Bilateral aniridia with Marfan's syndrome and dental anomalies--a new association.

A unique case of bilateral aniridia, partial anodontia, bilateral cervical ribs and Marfan's syndrome is presented. This is the first report in the literature of the established association of all these four entities in the same patient. The etiological possibilities and clinical significance are discussed.

Congenital pseudoarthrosis of the clavicle with bifurcation.

Congenital pseudoarthrosis of clavicle is a rare clinical entity. It usually presents as a swelling in the clavicular region at birth or soon after birth. Fitzwilliam's original description of 60 subtypes of congenital pseudoarthrosis of clavicle have addressed several anatomical variants, e.g. association with cervical rib and abnormally vertical and elevated upper ribs. However, congenital pseudoarthrosis of clavicle associated with bifurcation is an atypical anatomic variant. To the best of our knowledge, this variant has never been mentioned in the literature. In the present report, we hav…

Left-sided congenital pseudarthrosis of the clavicula.

This a well-documented case of a 13-year-old girl with unilateral pseudarthrosis of the left clavicle without the presence of cervical ribs or dextrocardia. Magnetic resonance imaging could not detect any abnormality that could be related to the left-sided pseudarthrosis.

Achondroplasia: pre- and postsurgical considerations for midface advancement.

Achondroplasia, the most common type of dwarfism, involves several physical characteristics that can cause pre- and intraoperative respiratory problems. The characteristics include rib-cage deformities, midface hypoplasia, choanal stenosis, muscular hypotonia, foramen magnum compression, and cervical abnormalities. Specific pre- and postoperative strategies for dealing with achondroplastic dwarfs are discussed.

[Familial anomaly of the seventh cervical vertebra. Radio-clinic comparison in a fourteen member family].

We report a fourteen member family presenting with an anomaly of the seventh cervical vertebra. The symptoms associate pain and paresthesias, a C8-D1 syndrome or arterial manifestations. The radiologic lesions noticed are bilateral in twelve cases, either cervical ribs or apophysomegaly of the seventh vertebra's transverse process. There is no correlation between the severity of the clinical impairment and the importance of the radiologic image. We insist on the familial character rarely reported in the literature of an anomaly noticed in radiologic practice.

Arterial thoracic outlet syndrome in an adolescent.

Arterial thoracic outlet syndrome (aTOS) is rarely diagnosed in children. A 15-year-old boy with recent-onset claudication was found to have aTOS on investigation. Bony abnormality other than the cervical rib might also cause aTOS. Enhanced awareness of aTOS as a cause of vascular symptoms in children is warranted.

Etiology and pathology.

Until the 1920s, TOS was believed to be a vascular condition caused by compression of the subclavian artery by a congenital anomaly, either a cervical rib or tight anterior scalen muscle. Today it is regarded primarily as a neurologic condition caused by neck trauma injuring and scarring the scalene muscles.

[Update peripheral regional anesthesia : Rib, clavicle and shoulder dislocation].

This CME article describes the current developments in peripheral regional anesthesia for shoulder dislocation, clavicle fractures and (serial) rib fractures. Ultrasound-guided regional anesthesia techniques are increasingly replacing analgesic sedation or general anesthesia for shoulder reduction, combining pain relief and muscle relaxation with greater safety. In clavicle fractures, the midclavicular block provides targeted analgesia without motor impairment as an alternative to established combined block of the brachial plexus and cervical plexus. For serial rib fractures, fascial plane blo

A severe case of vascular thoracic outlet syndrome.

We report a 21-year-old patient who presented at the outpatient department of angiology with incipient necroses in the fingertips of the right hand. Colour-coded duplex sonography and angiography demonstrated occlusion of the right brachial, ulnar and radial arteries, obviously resulting from an embolism from the subclavian artery narrowed by a cervical rib. After partly successful local thrombolysis this accessory rib was resected. We review angiologic signs and neurological symptoms of the thoracic outlet syndrome (TOS) and analyse the current literature regarding diagnostic procedures and t…

First rib resection in thoracic outlet syndrome.

Most patients with thoracic outlet syndrome (TOS) present with exercise-induced upper extremity paresthesia. Neurogenic TOS is the most common type where the brachial nerve plexus is compressed against a tight thoracic outlet. Vascular compromise although rare can result from thoracic outlet pressure against the subclavian artery or more commonly the subclavian vein. This article reviews the pathophysiology of TOS and describes several effective surgical interventions. Complete first rib resection with surgical decompression is an essential part of the treatment for TOS. First rib resection vi

[Neurologic thoracic outlet syndrome in children: about a case].

Thoracic Outlet Syndrome (NTOS) encloses the whole set of clinical manifestations associated with the compression of the trunks of the brachial plexus and/or of the subclavian vessels as they cross the thoracic outlet (cervico-thoraco-brachial junction). It rarely affects children. We here report the case of a young girl treated for neurologic NTOS caused by supernumerary cervical rib.

Klippel-Feil syndrome with other associated anomalies in a medieval Portuguese skeleton (13th-15th century).

Klippel-Feil syndrome, or synostosis of the cervical spine, is the result of an abnormal division of somites during embryonic development. This report analyses an adult male (exhumed from a Portuguese graveyard dating from the 13th to the 15th century) with malformations in the cranium and vertebral column. Besides the lesions that are typical of Klippel-Feil syndrome type II, other defects usually linked to this pathology are described (occipito-atlantal fusion, hemivertebrae, butterfly vertebrae, cervical rib, changes in normal number of vertebral segments and a possible Sprengel deformity).

Occipital encephalocele and MURCS association: case report and review of central nervous system anomalies in MURCS patients.

The combination of MURCS association (Müllerian duct and renal agenesis, upper limb and rib anomalies) and occipital encephalocele occurred in a stillborn girl of 41 weeks gestation. The malformations are compatible with a defect in the organization of the paraxial mesoderm that gives rise to occipital, cervical, and thoracic somites and adjoining intermediate mesoderm. These structures contribute to the occipital bone, cervical spine, upper limbs, and urogenital system. Brain imaging may be useful in assessing MURCS patients, if cranial malformations prove to be clinically important in these
