A nerve-sparing radical hysterectomy: guidelines and feasibility in Western patients.

Surgical damage to the pelvic autonomic nerves during radical hysterectomy is thought to be responsible for considerable morbidity, i.e., impaired bladder function, defecation problems, and sexual dysfunction. Previous anatomical studies and detailed study of surgical techniques in various Japanese oncology centers demonstrated that the anatomy of the pelvic autonomic nerve plexus permits a systematic surgical approach to preserve these nerves during radical hysterectomy without compromising radicality. We introduced elements of the Japanese nerve-preserving techniques and carried out a feasib

[Reconstruction for brachial injury by the accessory nerve--anatomy of the accessory nerve and its innervation of the trapezius--Japan].

At present reconstruction of not only elbow function, but also wrist and fingers function is possible for totally paralysed root avulsion type of brachial plexus injuries by means of multiple nerve transfers and free muscle transplantation. Although denervation of the trapezius muscle may be a problem, the accessory nerve is used as the donor nerve for reconstruction. Forty-seven cadaver dissections were performed to determine the innervation of the trapezius. In 98% of the regions, branches of the accessory nerve or the cervical nerves were found to be directly innervated with the accessory n

Homeotic shift at the dawn of the turtle evolution.

All derived turtles are characterized by one of the strongest reductions of the dorsal elements among Amniota, and have only 10 dorsal and eight cervical vertebrae. I demonstrate that the Late Triassic turtles, which represent successive stages of the shell evolution, indicate that the shift of the boundary between the cervical and dorsal sections of the vertebral column occurred over the course of several million years after the formation of complete carapace. The more generalized reptilian formula of at most seven cervicals and at least 11 dorsals is thus plesiomorphic for Testudinata. The m

Mapping axillary sensory cutaneous nerves for enhanced analgesic approaches in axillary surgery: a cadaveric study.

BACKGROUND: Innervation of the breast includes branches of thoracic intercostal nerves, the superficial cervical plexus, the brachial plexus, and the intercostobrachial nerve (ICBN). Commonly used blocks for breast surgery provide incomplete analgesia of the axillary region. This cadaveric study aims to identify and map the axillary sensory cutaneous nerves. METHODS: We conducted nine axillary dissections on cadavers of both sexes with cadavers in supine position and upper limbs abducted. Incisions along the anterolateral thorax and superior clavicle created laterally reflecting skin flaps, a…

Pourfour Du Petit syndrome after interscalene block.

Interscalene block is commonly associated with reversible ipsilateral phrenic nerve block, recurrent laryngeal nerve block, and cervical sympathetic plexus block, presenting as Horner's syndrome. We report a very rare Pourfour Du Petit syndrome which has a clinical presentation opposite to that of Horner's syndrome in a 24-year-old male who was given interscalene block for open reduction and internal fixation of fracture upper third shaft of left humerus.

Anatomical predisposition and neurological vulnerability to brachial plexus birth injury: a contemporary narrative review.

Erb's palsy, or brachial plexus birth injury (BPBI), is an injury sustained at birth to a vulnerable nervous system which even with the best care leaves many children with permanent impairments, growth retardation and pain. Established risk factors include maternal diabetes, increased foetal weight, shoulder dystocia and instrumental delivery. Yet these variables do not reliably predict which individual children will sustain injury. We propose an additional, largely neglected factor: developmental anatomical predisposition of the neonatal brachial plexus, with anomalies similar to those implic

Thoracic outlet syndrome in the pediatric population: case series.

We present 4 patients, 4 months to 10 years of age, with thoracic outlet syndrome. All were referred to the brachial plexus clinic. Three patients were diagnosed with vascular thoracic outlet syndrome after clinical evaluation and diagnostic imaging. Three had a cervical rib and 1 had an anomalous first rib. All patients were treated surgically through a supraclavicular approach and had resolution of the symptoms. No postoperative complications were noted.

An anatomic and dynamic study of the greater occipital nerve (n. of Arnold). Applications to the treatment of Arnold's neuralgia.

This study concerns the posterior ramus of the second cervical spinal n., or greater occipital of Arnold. By means of dissections in formalin embalmed cadavers, an attempt was made to define its winding course and to locate it in relation to clinical or radiographic landmarks, so as to provide a guide for infiltration of the nerve with local anesthetic. At the same time a dynamic study was made to elucidate the relations of the nerve to adjacent structures during the different movements of the neck. This allowed us to propose clinical tests of nerve involvement and to reveal the zones where th

[A case of subacute necrotizing lymphadenitis complicated with brachial plexus neuritis].

A 22-year-old female noted a low grade fever and swelling of the cervical lymph nodes in May 1997, and later developed a dry cough. She was diagnosed to have interstitial pneumonitis, and then administration of corticosteroids alleviated her symptoms. On February 6, 1998, however, a high fever recurred and her swollen cervical lymph node on the right side was biopsied on February 9, 1998. A histological examination revealed an increased number of histiocytes and karyorrhexis of the lymphocytes in the paracortical areas, and she was therefore diagnosed to have histiocytic necrotizing lymphadeni

A comprehensive review with potential significance during skull base and neck operations, Part II: glossopharyngeal, vagus, accessory, and hypoglossal nerves and cervical spinal nerves 1-4.

