[Static-dynamic computerized tomography in the diagnosis of traumatic lesions of alar ligaments. Preliminary results].

The patients affected with cervical injuries often complain of cervical pain, headache and dizziness even when no bone fractures are detected. Such patients are likely to have a post-traumatic injury of the cervical ligaments. Twenty-five symptomatic patients (19 women and 6 men) were examined with upper spine CT and functional CT scans (right and left rotation) to detect ligament injuries and hypermotility of the craniocervical junction, both related to traumatic events. Eleven patients showed no alterations, while unilateral densitometric alterations of the alar ligaments were observed in 14

[Severe trauma of neck and skull base ].

The concept of internal decapitation comprises lesions within the skull base, upper spinal column with partial or complete destruction of the cervical part of the medulla. By means of casualties due to internal decapitation we demonstrate that, besides the known causes, a rotational damage at the juncture of the upper spinal column to the skull base as well as fractures by axial stretching may cause such traumas. A decrease of such accident's consequences is hardly possible because of the amount of energy involved causing the damage. Due to a steady increase of traffic, particularly by motor-b

[Cervical vertebra-induced hearing and equilibrium disorders. Recent clinical aspects].

The contribution of cervical spine disorders to cochleovestibular symptoms was based on the evaluation of patients complaining of dizziness and/or inner ear hearing loss, treated in the ENT Department of Tubingen University between 1977 and 1982. Cervicogenic vertigo was provoked by movements of the cervical spine, but the latter rarely caused tinnitus and never induced hearing loss. The diagnosis between a vascular or proprioceptive nystagmus may be made on the presence of lesions of the cervical spine. These are disorders of the cranio-cervical or the thoraco-cervical region especially and m

Cerebellar infarctions secondary to cranio-cervical anomalies: a case report.

We report a case of cerebellar infarctions which occurred in the territories of the bilateral posterior inferior cerebellar arteries. This case was complicated with cranio-cervical anomalies composed of assimilation of the atlas, atlanto-axial dislocation, and basilar impression. The 40-year-old male patient had no detectable risk factors predisposing to atherosclerotic arterial occlusion or cardiogenic embolism, and there were no angiographic findings of atherosclerosis. It was, therefore, postulated that the cerebellar infarctions were secondary to those cranio-cervical anomalies. The develo

The clinical characteristics and therapy of syndrome of craniocerebral-cervical vertebral injury.

OBJECTIVE: To explore the clinical characteristics and new treatment for syndrome of craniocerebral-cervical vertebral injury. METHODS: The clinical data of 52 patients with head injury accompanied by neck injury were analyzed retrospectively. RESULTS: Craniocerebral injury could result in damage to cervical vertebrae, muscles, vessels and nerves, and even cause vertebral artery injury, which may lead to insufficient blood-supply of vertebral-basal artery. All patients were treated with cervical vertebral traction and the results were good. CONCLUSIONS: Acute craniocerebral injury with symp

MRI of the brain and craniocervical junction in Morquio's disease.

We reviewed MRI of the brain and cervical spine in 11 patients with Morquio's disease. No abnormality was seen in the brain. The odontoid peg was abnormal in all patients, with varying degrees of cord compression due to an anterior soft tissue mass and indentation by the posterior arch of the atlas. The degree of cord compression was more marked than suggested by the symptoms and signs. We recommend MRI of the cervical spine in children with Morquio's disease before the development of neurological symptoms, to optimise the timing and type of surgical intervention.

The atlas-axis complex in Dibamidae (Reptilia: Squamata) and their potential relatives: The effect of a fossorial lifestyle on the morphology of this skeletal bridge.

We report on the first detailed study of the atlas-axis complex in the lizard clade Dibamidae, a family of poorly known fossorial squamates distributed in tropical or subtropical climates. This skeletal bridge is characterized by several features, such as the complete absence of the first intercentrum or the appearance of the first free cervical rib on the axis (usually less developed in Dibamus relative to that in Anelytropsis). Our study shows morphological differences of the atlas-axis complex in the Mexican blind lizard Anelytropsis relative to those of Asian Dibamus, the only two known ex

Comparison of radiological and clinical outcomes after surgical reduction with fixation or halo-vest immobilization for treating unstable atlas fractures.

