Crush injury and crush syndrome: a consensus statement.

Crush syndrome remains rare in European practice but is common in areas of civil disorder and where society has given way to civil war or natural disaster. Clinicians in the west are becoming increasingly involved in such situations and there is no reason to believe that the few instances due to conventional causes, such as an elderly person collapsing or road traffic accidents, will cease. Therefore, it is important that clinicians who deal rarely with crush syndrome have access to appropriate guidelines. This consensus report seeks to provide such advice.

[Urban space, war and social diseases: the case of infantile trachoma in Valencia,1936-1937].

From late 1937 on, there was a resurgence in Spain of endemic diseases such as trachoma. This disease had been controlled since the beginning of the twentieth century, especially during the Second Republic (1931-1936); however, the decline in living conditions and the population displacements during the Spanish Civil War (1936-1939) facilitated the resurgence of endemic trachoma. This article seeks to analyze the consequences of the Civil War on the struggle against infantile trachoma in Valencia, a city in the rearguard of the Republican zone in which poverty and overcrowding appeared suddenl

[Risk area 'misdiagnosis'-Risks of criminal liability and behavioural tips in pathological practice].

The article presents the concept of misdiagnosis or malpractice to practising pathologists from a medical and legal perspective and highlights similarities and differences in this respect. In particular, the risk of criminal liability for pathologists in the context of their practice activities is addressed and the relationship between the legal judgements of criminal, civil and professional law is examined in more detail. Due to the lack of an existing reversal of the burden of proof in criminal law, the constellation may arise in practice that a treatment error under civil law is assumed. Ho

Sociostructural Determinants of Health for People Living With HIV During the COVID-19 Pandemic: A Policy Brief for Future Global Health Crises.

The dual pandemics of COVID-19 and HIV have underscored critical gaps in global health policy, particularly regarding disproportionate impacts on people living with HIV, often failing to address sociostructural determinants of health (sSDoH) that fuel inequities. Nurses and other health care practitioners, possessing unique knowledge and technical capacity, are vital in advocating for policy innovations that address the civil and human rights of vulnerable populations. This policy brief, informed by historical health frameworks like the Alma-Ata Declaration and contemporary understandings of s

The twilight envelope: a user-centered approach to describing roadway illumination at night.

Visual recognition functions, such as acuity and contrast sensitivity, deteriorate rapidly over the declining luminances found during civil twilight. Thus civil twilight, a critical part of the transition between daylight and darkness, represents lighting conditions that may be useful to describe artificial illumination. Automotive headlamps project a three-dimensional beam that ranges from illumination levels comparable to daylight at the vehicle to the dark limit of civil twilight (3.3 1x) at some distance ahead. This twilight envelope is characterized as a distance beyond which foveal visua

Congenital anomalies of the limbs in mythology and antiquity.

Congenital anomalies of the limbs have been observed since ancient human civilizations, capturing the imagination of ancient physicians and people. The knowledge of the era could not possibly theorize on the biologic aspects of these anomalies; however, from the very beginning of civilization the spiritual status of people attempted to find a logical explanation for the existence of such cases. The next logical step of the spiritual and religious system of the ancients was to correlate these anomalies with the Gods and to attribute them to a different level of existence in order to rationalize…

Tracking global resources and capacity for health research: time to reassess strategies and investment decisions.

The COVID-19 pandemic and more recently the Monkeypox outbreak emphasize the urgency and importance of improving the availability and equitable distribution of resources for health research across rich and poor countries. Discussions about the persistent imbalances in resource allocation for health research between rich and poor countries are not new, but little or no progress has been made in redressing these imbalances over the years. This is critical not only for emergency preparedness, but for the worlds' ability to improve population health in an equitable manner. Concerned with the lack

The rule of law and bioterrorism.

Bioterrorism is multi-faceted. Its impact will extend beyond the victims, the agent used, and the perpetrator(s). The rule of law must be considered in the wake of September 11 and the fall 2001 anthrax attacks. Bioterrorism preparedness should address rule of law and social issues. Laboratory professionals must be prepared for professional, civil justice, and social impact in the event of a future major bioweapons event.

