Novel use of dexmedetomidine for the treatment of anticholinergic toxidrome.

INTRODUCTION: We report the case of an adolescent with anticholinergic toxidrome from diphenhydramine overdose, whose symptoms were treated with a novel application of dexmedetomidine. CASE REPORT: A 13-year-old female developed an anticholinergic toxidrome after intentionally ingesting 9.5 mg/kg of diphenhydramine. Despite routine supportive therapies, to include appropriate doses of lorazepam, she continued to have significant agitation, psychosis, and hallucinations. A dexmedetomidine infusion was started to aid in the treatment of her agitation and psychosis with marked improvement of her

Dementia and lower urinary dysfunction: with a reference to anticholinergic use in elderly population.

Urinary incontinence is common in patients with dementia, and is more prevalent in demented than in non-demented older individuals. Neurogenic incontinence is common in multiple cerebral infarction and dementia with Lewy bodies, and in both diseases walking difficulty and falls are common. In contrast, functional incontinence is common in Alzheimer's disease due to cognitive disability and decreased motivation. Central cholinergic stimulation is the mainstay in the treatment of cognitive decline. In contrast, to date, the use of anticholinergic medications for detrusor overactivity in the elde

Extended-release oxybutynin.

Extended-release oxybutynin (Ditropan XL) uses an osmotic system (OROS) to deliver a controlled amount of oxybutynin chloride into the gastrointestinal tract over a 24-hour period when taken once daily. Oxybutynin binds to M3 muscarinic receptors on the detrusor muscle of the bladder, preventing acetylcholinergic activation and relaxing the muscle. Mean peak plasma concentrations are lower with extended-release oxybutynin 15mg once daily than with conventional immediate-release oxybutynin 5mg taken 3 times daily. Relative bioavailabilities of parent drug and metabolite N-desethoxybutynin are 1

[Clinical and therapeutical features in 135 patients with dystonia: experience of movement disorders unity of the Hospital de Clínicas of the Universidade Federal do Paraná].

This study aims to describe the clinical patterns and therapeutic responses in 135 patients with dystonia. According to the classification, 54% were focal; 17.8% were segmental; 8.1% hemidistonia; 18.6% generalized and 1.5% were multifocal. There was a positive familial history in 5.9% of the cases. The treatment of the idiopathic dystonias is divided in: specific and symptomatic, and it can be local with botulinum toxin, or systemic with oral drugs. The most common drugs used in the treatment were anticholinergics and benzodiazepines, with poor responses in the generalized forms. Botulinum to

Noradrenergic and cholinergic modulations of corticocerebellar activity modify the gain of vestibulospinal reflexes.

In addition to mossy fibers and climbing fibers, the cerebellar cortex receives noradrenergic and cholinergic afferents. Since the Purkinje (P) cells of the cerebellar vermis (culmen) respond to roll tilt of the animal with a discharge pattern that is out of phase with respect to that of the related lateral vestibular neurons, thus exerting a facilitatory influence on the gain of the vestibulospinal (VS) reflex, we tested the effects of local microinjection into the anterior vermis of noradrenergic and cholinergic agents on these reflexes. In decerebrate cats, unilateral microinjection in the

Ectopic alpha-adrenergic mediated accumulation of guanosine 3',5'-monophosphate in isolated adrenocortical carcinoma cells.

In the normal adrenal cell, epinephrine does not activate the rise of either cGMP or cAMP concentrations. In contrast, epinephrine activates the rise of cGMP but not cAMP concentrations in a concentration-dependent manner in isolated adrenocortical carcinoma cells. This effect was duplicated by the alpha-adrenergic agonist, phenylepherine, but was unaffected by the beta-adrenergic agonist, isoproterenol. The epinephrine-activated increase in cGMP was blocked by the alpha-adrenergic antagonist, phentolamine, but was not interfered with by the beta-adrenergic antagonist, propranolol. Neither ace

Pharmacist-driven interventions to de-escalate urinary antimuscarinics in the Programs of All-Inclusive Care for the Elderly.

BACKGROUND: Given associations with serious cognitive and physical adverse effects (e.g., dementia, falls), strong anticholinergics, like urinary antimuscarinics (UAMs), should be avoided in older adults. This feasibility study aimed to (1) evaluate the implementation rate of pharmacists' recommendations intended to de-escalate UAMs, (2) quantify the change in overall anticholinergic dosing exposure from these recommendations, and (3) investigate factors that predict recommendation implementation. METHODS: This was a retrospective, observational, before-and-after study. Pharmacists (n = 18) d

Acetylcholine exerts inhibitory and excitatory actions on mouse ileal pacemaker activity: role of muscarinic versus nicotinic receptors.

