Performative Gratitude - A Barrier to Understanding the Needs of Older Migrants Experiencing Homelessness in Canada.

BACKGROUND: Canada's aging population faces a growing homelessness crisis, affecting groups such as older migrants. Older migrants experiencing homelessness may encounter intersecting forms of housing precarity, exclusion, and marginalization that shape their relationships with the host country and service systems. OBJECTIVE: Using a postcolonial lens, this study explores how older migrants experiencing homelessness navigate housing precarity and relate to the host country, with particular attention to the role of gratitude in their experiences of housing and support. METHODS: The study draw

Pre-hospital blood transfusion in non-traumatic major haemorrhage: a retrospective observational study.

BACKGROUND: Blood component administration for major traumatic haemorrhage is established clinical practice in pre-hospital emergency medicine. However, transfusion practices for non-traumatic haemorrhage lack specific guidance. We analysed historical data on non-traumatic major haemorrhage to describe the incidence and current blood product administration practices. METHOD: This retrospective observational cohort study reviewed Air Ambulance Charity Kent Surrey Sussex patient data from 11 November 2013 to 9 August 2024. The cohort included patients receiving pre-hospital blood components for

Pain experience of people with inflammatory bowel disease: a qualitative study.

INTRODUCTION: People with inflammatory bowel disease (IBD) commonly experience pain, whether during active disease or remission, which interferes with daily life and major goals and causes distress. Current psychological methods of pain management draw from musculoskeletal pain interventions, but it has not been established that the musculoskeletal model is a good fit. We aimed to outline a psychological model of IBD pain. METHODS: We used qualitative methods: a very open interview (Grid Elaboration Method), conducted online and transcripts analysed for themes and subthemes. 15 men and 15 wom

A Study of Comorbidities Among Epilepsy Patients in Selected Hospitals of Khartoum State, Sudan.

BACKGROUND: Comorbid conditions in epileptic patients depend on many factors like etiology, type of epileptic syndromes, social and environmental factors, which may differ from one region to another, which encourages us to do this study in Sudan, which is one of the low-income countries. Limited information is available regarding this in our region. METHODS: This is a hospital-based, prospective, descriptive, and cross-sectional study aimed at determining the common medical, neurological, psychological, and cognitive comorbidities. Data were obtained by interviewing 115 epileptic patients att

Supporting continuing bonds for parents with infants with uncertain futures on neonatal units in the United Kingdom: co-designing a culturally sensitive music therapy intervention.

INTRODUCTION: Neonatal intensive care is a traumatic environment for parents, infants and staff. Services need to collaborate with their users to develop acceptable and sustainable support. Music therapy can have positive impacts on physiological and psychological outcomes for infants and has the potential to help create a supportive service environment. This study aimed to co-design a culturally sensitive music therapy intervention to support the development of continuing bonds for parents with infants with uncertain futures on neonatal units. METHODS: An Experience-based Co-design approach

Health inequality in medieval Cambridge, 1200-1500 CE.

Health inequality is not only a major problem today; it left its mark upon past societies too. For much of the past, health inequality has been poorly studied, mostly because bioarchaeologists have concentrated upon single sites rather than a broader social landscape. This article compares 476 adults in multiple locations of medieval Cambridge (UK). Samples include ordinary townspeople (All Saints), people living in a charitable institution (the Hospital of St. John), and members of a religious order (the Augustinian Friary). These groups shared many conditions of life, such as a similar range

The Burden of Care of Nasoalveolar Molding: An Institutional Experience.

Nasoalveolar molding (NAM) is an early presurgical intervention to facilitate primary cleft lip repair by reducing cleft severity and improving labial and nasal form. However, it continues to be associated with the burden of care that influences access and completion of therapy. The authors, therefore, aim to determine the burden of care of NAM therapy for families seeking treatment at a high-volume urban cleft center. A retrospective study of all patients undergoing primary cleft repair between 2012 and 2020 was performed. Patients were grouped based on whether or not NAM therapy was offered.

Medical management of painful achalasia: a patient-driven systematic review.

Achalasia is a rare esophageal disorder characterized by abnormal esophageal motility and swallowing difficulties. Pain and/or spasms often persist or recur despite effective relief of the obstruction. A survey by UK charity 'Achalasia Action' highlighted treatments for achalasia pain/spasms as a key research priority. In this patient-requested systematic review, we assessed the existing literature on pharmacological therapies for painful achalasia. A systematic review of the literature using Medline, Embase and Cochrane databases was performed to identify studies evaluating pharmacological th

From morphology of human body to reshaping a morphology of the society: heritage in the field of pathology of infectious diseases of Wilhelm Ebstein and work of councilman Sigismund Asch. A story how to counteract epidemics and to transform feudal system into civil democracy.

