Fine-needle aspiration biopsy can be utilized effectively in the diagnosis of many bone lesions. Often, these lesions are suspected to be foci of metastatic disease based on clinical and/or radiographic findings. On occasion, microscopic examination of the aspirate yields a diagnosis of a primary neoplasm of chondroid origin. We discuss the cytologic features observed in crush preparations of the most frequently seen primary chondroid tumors of bone and how these lesions can be differentiated from each other and from metastatic lesions.

PURPOSE: Solitary enchondroma is the most common bone tumour at the hand. Nevertheless there are only a few studies referring to a recurrence of this lesion after operative treatment.
PATIENTS AND METHOD: 21 patients (17 women/4 men) were examined retrospectively 2 to 18 (mean: 9) years after operative treatment. Clinical and radiological examinations were performed. 3 to 4 years later 9 of these patients were re-examined. 1 patient was first seen at the time of the second examination. Enchondromata are among the most common primary neoplasms of the hand, which often present as pathologic fractures. The purpose of this study is to determine whether there are any differences between cases in which both fracture and tumor were treated primarily and those in which tumor treatment was delayed. 29 cases of chondromyxoid fibroma (CMF) from the material submitted to the Bone Tumour Register in Münster were reviewed. The tumour was observed most frequently in the second and third decades of life, with a slight female preponderance in our series. The diaphyseal and metaphyseal regions of the lower limb were most commonly affected, with a particular predisposition for the knee. Benign and malignant cartilage- and bone-forming tumors arising in soft tissues are unusual. Hyaline cartilage tumors may easily be mistaken as malignant if the histologic criteria of intraosseous lesions are applied directly. Other chondroid- and bone-forming tumors also may be difficult to recognize. The correct pathologic diagnosis within this group of neoplasms requires close clinical correlation, as well as familiarity and careful interpretation of a broad histologic spectrum.

Three cases are reported in which an osteosarcoma developed in relation to an enchondroma in a long bone. Two of the cases were in the proximal femur whilst one occurred in the proximal humerus, both recognised sites for old calcified enchondromas or "cartilage rests". The ages of the patients at presentation were 55, 63 and 84 years and all were women. Two patients died with pulmonary metastases within six months of the onset of clinical symptoms. A 60-year-old man suffered locally from a rather aggressive chondromatosis of the elbow. Clinically the presentation was rather uncommon, with some radiological aspects (juxta-articular bone rarefaction) differing from those of ordinary chondromatosis. Neither the pathologist nor the radiologist recognized signs of malignancy. Regional osteoporosis, although rarely described in synovial chondromatosis, was considered as the diagnosis in this patient, but a low grade primitive chondrosarcoma must be sought in such cases.

Cartilaginous lesions of the skeleton are very unusual. It is extremely important to correlate the roentgenographic features, the clinical features, and the histological features to arrive at a definite diagnosis. Most cartilaginous lesions are benign or of low-grade malignancy. However, there are some subtypes of chondrosarcoma that behave in a highly aggressive fashion.