The effect of acetylcholine (ACh) on pacemaking and spontaneous contractions in the gastrointestinal tract is not well characterized. The current study aims to profile the effect of several muscarinic and nicotinic receptor agonists and antagonists on pacemaker potentials in the ICR mouse ileum. Pacemaker potentials of whole thickness mouse ileal segments were recorded extracellularly using a 60-channel microelectrode array (MEA) platform. A spatiotemporal analysis integrated the frequency, amplitude, and velocity measurements of pacemaker currents. Comparative data were obtained by recording

Non-hormonal systemic medications and dry eye.

Many drugs used for chronic illnesses can contribute to dry eye syndrome, and elderly patients who have dry eye may concurrently be on systemic medications that worsen the condition. Such medications include anticholinergic drugs, eg, antidepressant, antipsychotic, anti-Parkinson's disease, and antihistamine drugs. Other drugs such as anti-acne preparations and antihypertensives can also cause dry eye. In some cases, the adverse effects of the drug on dry eye is dose-related and can be relieved by reducing the dosage. Alternatively, a different drug within the same drug family may alleviate th

Anticholinergic agents for the treatment of "death rattle" in patients with myasthenia gravis.

Difficulty clearing upper airway secretions (death rattle) is a frequent problem at the end of life. Treatment often includes the use of anticholinergic drugs. Myasthenia gravis is a disease characterized by muscle weakness and fatigue caused by an immune-mediated deficiency of acetylcholine receptors at the neuromuscular junction, and it is treated with anticholinesterase agents. We report the case of a patient dying of myasthenia gravis who had problems with the "death rattle" and who presented a dilemma as to whether the use of anticholinergics would be helpful or would cause deterioration

The control of oral secretions in bulbar ALS/MND.

For the bulbar ALS/MND patient with dysphagia, the accumulation of oral secretions is a distressing problem. The control of these secretions, particularly the thick mucus form, is very difficult. In approaching this problem it is important to realise that the source of these secretions is not just from the oral salivary glands. The nose and lungs also contribute a constant flow of serous and particularly mucoid fluids. The neural control of these different forms of secretion is of importance in controlling them. Stimulation of cholinergic receptors produces thin serous secretions whereas beta

The alpha9 nicotinic acetylcholine receptor shares pharmacological properties with type A gamma-aminobutyric acid, glycine, and type 3 serotonin receptors.

In the present study, we provide evidence that the alpha9 nicotinic acetylcholine receptor (nAChR) shares pharmacological properties with members of the Cys-loop family of receptors. Thus, the type A gamma-aminobutyric acid receptor antagonist bicuculline, the glycinergic antagonist strychnine, and the type 3 serotonin receptor antagonist ICS-205,930 block ACh-evoked currents in alpha9-injected Xenopus laevis oocytes with the following rank order of potency: strychnine > ICS-205,930 > bicuculline. Block by antagonists was reflected in an increase in the acetylcholine (ACh) EC50 value, with no

Cisapride and 5-hydroxytryptamine enhance motility in the canine antrum via separate pathways, not involving 5-hydroxytryptamine1,2,3,4 receptors.

Prokinetic benzamides (e.g., cisapride) enhance gastrointestinal motility and transit. In vitro studies on the guinea pig ileum suggest that their effect is mediated via serotonergic 5-hydroxytryptamine (5-HT4) receptors, resulting in a facilitation of cholinergic neurotransmission. However, most in vivo studies have been performed on the canine stomach. Therefore, our aim was to determine whether the findings obtained on the guinea pig ileum can be extrapolated to another species and another organ. Does a benzamide facilitate cholinergic neutrotransmission on strips of the canine stomach in v

Cholinergic inputs to motoneurons in the spinal cord augment focal muscle spasticity in rodents.

BACKGROUND AND PURPOSE: Cholinergic premotor inputs to motoneurons support high levels of motor output when required. However, the possible role of these inputs in pathologically elevated muscle tone has not been characterised. To address this, we examined the modulatory effect of spinal acetylcholine (ACh) on experimental spastic paralysis. EXPERIMENTAL APPROACH: Rats were injected with tetanus neurotoxin (TeNT, 33-100 pg, im) into gastrocnemius to induce sustained rigid hind-paw extension and resistance to passive ankle flexion. The effects of spinal ACh on experimental hypertonia were prob

Nauclea officinalis extract rescues working memory deficits in adolescent maternal immune activation offspring by restoring cholinergic signaling.

OBJECTIVE: Maternal immune activation (MIA) is a key risk factor for schizophrenia-related cognitive deficits, linked to prefrontal cholinergic dysfunction. Nauclea officinalis extract (NOE) possesses documented anti-inflammatory and neuroprotective properties, but its effects on MIA-induced cognitive and neuronal dysfunctions are unknown. This study aimed to investigate whether NOE ameliorates MIA-induced working memory deficits by modulating infralimbic medial prefrontal cortex (IL-mPFC) cholinergic signaling and neuronal activity. METHODS: Using a poly (I:C)-induced MIA rat model, adolesce
