Pathophysiology of autonomic dysreflexia: long-term treatment with terazosin in adult and paediatric spinal cord injury patients manifesting recurrent dysreflexic episodes.

INTRODUCTION: Spinal cord injury (SCI) results in disruption of synaptic influences on the sympathetic preganglionic neurones. Remodelling of spinal cord circuits takes place in spinal neurones caudal to cord injury. There is an increased vascular alpha-adrenoceptor responsiveness, and peripheral afferent (bladder) stimulation in SCI subjects induces a marked noradrenaline spillover below the level of spinal lesion. These neurophysiological changes possibly contribute to the development of autonomic dysreflexia, a condition of sympathetic hyper-excitability that develops after cervical, or upp

Does two-level hybrid surgery promote early fusion compared with two-level anterior cervical discectomy and fusion?

BACKGROUND CONTEXT: Hybrid surgery (HS), which involves both anterior cervical discectomy and fusion (ACDF) and cervical disc replacement (ACDR), is increasingly used to treat multilevel cervical disc degenerative disease, yielding satisfactory clinical outcomes. Early fusion is critical after anterior cervical fusion surgeries, but there are no studies comparing the rate of early fusion of HS with that of ACDF. PURPOSE: The purpose of this study was to compare the rate of early fusion (3-6 months postoperatively) of two-level HS with that of two-level ACDF surgery. STUDY DESIGN: The fusion

Vestibular prognosis in idiopathic sudden sensorineural hearing loss with vestibular dysfunction treated with oral or intratympanic glucocorticoids: a protocol for randomized controlled trial.

BACKGROUND: Idiopathic sudden sensorineural hearing loss (ISSNHL) is a rapid-onset sensorineural hearing impairment with unclear etiology and unsatisfying treatment effects. Vestibular dysfunction has been considered as a poor indicator in the clinical manifestations and prognosis of ISSNHL, which occurred in approximately 28-57% cases. Glucocorticoids, administered through oral or intratympanic way, are currently regularly and standardly applied for ISSNHL to improve the hearing outcome. However, the vestibular prognosis of ISSNHL after routine treatments remains seldom explored. This study a

Aggressive primary uterine non-Hodgkin's lymphoma presenting as acute kidney injury: a case report.

INTRODUCTION: Primary malignant lymphoma of the cervix is an extremely rare condition, accounting for only 0.008% of all cervical tumors and 2% of female extranodal lymphomas. The most common histological subtype is diffuse large B-cell lymphoma. This malignancy is often asymptomatic in its early stages, but advanced cases may include systemic symptoms, pelvic discomfort, and vaginal bleeding. Diagnosis is challenging due to its nonspecific clinical presentation and similarity to other gynecological conditions. Imaging and immunohistochemistry play essential roles in diagnosis and staging. Practice Patterns in Pharmacological and Non-Pharmacological Therapies for Children with Mild Traumatic Brain Injury: A Survey of 15 Canadian and United States Centers.

Given the lack of evidence regarding effective pharmacological and non-pharmacological interventions for pediatric mild traumatic brain injury (mTBI) and the resultant lack of treatment recommendations reflected in consensus guidelines, variation in the management of pediatric mTBI is to be expected. We therefore surveyed practitioners across 15 centers in the United States and Canada who care for children with pediatric mTBI to evaluate common-practice variation in the management of pediatric mTBI. The survey, developed by a panel of pediatric mTBI experts, consisted of a 10-item survey instr

Bilateral Vestibulopathy: Vestibular Function, Dynamic Visual Acuity and Functional Impact.

Introduction: Patients suffering from bilateral vestibular hypofunction (BVH) often experience ataxia as well as visual instability. Even though progress has been made in vestibular testing, insights regarding vestibular deficit in BVH remain incomplete since no method allows evaluation of frequency ranges of vestibular sensors in a continuous way. The aim of our study was to give a detailed description of the level of vestibular deficit in different ranges of vestibular stimulation and an exhaustive evaluation of the functional impact including dynamic visual acuity (DVA) in a cohort of BVH p

Patterns of abnormality in cVEMP, oVEMP, and caloric tests may provide topological information about vestibular impairment.

BACKGROUND: The cervical vestibular evoked myogenic potential (cVEMP) is recorded from the sternocleidomastoid muscle (SCM) and represents a stimulus-evoked attenuation of electromyographic (EMG) activity following activation of the saccule and inferior vestibular nerve. In addition to the cVEMP, it is possible to record a biphasic response from the infraorbital region following stimulation that is identical to that used to record the cVEMP. This response is known as the ocular VEMP (oVEMP). The peripheral vestibular origins of the oVEMP elicited with air conduction remain controversial as som

The treatment of acute vertigo.

Vertigo and dizziness are very common symptoms in the general population. The aim of this paper is to describe the physical and pharmacological treatment of symptoms characterized by sudden onset of rotatory vertigo. Acute vertigo can be subdivided into two main groups: (1) spontaneous vertigo and (2) provoked vertigo, usually by postural changes, generally called paroxysmal positional vertigo (PPV). Sudden onset of acute vertigo is usually due to acute spontaneous unilateral vestibular failure. It can be also fluctuant as, e.g., in recurrent attacks of Ménière's disease. Pharmacotherapy of ac

Associations Between Cognitive Performance, Depression, Anxiety, and Stress in Patients With Chronic Vestibular Dysfunction: A Cross-Sectional Analysis.

BACKGROUND: The vestibular system is critical for sensorimotor function, balance control, and gaze stability. Recent research highlights its role in cognitive functions, including attention, memory, and visuospatial skills. The integration of this system is lateralized in the brain. These disorders represent a substantial burden on healthcare systems, and a negative impact on quality of life. OBJECTIVES: Previous research has shown that increased cognitive load can alter vestibular evoked myogenic potentials (VEMPs), suggesting that vestibular function is influenced by cognitive demands. Is Vestibular Meniere's Disease Associated With Endolymphatic Hydrops?

Background: Vestibular Meniere's disease (American Academy of Ophthalmology and Otolaryngology, 1972) also known as possible Meniere's disease (American Academy of Otolaryngology Head and Neck Surgery, 1995) or vestibular type of atypical Meniere's disease (V-AMD) (Japan Society for Equilibrium Research, 2017) is characterized by an episodic vertigo without hearing loss. Though named as Meniere's disease (MD), this entity may not be caused solely by endolymphatic hydrops (EH). Objective: To estimate the role of EH in vestibular Meniere's disease in comparison with definite Meniere's disease. M

Diagnostic challenges in internal carotid artery fenestration: a 78-year-old woman presenting with transient ischemic attack-a case report.

BACKGROUND: Internal carotid artery fenestration is a rare vascular anomaly that may present with transient ischemic attack symptoms due to localized flow disturbances. It is important to report such cases, as they present unique diagnostic challenges and clinical implications, particularly when associated with systemic vascular risk factors such as diabetes and hypertension. Differentiating internal carotid artery fenestration from other conditions, such as internal carotid artery dissection, is crucial for appropriate management and patient outcomes. CASE PRESENTATION: A 78-year-old Iranian