Collected, primary resources enabled us to extract data that are scarcely present in medical literature of the two Breslauer morphologists of both the human body and - metaphorically - the society: Wilhelm Ebstein (1836-1912) and Sigismund Asch (1825-1901), particularly the latter, who described morphology of melanosis in his doctoral dissertation in 1846, to switch on reshaping social morphology of Wrocław (Breslau) in Virchow-like manner. In contrast to the main perspective of Ebstein's anomaly that has been finely described in past biographical papers, a primary aspect of infectious disease

Evaluation of the prehospital use of a Valsalva assist device in the emergency treatment of supraventricular tachycardia (EVADE SVT): study protocol for a stepped wedge cluster randomised controlled trial.

INTRODUCTION: Patients with episodes of supraventricular tachycardia (SVT), a common heart arrhythmia, are often attended by ambulance services. International guidelines advocate treatment with the Valsalva manoeuvre (VM), but this simple physical treatment has a low success rate, with most patients requiring conveyance to hospital. The Valsalva Assist Device (VAD) is a simple device that might help practitioners and patients perform a more effective VM and reduce the need for patients to be taken to hospital. METHODS AND ANALYSIS: This stepped wedge cluster randomised controlled trial, condu

Opportunistic Non-Governmental Organisation Delivery of a Virtual Stop Smoking Service in England during the COVID-19 Lockdown.

Smoking cessation services have rapidly transformed during the COVID-19 pandemic. Changes include pivoting from face-to-face to telephone and video call support, remote provision of stop smoking aids and more flexible appointments. This study reports an evaluation of a charity-led smoking cessation service rapidly conceived and launched in this context. The pilot service accepted self-referrals in Yorkshire, England from 20 May 2020 to 5 June 2020. A dedicated smoking cessation practitioner provided 12 weeks of weekly behavioural support over telephone or video call. NRT and/or medication and/

Exposed and Abandoned. Origins of the Foundling Hospital.

Abandoning undesired newborn infants was a Roman form of family limitation. They were exposed or given to foster mothers. Christianization alleviated their lot when in 374 CE, Emperor Valentinian's law provided some protection. The Milan Foundling Hospital was established in 787 CE. When the Carolingian Empire fell apart during the 10th century, monastic networks (the Holy Spirit Order and Daughters of Charity) took over social support for the poor, the sick, and the insane. Foundling hospitals proliferated in Italy between the 13th and 15th centuries, in France during the 16th and 17th, and i

Association of Toll-like receptor 7 variants with life-threatening COVID-19 disease in males: findings from a nested case-control study.

BACKGROUND: Recently, loss-of-function variants in TLR7 were identified in two families in which COVID-19 segregates like an X-linked recessive disorder environmentally conditioned by SARS-CoV-2. We investigated whether the two families represent the tip of the iceberg of a subset of COVID-19 male patients. METHODS: This is a nested case-control study in which we compared male participants with extreme phenotype selected from the Italian GEN-COVID cohort of SARS-CoV-2-infected participants (<60 y, 79 severe cases versus 77 control cases). We applied the LASSO Logistic Regression analysis, con

Anxiety and Depression in Parents of Children Born with Esophageal Atresia: An International Online Survey Study.

PURPOSE: Caring for a chronically ill child can be stressful and requires additional care from parents. Parental mental health and stress may impact both parental and child wellbeing, leading to maladaptive parenting practices, and interference with illness management. The aim of the study was to explore the levels of anxiety and depression in parents of children born with esophageal atresia (EA) and to investigate whether parental anxiety and depression were associated with child's medical characteristics or with parental factors. DESIGN AND METHODS: An international online survey was develo

A thematic analysis exploring the psychological well-being of adults born with esophageal atresia.

BACKGROUND: Living with a rare and chronic health condition can have a significant impact on psychological well-being and mental health. There is a growing understanding that Esophageal Atresia (EA), a rare birth defect often accompanied by a Trachea-Esophageal Fistula (TEF), is a complex health condition that requires lifelong medical attention beyond pediatric care into adulthood. Given the reciprocal relationship between one's physical and psychological well-being, the aim of this study was to develop a better understanding of the mental health of adults born with EA/TEF. METHODS: An inter

Does Insurance Status Affect Access to Care Among Ankle Fracture Patients? An Institutional Retrospective Study.

Delayed access to care for patients with ankle fractures may increase risk of complications, particularly if surgical management is warranted. Medicaid is a state and federal insurance program in place for those with low income, which has previously been associated with delayed access to care among patients with ACL tears and total hip arthroplasties. The purpose of this study is to assess whether patient insurance status affects access to care for ankle fracture patients, using data from a single institution. A retrospective cohort study (N = 311 patients)was performed on individuals that und

Do safety-net hospitals provide equitable care after decompressive surgery for acute cauda equina syndrome?

INTRODUCTION: Safety-net hospitals provide care to a substantial share of disadvantaged patient populations. Whether disparities exist between safety-net hospitals and their counterparts in performing emergent neurosurgical procedures has not yet been examined. OBJECTIVE: We used the Nationwide Inpatient Sample (NIS), a national all-payer inpatient healthcare database, to determine whether safety-net hospitals provide equitable care after decompressive surgery for acute cauda equina syndrome (CES). METHODS: The NIS from 2002 to 2011 was queried for patients with a diagnosis of acute CES who
