Prospective comparison of viscoelastic coagulation monitor (VCM) and rotational thromboelastometry (ROTEM) in the assessment of cirrhosis coagulopathy.

BACKGROUND: Rotational thromboelastometry (ROTEM) provides comprehensive hemostasis assessment in cirrhosis but relies on exogenous activators, potentially limiting its reflection of in vivo conditions. The Viscoelastic Coagulation Monitor (VCM) is a new, activator-free device to assess whole-blood coagulation dynamics. OBJECTIVES: To investigate the concordance between VCM and ROTEM and to explore their association with clinical outcomes in cirrhosis. METHODS: We prospectively included 110 patients with cirrhosis admitted to our service. VCM and ROTEM were performed at hospitalization. All

Trends in Homicide Hospitalization and Mortality in Taiwan, 1998-2015.

In Taiwan, the national research on homicide is rare, mostly discussing the issue of child abuse. We sought to better understand the characteristics and risk factors of homicide through a retrospective cohort study from 1998 to 2015. "Child battering and other maltreatment" ranked first for the 0-4 age group and second for the 5-14 age group. The hospital mortality was 511 deaths. We found that the 25-44 age group had the highest risk and accounted for 44.76% of hospitalization. The most common causes were "fight, brawl, and rape" (49.12%), "cutting and piercing instruments," (13.16%) and "chi

Clinical and EEG characteristics analysis of autoimmune encephalitis in children with positive and negative anti-N-methyl- D-aspartate receptor antibodies.

BACKGROUND: Autoimmune encephalitis is complex and varied, but it is a curable disease. However, the diagnosis and treatment of children with Autoimmune encephalitis remains challenging. Therefore, we conducted this study to analyze the clinical features, electroencephalogram (EEG) characteristics, treatment and prognosis of autoimmune encephalitis in children with negative and positive anti-N-methyl-D-aspartate receptor (NMDAR) antibody. METHODS: From January 2015 to January 2017, 28 child patients with autoimmune encephalitis were hospitalized in the Neural Ward of the Children's Medical Ce

[Pediatric burkitt lymphoma primarily involving the adenoids: a case report and literature review].

Burkitt Lymphoma(BL) is a highly aggressive B-cell non-Hodgkin lymphoma characterized by extremely rapid proliferation and specific genetic alterations, representing the fastest-growing human malignancy. Early detection is associated with high treatment success rates, while advanced stages carry a poor prognosis. Our department admitted a pediatric patient with BL primarily originating in the adenoids. The child was initially hospitalized for adenoid hypertrophy and underwent surgical resection, with routine postoperative pathology showing no significant abnormalities. Three weeks postoperativ

[Consequences of exclusive breast-feeding in vegan mother newborn--case report].

We report on the case of an infant who was hospitalized because of failure to thrive, megaloblastic anemia, and delayed psychomotor development. He was 10 months old and had been exclusively breast-fed by his vegan mother. Investigations showed vitamin B(12) deficiency with hematocytopenia and pervasive developmental disorders as well as vitamin K and vitamin D deficiencies. The infant's mother presented the same deficiencies. Introduction of vitamin supplementation normalized the biological disorders, and the infant showed weight gain and neurological improvement. This case highlights that a

Infant botulism: case report and clinical update.

Infant botulism occurs in infants between 1 week and 11 months of age and results from the in vivo production of neurotoxin by Clostridium botulinum. The clinical spectrum ranges from asymptomatic carriers, through various degrees of paralysis, to sudden death. The classic clinical presentation is an afebrile child with constipation and generalized weakness manifested by poor head control, poor suck, and weak cry. Symptoms can progress to include cranial nerve palsies, respiratory arrest, and adynamic ileus. Treatment is supportive in an intensive care setting. Antibiotics and antitoxin are no

Rotavirus gastroenteritis in infants and young children.

The electronmicroscopic examination of stool samples from 18 infants and young children with gastroenteritis, hospitalized at the Clinic of Infectious Diseases in Prague, was carried out. In ten children rotavirus was found in the faeces and the bacteriological findings were negative. Rotavirus particles were aggregated by convalescent child sera and by normal human gamma globulin. The clinical picture was characterized by the sudden onset of vomiting and fever, in one case with febrile convulsions. Diarrhoea was watery and yellow-green, and usually persisted for four to five days. The conditi

Nephronophthisis and tapetoretinal degeneration associated with liver fibrosis.

A 12 year-old boy was referred because of general weakness, enuresis and pallor which had been present for at least six months. Previously, the child had been hospitalized at the age of five, because of mental retardation and hepatosplenomegaly, for which no cause could be found. He had severe renal insufficiency, with all the hallmarks of nephronophthisis. In addition his vision was very poor and fundoscopy revealed tapetoretinal degeneration. The liver and spleen were grossly enlarged. Liver function was almost completely normal, but histology showed diffuse periportal febrosis with profifer

Keeping Up With School During Hospitalization for Children With Chronic Illnesses, Siblings and Parents.

BACKGROUND: Children who have chronic illnesses (CIs) and their siblings often miss school when the child with a CI is hospitalized. Understanding the perspectives of the child with an illness, siblings and parents will provide information about how they keep up with schoolwork, their perceptions of not being in school, or of homeschooling that will inform educators, families and medical professionals. METHODS: English-speaking children diagnosed with CI, siblings and parents participated. They were residing at a Ronald McDonald House. A qualitative study using interviews with children with C

A life-threatening presentation of child physical abuse: jejunal perforation.

