The purpose of this study was to explore the decision-making process of pregnant women with positive reaction to maternal serum screening for Down's syndrome when facing amniocentesis. The objective was to determine whether CA-125 levels have any clinical utility in gestational trophoblastic disease. Fifty-one patients with a pathologically confirmed diagnosis of complete hydatidiform mole had a CA-125 level obtained prior to suction molar evacuation. The authors report on the changing aspects in diagnostics and surgical treatment of tubal pregnancy during a period of 7 years (1983-89). In that period, 432 cases of tubal pregnancy were diagnosed and treated, 311 of which were treated by endoscopic abdominal surgery. Diagnosis of tubal pregnancy now depends on radioimmunology for identifying the presence of beta-HCG in the serum as well as on sonographic diagnosis. A series of data on amniotic fluid levels of alpha-fetoprotein (AFP) and beta-human chorionic gonadotropin (HCG) are presented. Both hormones were observed to decrease with advancing gestational age. The diagnostic importance of AFP levels is confirmed, and the very high levels of beta-HCG in twin gestations are documented. A statistically significant correlation between the concentration of both hormones is demonstrated.