BACKGROUND: Unstable atlas fractures with concomitant transverse atlantal ligament (TAL) injury may be conservatively managed by halo-vest immobilization (HVI) or surgically treated by various fixation techniques. Many surgeons prefer surgical management due to complications, nonunion, and further dislocations with HVI. There are no comparative studies on surgical and nonsurgical management of unstable atlas fractures. We retrospectively assessed the radiological and clinical outcomes of surgical reduction with fixation vs. non-operative treatments for unstable atlas fractures with TAL rupture

Ultrasound-guided needle-knife release for treatment-refractory cervicogenic headache: a case report.

BACKGROUND: Cervicogenic headache (CEH) is a chronic secondary headache syndrome originating from the upper cervical spine. Although conventional treatments such as pharmacological management, nerve blocks, and radiofrequency ablation are effective for many patients, a subset remains refractory to standard therapies. Soft tissue adhesions around the atlas (C1) are a potential but often overlooked pain generator. This report describes the successful management of intractable CEH through use of ultrasound-guided needle-knife release targeting the transverse process of the atlas. CASE DESCRIPTIO

Perioperative Complications of the Occipitocervical Fixation Surgery for Cervical Trauma in Patients Aged Over 80 Years.

INTRODUCTION: Although severe perioperative complications have been reported after occipitocervical fixation surgery for cervical trauma, no studies have specifically evaluated these complications in patients aged over 80 years. This study aimed to assess perioperative complications following occipitocervical fixation surgery for cervical trauma in patients older than 80 years. METHODS: Among the 1,512 patients with cervical injury treated between 2010 and 2020, 41 patients aged ≥65 years who underwent occipitocervical fixation surgery were enrolled at 77 institutions in Japan. Patients were

Developmental abnormalities of the cervical spine below the axis.

The majority of cervical spine anomalies of clinical significance occur in the occipital, atlas, and axis areas. Below the axis, changes may occur as local solitary anomalies simulating significant disease, or in the form of more widespread segmentation and ossification anomalies categorized as the Klippel-Feil syndrome.

Atypical extensive extratemporal hyperpneumatization of the skull base including the cervical spine: case report and review of the literature.

STUDY DESIGN: Case report and clinical discussion. OBJECTIVE: To describe a rare case of hyperpneumatization of the skull base including the cervical spine with the atlas bone. SUMMARY OF BACKGROUND DATA: Initial imaging studies of physically traumatized patients consist of conventional radiographs. An atypical radiolucency is often misdiagnosed as a primary malignancy or a secondary osteolytic metastasis. Further imaging studies may reveal an underlying atypical hyperpneumatization as a very rare benign differential diagnosis. Pathophysiologically, embryological developmental anomalies as w

[A case of atlanto-axial rotatory fixation associated with Jefferson's fracture].

The authors present a case of atlanto-axial rotatory fixation associated with Jefferson's fracture. A 52-year-old man was admitted to our hospital complaining of severe neck pain and torticollis after a traffic accident. Cervical x-ray films showed fracture of the atlas. CT scans demonstrated traumatic subarachnoid hemorrhage and atlanto-axial rotatory fixation. The patient was managed with skull traction and reposition was achieved. Although he was maintained in a halo device for 4 months, rotary fixation recurred. He underwent posterior fusion between the atlas and axis. We review the litera

Isolated fracture of the medial portion of the lateral mass of the atlas: a previously undescribed entity.

Four cases of fracture of the medial aspect of the lateral mass of the atlas are presented. Unlike most cases of atlantal fracture, definite history of a blow to the vertex could not be established. While the exact mechanism of injury is unknown, it is postulated that the bone fragment is produced by a combination of stretching the ligamentum transversarium and pressure on the lateral mass due to contraction of the neck muscles. All four patients were neurologically intact and complained of pain in the distribution of the greater occipital nerve. It is suggested that this fracture might be ide
