Moyamoya disease in pediatric patients: outcomes of neurosurgical interventions.

Neurosurgical interventions for moyamoya disease (MMD) in pediatric patients include direct, indirect, and combined revascularization procedures. Each technique has shown efficacy in the treatment of pediatric MMD; however, no single study has demonstrated the superiority of one technique over another. In this review, the authors explore the various studies focused on the use of these techniques for MMD in the pediatric population. They summarize the results of each study to clearly depict the clinical outcomes achieved at each institution that had utilized direct, indirect, or combined techni

Cerebral hemodynamics in acute stroke: pathophysiology and clinical implications.

BACKGROUND: The capacity of the brain to regulate its blood flow in order to meet metabolic demands and to compensate for acute and chronic changes in cerebral perfusion pressure (cerebral autoregulation) is an essential protecting mechanism against cerebral ischemia. METHODS: We reviewed existing data on methods of assessing cerebral blood flow and autoregulation. RESULTS: Cerebral autoregulation is mechanistically complex and depends on myogenic, neuronal, endothelial, and metabolic factors. There are numerous methods of estimating cerebrovascular reserve (CVR) non-invasively including Pos

Changes in periventricular anastomosis after indirect revascularization surgery alone for adult patients with misery perfusion due to ischemic moyamoya disease.

Periventricular anastomosis in moyamoya disease (MMD) is an unusual angiographic finding that arises from perforating arteries such as the lenticulostriate artery (LSA), thalamic artery (THA), and anterior choroidal artery (AChA). This anastomosis is associated with increased hemorrhagic risk in MMD and can be corrected by direct revascularization surgery. The present supplementary analysis on a prospective cohort aimed to elucidate changes in periventricular anastomosis after indirect revascularization surgery alone for adult patients with misery perfusion due to ischemic MMD. Twenty-two pati

External carotid revascularization: review of a ten-year experience.

In the presence of ipsilateral internal carotid artery (ICA) occlusion, external carotid artery (ECA) revascularization can improve cerebral perfusion or eliminate an embolic source. From 1974 through 1984, 37 patients at The Cleveland Clinic underwent 42 ECA reconstructions; autologous patch angioplasty and intraluminal shunting were used when feasible. Thirty procedures were limited to primary ECA revascularization, whereas 12 extended procedures were performed as reoperations after previous ECA endarterectomy or required complementary subclavian or intracranial bypass. There were no early p

Preoperatively estimated graft flow rate contributes to the improvement of hemodynamics in revascularization for Moyamoya disease.

OBJECTIVES: Superficial temporal artery (STA)-middle cerebral artery (MCA) bypass operation is an effective treatment for patients with Moyamoya disease, and the hemodynamic parameters are reported to be improved after operation. However, there is no report concerning hemodynamic changes from the viewpoint of the preoperative anatomical structure of grafts. In this study, we evaluated the correlation between the preoperatively estimated blood flow of the graft obtained through image-based computational fluid dynamics (CFD) analysis and the hemodynamic changes in the acute phase after revascula

Multiterritorial tailored revascularization in adult Moyamoya patients: expanding revascularization approaches to ACA and PCA territories.

Extracranial-intracranial (EC-IC) bypass surgery within the middle cerebral artery (MCA) territory is the gold standard for treating Moyamoya patients once hemodynamic insufficiency is confirmed. Although the anterior (ACA) and posterior cerebral arteries (PCA) are frequently involved, revascularization of these territories is not yet part of the standard treatment regimen; most centers focus exclusively on the MCA. Our approach, however, targets all hemodynamically compromised regions by providing tailored, multi-territorial revascularizations. Indications for additional ACA and/or PCA bypass

'Y'-Shaped Palmar Common Digital Artery Interposition Bypass in the Treatment of an Infantile M2 Large Aneurysm and Literature Review.

OBJECTIVE: Both craniotomy and interventional embolization are difficult and risky to treat complex middle cerebral artery (MCA) aneurysms in infants. Trapping with revascularization is a therapeutic option for giant aneurysms that cannot be clipped or coiled alone. METHOD: We describe a technical method using revascularization with a natural Y-shaped palmar common digital artery interposition graft that provides a normal variation for a complex MCA aneurysm in an infant with intracerebral hemorrhage at 37 days of age. Conservative treatment was performed at that time. Seven months later, the

[Pulsatile echography (ultrasonic cerebral tomosphygmography) in carotid and vertebral pathology. Surgical applications].

Ultrasonographic cerebral tomosphygmography (U.C.T.S.) is a method for determining cerebral pulsatility by centimeter-thick sections. The prototype uses a 2 MHz ultrasound wave emitted in the direction of cerebral structures. The reflected wave is captured by centimeter-thick sections, the mean determined and a pulsatile index defined. This index varies according to the regions studied (cortico-subcortical, superficial sylvian, deep sylvian and vertebrobasilar). Physiologic pulsatility was determined from a series of 150 healthy subjects. This technique was used to evaluate effects of carotid

The internal mammary artery as an alternative graft for internal maxillary artery-middle cerebral artery bypass: illustrative case.

BACKGROUND: Extracranial-intracranial revascularization has evolved to include bypasses using interposition grafts, most commonly the saphenous vein (SV) or radial artery (RA). These high-flow conduits are essential for treating giant or complex aneurysms, skull base tumors encasing the carotid or middle cerebral arteries, and selected cases of ischemic cerebrovascular disease requiring preservation or augmentation of cerebral blood flow. OBSERVATIONS: This report describes the first case of cerebral revascularization using the internal mammary artery (IMA) as an interposition graft. The pati…

In Situ M2 Bypass for Treatment of Fusiform Middle Cerebral Artery Bifurcation Aneurysm: Technical Case Instruction and Operative Video.

BACKGROUND AND IMPORTANCE: Fusiform middle cerebral artery (MCA) bifurcation aneurysms can be challenging to treat with standard endovascular or microsurgical techniques. The in situ side-to-side bypass technique represents an elegant revascularization option for these aneurysms when trapping becomes necessary. CLINICAL PRESENTATION: A man in his 50s presented for evaluation of an incidentally found fusiform, 10 mm, right MCA bifurcation aneurysm with involvement of both the inferior and superior M2 trunks. Optimizing the Approach to STA-MCA Bypass Surgery and Reducing Wound Complications.

BACKGROUND AND OBJECTIVES: The treatment of complex intracranial aneurysms with bypass surgery using 2 branches of the superficial temporal artery (STA) proves to be an effective surgical option. However, the harvest of these 2 STA branches, combined with a pterional craniotomy, carries the potential risk of delayed wound healing of the skin flap. This study undertook a retrospective analysis to examine and identify the factors associated with this delayed wound healing. METHODS: A total of 56 consecutive cases, including both ruptured and unruptured complex intracranial aneurysms, that under
