[Scapholunate instability. Diagnosis--classification--treatment].

Scapholunate instability is more or less caused by injury. The ligament instability between lunate and scaphoid is not easily to be identified even there is only a little widening of this space between both bones. The instability may be proved by physical examination, by x-ray and by arthroscopy. The scapholunate instability may occur in a different manner. It is divided in three stages. In stage I a none operative treatment is sufficient. For the other stages an operative treatment is necessary: The operative treatment begins with suture of the ligament, continues with ligamentoplasty and end

[TNM classification for urological cancer].

The 5th edition of the new TNM classification for urological cancer has been published by UICC in 1997. Herein, the classification of 4 urological carcinomas (kidney, urinary bladder, renal pelvis and ureter, and urethra) is presented and discussed in comparison with the latest revisions in 1987 and 1992. In the 5th edition, the main revised points are as follows: As for kidney, the primary tumor cut off between T1 and T2 was changed from 2.5 cm to 7.0 cm, and the N classification was simplified as for urinary bladder, all muscle invasive tumors (T2 or T3a in the 1992 classification) are inclu…

[Acute pancreatitis. Classification, diagnosis, therapy].

Acute pancreatitis is a multietiologic entity with rather diverse clinical courses. Whereas edematous pancreatitis has a mortality of less than 1%, nowadays; still approximately 20% of all patients with the necrotizing form succumb to the disease. To further improve therapeutic results a standardized approach should be used. For effective treatment the differentiation between edematous and necrotizing pancreatitis is crucial. All patients with signs of pancreatic necroses during abdominal ultrasound and patients with organ insufficiencies should undergo a CT-scan to define exactly the nature a

Laryngomalacia: a proposed classification form.

Laryngomalacia is a common cause of stridor in children. The disorder has a heterogenous presentation, from the mildest form, which resolves … rogeneity of the disorder to be organized. Wider application of this form across institutions, with classification of patients with laryngomalacia by historical and anatomic factors, should allow the accumulation of sufficient numbers of patients to allow statistical analyses of treatment and outcome as they relate to the initial presentation of this disorder of …

Recent progress in lymphoma classification.

In the Revised European-American list of lymphoid neoplasms recently proposed by the hematopathologists of the International Lymphoma Study Group, the disease entities currently recognized in the literature among the B-cell and T/natural killer (NK)-cell lymphomas and leukemias and variants of Hodgkin's disease are collected and defined. This proposal is here to stay. The main challenge for the future is to present the information in this list in a form that facilitates the understanding, learning, and teaching of these disorders. A meaningful classification for clinical use has already been b…

[Classification of regular atrial tachycardia].

ECG criteria which for many years formed the basis of the classification of regular atrial tachycardias may now be completed by the results of endocavitary studies (stimulation and mapping). Flutter is a macroreentry phenomenon in the right atrium, anticlockwise in typical, common or classical forms, and more variable in atypical forms: sometimes antidromic and clockwise, sometimes functional more rapid without a gap of excitability, or in other cases, skirting around the sears of atriotomy, especially right-sided. Tachycardias are paroxysmal and often secondary to reentry; more rarely permane…

Multidimensional classification of affective disorders.

Research data on biological markers, clinical presentation and course of affective disorders justify proposal of a new multidimensional classification of these disorders. The classification is made according to six parameters: clinical picture, severity, course, duration of episodes and lucid intervals, etiology and premorbid personality. The clinical picture defines the predominant syndrome types and subtypes. The severity of the disease is graded as mild, moderate and severe. The course type includes two subgroups: unipolar and bipolar. The fourth parameter gives the duration of the episodes…

Pyoderma gangrenosum: classification and management.

Pyoderma gangrenosum (PG) has four distinctive clinical and histologic variants. Some have morphologic and histologic overlapping features with other reactive neutrophilic skin conditions. PG often occurs in association with a systemic disease, and the specific clinical features of the skin lesion may provide a clue to the associated disease. Management of PG depends on its type and severity and usually requires aggressive local and systemic treatment.

[Morphologic classification of chronic hepatitis].

The diagnosis of chronic hepatitis (CH) should include the information on CH etiology, the presence or absence of CH activity as well as its morphological characteristics. CH may be of a viral etiology (the type of the virus should be mentioned in the diagnosis), autoimmune, drug-induced and alcoholic. The etiology can be established by means of the clinicomorphological analysis using immunological and immunohistochemical techniques. Sometimes it is possible to do so after routine staining of biopsies. Portal, lobular and periportal hepatitis are distinguished morphologically. Instead of the i

Practical classification of chin deformities.

A visually pleasing chin is an important component of facial harmony. This study was undertaken to introduce a practical classification of chin dysmorphology, which can be used to guide the surgeon toward the appropriate surgical approach to chin repair. Analysis of life-size photography, based on standard anthropometric measurements, was completed on 2,879 patients from 1981 to 1991. Six hundred eighty-four patients were noted to have normal occlusion with some form of chin deformity. Analysis of this group of patients identified seven categories of chin dysmorphism: Class I, macrogenia (n = …

Classification of morphea (localized scleroderma).

OBJECTIVE: To classify and describe morphea (localized scleroderma). DESIGN: A review of morphea and its subtypes is presented. RESULTS: The current classification of morphea is incomplete and confusing. As knowledge of the spectrum of disease continues to evolve, the controversy and confusing nature of its multiple subtypes present a challenge for the physician who encounters a patient with this condition. Thus, we propose that morphea be classified into the following five groups: plaque, generalized, bullous, linear, and deep. This classification, based on clinical morphologic findings, wi…

Lymphoproliferative diseases of uncertain classification.

In spite of the recent progress in the biology and histogenesis of the lymphoproliferative diseases, many doubts still remain about their possible evolution. On the one hand, many cases have been studied showing a benign course in spite of a typical monoclonal phenotype; on the other, some clinically aggressive forms of lymphoproliferation prove to be polyclonal throughout their course, even when checked with highly sophisticated techniques. In this article we have reviewed, in both clinical and etiopathogenetic terms, the classification of these processes in the light of the new findings prov…

[Diaphragmatic injuries, classification and therapy].

Injuries of the diaphragm are caused either by direct or indirect violence. In all such ruptures various portions of the abdominal viscera may have ascended into the thoracic cavity. The clinical manifestations are unpredictable and of infinite variety, and especially in massively traumatized patient masked by other injuries. A presumptive preoperative diagnosis of diaphragmatic injury from chest x-ray findings is only possible in about 50% of the cases. In other cases the diagnosis can be confirmed by water.soluble contrast enema into the stomach or colon. Rupture of the diaphragm is a strong