Knowledge of the possible neural interconnections found between the lower cranial and upper cervical nerves may prove useful to surgeons who operate on the skull base and upper neck regions in order to avoid inadvertent traction or transection. We review the literature regarding the anatomy, function, and clinical implications of the complex neural networks formed by interconnections between the lower cranial and upper cervical nerves. A review of germane anatomic and clinical literature was performed. The review is organized into two parts. Part I discusses the anastomoses between the trigemi

Simultaneous MR neurography and apparent T2 mapping of cervical nerve roots before microendoscopic surgery to treat patient with radiculopathy due to cervical disc herniation: Preliminary results.

There is no imaging modality to quantitatively evaluate compressed cervical nerve roots in cervical radiculopathy. Here we sought to evaluate the usefulness of simultaneous apparent T2 mapping and neurography with nerve-sheath signal increased with inked rest-tissue rapid acquisition of relaxation-enhancement imaging (SHINKEI-Quant) to evaluate compressed nerves quantitatively in patients with cervical radiculopathy due to cervical disc hernia before microendoscopic surgery. Visualization of cervical nerve roots and their distal nerve fibers by diffusion-weighted scanning using parallel imaging.

PURPOSE: To report a technique developed for visualizing cervical nerve roots and distal nerve fibers using diffusion-weighted magnetic resonance imaging employing parallel imaging. MATERIAL AND METHODS: We performed maximum intensity projection for a stack of isotropic axial diffusion-weighted images obtained with parallel imaging applying a motion-probing gradient in six directions with a b-value of 500 s/mm2 in a preliminary series of 13 subjects. RESULTS: This method worked well for visualizing the spinal cord and most of the nerve roots, the dorsal root ganglia, and proximal peripheral

The pharyngovertebral veins: an anatomical rationale for Grisel's syndrome.

UNLABELLED: Non-traumatic subluxation of the atlanto-axial joint following peripharyngeal inflammation (Grisel's syndrome) has been attributed to laxity of the transverse ligament caused by hyperemia, but a satisfactory anatomical rationale for the association of these conditions has been lacking. Injection of the cervical epidural sinuses of a series of human perinatal cadavera showed retrograde filling of a previously undescribed system of veins with frequent lymphovenous anastomoses. This system appears to drain the posterosuperior pharyngeal region. CLINICAL RELEVANCE: Because of their di

Reconstruction of complex defects of the extracranial facial nerve: technique of "the trifurcation approach".

PURPOSE: Reconstruction of complex defects of facial nerve (FN) after extensive cancer surgery requires individualized solutions. We describe the trifurcation technique as a modification of the combined approach on example of two patients with locally advanced parotid cancer. METHODS: Due to perineural invasion, extensive resection of the FN from the mastoid segment to the peripheral branches was required. For reanimation of the upper face, a complex cervical plexus graft was sutured end-to-end to the mastoid segment of the FN trunk. The branches of the graft enabled reanimation of three peri…

Neurophysiological identification of position-induced neurologic injury during anterior cervical spine surgery.

This study was a retrospective review of 3,806 patients who underwent anterior cervical spine surgery with multi-modality neurophysiological monitoring consisting of transcranial electric motor evoked potentials, somatosensory evoked potentials and spontaneous electromyography between 1999-2003. The objectives of this study were twofold: (1) to evaluate the role of transcranial electric motor evoked potential tceMEP and ulnar nerve somatosensory evoked potential (SSEP) monitoring for identifying impending position-related stretch brachial plexopathy, peripheral nerve entrapment/compression or

[Diaphragmatic paralysis caused by a phrenic nerve lesion secondary to internal jugular vein cutdown].

We discuss the case of a premature baby weighing 1,200 g who developed right diaphragmatic paralysis when a venous cutdown was performed on his internal jugular vein. Acquired unilateral diaphragmatic paralysis has been associated with obstetric trauma of the cervical plexus and surgical procedures of the neck and thorax, when the phrenic nerve is injured. We suggest fluoroscopy as the best diagnostic method; early diaphragmatic plication reduces the morbimortality of these patients.

Extremity shortness in obstetric brachial plexus lesion and its relationship to root avulsion.

A total of 73 patients with obstetric brachial plexus palsy and extremity shortness were evaluated clinically, electrophysiologically, and with cervical magnetic resonance imaging. Patients were separated into groups according to age and the level of lesion. The differences of the length of the humerus, ulna, radius, and the second and fifth metacarpal bones were significant between the involved and uninvolved extremities. The difference in shortness increased in relation to the age of the groups and stabilized to approximately 10% in the groups aged 4 to 8 years and 8+ years. A significant re
