaplastic squamous cell carcinoma of the breast.

Conservative management of gynecomastia in Peutz-Jeghers syndrome: Case series and review of the literature.

Peutz-Jeghers syndrome (PJS) is a childhood-onset cancer predisposition syndrome that is associated with oral freckling and gastrointestinal polyposis. Male patients with PJS are at risk for large-cell calcifying Sertoli cell tumors in childhood. These tumors are estrogen-producing and can cause symptoms of precocious puberty, gynecomastia, and growth acceleration. Here we discuss our experience with spontaneous resolution or stabilization of breast enlargement without medical intervention in three patients with PJS and gynecomastia. These cases indicate that a watchful waiting approach can be

Carney complex with biatrial cardiac myxoma.

Cardiac myxomas make up approximately 50% of all benign cardiac tumors and represented 86% of all surgically treated cardiac tumors. Most of them originated from the left atrium, in some cases from both of atria. We report a case of male patient with biatrial myxomas and other extra-cardiac involvement: hypophyseal adenoma, enlargement of thyroid gland, tubular adenoma polyp of colon and bilateral large cell calcifying Sertoli cell tumor (LCCSCT) of testis. These findings led to the diagnosis of Carney's complex, which is a syndrome with multiple neoplasias, cardiac myxomas, lentigines, and en

[Circulating lymphocyte populations and B-cell differentiation in a young female patient with multicentric giant lymph node hyperplasia].

Giant multicentric hyperplasia of the lymph nodes (GMHLN) is currently regarded as a disseminated form of angiofollicular lymphoid hyperplasia, or Castleman's disease. An immunologic study was carried out on a 24 year-old Caucasian woman who was admitted to hospital with generalised lymph node enlargement, jaundice and fever, and showed an excellent response to steroid therapy. No alterations were found in the lymphocytic subsets, the intrinsic B cell function or the T-B cooperative capability. These data disagree with the hypothesis than GMHLN is due to a deficiency of the T cell suppressive

Plasma cell pododermatitis with chronic footpad hemorrhage in two cats.

Plasma cell pododermatitis was diagnosed in 2 cats with enlargement of the metacarpal and metatarsal footpads, ulceration of one of the affected footpads, and a history of chronic hemorrhage from the ulcerations. One cat was anemic (PCV, 14.6%). The ulcers were debrided and sutured to control hemorrhage, and the cats were treated with immunosuppressive doses of corticosteroids. Both cats had considerable reduction in footpad size after 3 to 4 weeks of treatment. Although there is evidence to suggest that plasma cell pododermatitis might be immune-mediated, or perhaps an allergic disease, the c

Mandibular salivary gland plasma cell tumor in a dog.

An 8-year-old, intact male, Jack Russell Terrier dog was admitted with a 10-day history of an enlargement in the right submandibular area. Clinical examination revealed a subcutaneous, solid, firmly attached and painless mass (3 × 2 cm) located in the area of the right mandibular salivary gland, in an otherwise seemingly healthy dog. Fine-needle-aspiration cytology suggested a round cell neoplasia, most consistent with a plasma cell tumor. Histopathologic and immunohistochemical (MUM1) examination, following surgical excision of the mass, confirmed the diagnosis of a plasma cell tumor embedded

Virilizing Juvenile Granulosa Cell Tumor in a Prepubertal Girl with Bilateral Inguinal Hernias: A Case Report.

BACKGROUND: Juvenile granulosa cell tumors usually cause estrogenic precocious puberty; virilization is rare. CASE: After staged herniotomies for bilateral inguinal hernia, a 4-year-old girl was noted to have pubic hair, clitoromegaly, adult-type body odor, bilateral breast enlargement, a large pelvic mass, and vaginal bleeding. Gonadotropins were suppressed, while estradiol, testosterone, anti-Müllerian hormone, inhibin A, and CA 125 were elevated. Imaging showed a left adnexal mass with ascites. Left oophorectomy confirmed stage IA juvenile granulosa cell tumor. Sex steroids normalized with

Dentinogenic ghost cell tumor of the peripheral variant mimicking epulis.

Dentinogenic ghost cell tumor (DGCT) is an uncommon locally invasive odontogenic tumor regarded by many as a variant of calcifying odontogenic cyst. The peripheral variant of this clinical rarity appears as a well-circumscribed mass mimicking a nonspecific gingival enlargement. Microscopic appearance of odontogenic epithelium admixed with focal areas of dentinoid formation and sheets of ghost cells giving the definitive diagnosis of dentinogenic ghost cell tumor imply that microscopic examination is compulsory for any gingival mass. Van Gieson histochemical stain further confirmed the nature o

Occult gastric signet ring cell carcinoma presenting as spermatic cord and testicular metastases: "Krukenberg tumor" in a male patient.

Krukenberg tumor is a well-known ovarian metastasis, usually of gastric signet ring cell carcinoma in female patients. Although gastric carcinoma is more frequent in men, to our knowledge, only few cases of counterpart testicular metastases have been described as yet. We report a 64-year-old patient who complained of right testicular pain. Right-sided orchiectomy was performed, and metastatic signet ring cell carcinoma of spermatic cord and testis was diagnosed through histological examination and immunohistochemistry. The following stomach biopsy confirmed the primary signet ring cell carcino