We present the case of spontaneous damage to the patellar tendon in a patient with pathologic obesity (BMI = 41), without comorbid conditions. Damage occurred during normal load combined with the movement of the knee extension when inserting a foot in flip-flops. Surgical treatment was performed in a matter of urgency and the patellar tendon was sutured in half the length of. Six weeks of immobilization was used and then rehabilitation. After four months efficient gait and a good range of motion was achieved. Please note that obesity is a disease of civilization and cause further disease. Thes…

[Some medical documents written by Ahmed Cevdet Pasha].

The assumption that civilization was originated in the East, was one of the most important points emphasized by the intelligentsia of the late Ottoman reign. This conviction was appreciated in the thought of Ahmed Cevdet Pasha (1823 - 1895), a famous figure of the period. Cevdet Pasha, who believed that the source of science and education was started in the East, claimed that Europe was acquainted with Islamic science during the Crusades. In this paper Cevdet Pasha's ideas related with small-pox, cholera, plague, poisons and microbes are mentioned. This study is based on Cevdet Pasha's documen…

Progress toward poliomyelitis and dracunculiasis eradication--Sudan, 1999-2000.

Sudan began poliomyelitis and dracunculiasis eradication activities in 1994 and 1995, respectively, in response to resolutions by the World Health Assembly of the World Health Organization (WHO). Sudan poses special obstacles to global eradication campaigns as a result of the disruption caused by ongoing civil war in the vast southern part of the country. The activities of both programs are summarized in this report, which indicated that substantial progress was made to eradicate polio and control of dracunculiasis improved slightly. Continued commitment of resources, access to persons in area

[Characteristics of the blood lipoprotein spectrum in commercial aviation pilots].

The study of lipids of civil pilots, aged 30-59 years, showed that their average levels of cholesterol and triglycerides were higher than those in a random sample of the male population of Moscow. The average level of HDL cholesterol in pilots was lower than in the sample. The prevalence of dislipoproteinemias in pilots was significantly higher than in the sample. The above lipoprotein changes in pilots versus nonpilots and the prevalence of hyperlipidemias suggest that they are "aterogenic" and produced by the flying profession. These observations also indicate that civil pilots should be reg

A review of visceral leishmaniasis during the conflict in South Sudan and the consequences for East African countries.

BACKGROUND: Visceral leishmaniasis (VL), caused predominantly by Leishmania donovani and transmitted by both Phlebotomus orientalis and Phlebotomus martini, is highly endemic in East Africa where approximately 30 thousands VL cases are reported annually. The largest numbers of cases are found in Sudan - where Phlebotomus orientalis proliferate in Acacia forests especially on Sudan's eastern border with Ethiopia, followed by South Sudan, Ethiopia, Somalia, Kenya and Uganda. Long-standing civil war and unrest is a dominant determinant of VL in East African countries. Here we attempt to identify

Clinical microbiologists facing an anthrax alert.

Microbiological war and terrorist attacks are made to weaken populations by transmitting pathogenic and epidemic microorganisms. These bacteria or viruses are often difficult to diagnose. Anthrax alerts following September 2001 showed that most clinical microbiology laboratories were not adequately prepared, using obsolete diagnostic methods or being too slow to use accurate tools when facing a major threat. Following this period, most microbiology laboratories were prepared for bioterrorism alerts, in order to provide accurate and rapid results, although such events are rare and unexpected. I

Bioterrorism and the people: how to vaccinate a city against panic.

Bioterrorism policy discussions and response planning efforts have tended to discount the capacity of the public to participate in the response to an act of bioterrorism, or they have assumed that local populations would impede an effective response. Fears of mass panic and social disorder underlie this bias. Although it is not known how the population will react to an unprecedented act of bioterrorism, experience with natural and technological disasters and disease outbreaks indicates a pattern of generally effective and adaptive collective action. Failure to involve the public as a key partn

Punitive legal responses to prenatal drug use in the United States: A survey of state policies and systematic review of their public health impacts.

BACKGROUND: Punitive legal responses to prenatal drug use may be associated with unintended adverse health consequences. However, in a rapidly shifting policy climate, current information has not been summarized. We conducted a survey of U.S. state policies that utilize criminal or civil legal system penalties to address prenatal drug use. We then systematically identified empirical studies evaluating these policies and summarized their potential public health impacts. METHODS: Using existing databases and original statutory research, we surveyed current U.S. state-level prenatal drug use pol