Intra-abdominal injuries from impacts are the second most common cause of death in battered children. However, it may be difficult to distinguish between accidental abdominal injury and abuse, especially in the absence of other clinical findings. Published reports are also limited about the diagnosis of abuse in children with intra-abdominal injury. We report a case with jejunal perforation, multiple soft tissue injuries, and occipital fracture secondary to child abuse who was initially admitted to our hospital with complaint of fever, cough, and vomiting. An exploratory laparotomy revealed pe

Bereaved parents' perspectives of factors influencing decision-making about place of end-of-life care for children with life-limiting, life-threatening conditions: an all-Ireland qualitative study.

BACKGROUND: The need for children's palliative care and end-of-life services is increasing. However, there is limited evidence to inform health and social care professionals about parental preferences for place of end-of-life care for their child, or on factors influencing families in deciding on their preferred place of death for their child. The aim of this study was to explore factors which influence parental decision-making in relation to place of end-of-life care for children with life-limiting, life-threatening conditions, with respect to experiences of end-of-life care in different sett

[Early necrectomy, homo- and autotransplantation of skin in a 19-month-old child with deep burns over 45% of the body].

The authors present a case of a 19 months old baby, twin, with deep burns (45% of the skin), caused by boiling beans. The child was transported to the hospital immediately after the injury. Following the successful anti-shock therapy (whose scheme has been given), the child was operated upon on the VI day: early necrectomy with homotransplantation of his father's skin. In the second stage an autotransplantation (Thiersch) has been performed. The transitory complications were a melaenå appearing on the VII day, and later a contracture of the elbow. Finally, the child left the hospital with no K

The role of cranial MRI in identifying patients suffering from child abuse and presenting with unexplained neurological findings.

OBJECTIVE: The aim of this study was to demonstrate the usefulness of cerebral MRI to detect possible child abuse in children with unexplained neurologic findings. METHOD: Between 1990 and 1997, 208 children were referred for suspected physical child abuse to the Child Protection Clinic of Ste-Justine Hospital, a tertiary care pediatric hospital. Among them, 39 children presented initially with neurological findings. For 27 of them, the CT Scan results prompted the diagnosis of child abuse. However, in 12 children, even if a CT-Scan was performed, the diagnosis and/or the mechanisms of the ne

Serum complement levels before and after the onset of acute post-streptococcal glomerulonephritis. A case report.

Low serum C3, properdin, and C5 levels found in the acute stage of acute post-streptococcal glomerulonephritis (APSGN) indicate the presence of aggressive complement activation. We followed serum complement component levels in a child hospitalized with erysipelas who developed APSGN on the 2nd hospital day. Her initial serum sample, obtained prior to the clinical onset of nephritis, had a low properdin level and normal C3 and C5 levels despite the presence of C3 splitting activity. Two days later she developed gross hematuria and subsequent sera contained low C3, properdin, and C5 levels, as i

Shigella dysenteriae type 1--Guatemala, 1991.

On March 14, 1991, physicians at a hospital in Guatemala City reported to the Institute of Nutrition of Central America and Panama (INCAP) that a 2-year-old boy living in an orphanage in Guatemala City had been hospitalized with dysentery; stool cultures yielded Shigella dysenteriae type 1. Another child from the orphanage had recently died from dysentery. During March 18-21, two other young children from the orphanage were diagnosed with S. dysenteriae type 1. On March 21, health officials in Rabinal, in the department of Baja Verapaz, reported more than 100 cases of dysentery to the Division

Fear of clowns in hospitalized children: prospective experience.

UNLABELLED: Medical clowns (MC) have become an integral part of the pediatric staff of hospital wards. While several studies have demonstrated the huge benefits of MC, there are almost no data regarding fear of clowns, a known phenomenon that means an irrational fear of clowns. In the current study, we sought to examine the prevalence of fear of clowns in pediatrics wards, and to characterize the affected children. The clinical work of three certified MCs was prospectively assessed. Every child with fear of clowns was noted, data were retrieved from the medical records, and the parents/child c

Burden and spectrum of bacterial infections among sickle cell disease children living in Cameroon.

BACKGROUND: Although sickle cell disease (SCD) children are highly susceptible to bacterial infections (BIs), there is a dreadful lack of data related to the burden and spectrum of BIs in sub-Saharan Africa (SSA), the highest affected region with SCD. This study aimed to determine the burden and spectrum of BIs among SCD children hospitalized in a pediatric reference hospital in Cameroon, a SSA country. METHODS: We conducted a retrospective analysis of records of children hospitalized from November 2012 to August 2015 in the SCD unit of the Mother and Child Centre of the Chantal Biya Foundati

Prevalence of retinal hemorrhages in pediatric patients after in-hospital cardiopulmonary resuscitation: a prospective study.

OBJECTIVE: Child abuse occurs in 1% of children in the United States every year; 10% of the traumatic injuries suffered by children under 5 years old are nonaccidental, and 5% to 20% of these nonaccidental injuries are lethal. Rapid characterization of the injury as nonaccidental is of considerable benefit to child protection workers and police investigators seeking to safeguard the child care environment and apprehend and prosecute those who have committed the crime of child abuse. Physically abused children present with a variety of well-described injuries that are usually easily identifiabl
