Modified tension band fixation and coracoclavicular stabilisation for unstable distal clavicle fracture.

BACKGROUND: The purpose of this study was to report a new surgical technique for the treatment of unstable distal clavicle fracture with modified tension band fixation (MTBF) and coracoclavicular (CC) stabilisation, and evaluate the radiologic and clinical outcome of these patients. MATERIAL AND METHODS: Thirteen patients who had a fracture of the distal clavicle (Neer classification type IIb) were treated with MTBF and CC stabilisation. After the CC stabilisation was performed with a suture anchor or flip button, internal fixation using MTBF was carried out. Each patient was followed up for …

Functional and radiographic outcomes of nonoperative treatment of displaced adolescent clavicle fractures.

BACKGROUND: Studies of adult patients suggest that nonoperative treatment of clavicle fractures may result in functional disability, but this has not been demonstrated in adolescents. The purpose of this study was to determine the functional outcomes after nonoperative treatment of displaced, shortened, midshaft clavicle fractures in adolescents. METHODS: Adolescents ten to eighteen years of age with an isolated, completely displaced, shortened, midshaft clavicle fracture sustained between 2009 and 2011 were recruited for this study. Injury and final radiographs were assessed for displacement…

Iatrogenic subclavian arteriovenous fistula: rare complication of plate osteosynthesis of clavicle fracture.

Iatrogenic subclavian arteriovenous fistula is rare and has not been reported as a complication of plate osteosynthesis of clavicle fracture. This article describes the first case of iatrogenic subclavian arteriovenous fistula caused by plate osteosynthesis. A 36-year-old man sustained a right middle clavicle fracture in an injury and underwent open reduction and internal fixation with clavicular compression plate 3 days later in a local hospital. On the second postoperative day, a pulsatile mass and thrill were detected at the right supraclavicular region. The patient was discharged 3 days po…

[Minimally invasive treatment for fresh acromioclavicular dislocation and the distal clavicle fracture].

OBJECTIVE: To explore the minimally invasive treatment for fresh acromioclavicular dislocation and the distal clavicle fracture. METHODS: Thirty skeletons of human shoulder were measured and compared, and the normal data on healthy people were measured with the help of ultrasound-guided. So the invasion point was located at the cross between subclavian axis and the line from coracoid tip to apophysis behind cone ligament node. From January 2001 to January 2010, 127 patients with fresh acromioclavicular dislocation and distal clavicle fracture were treated with minimally invasive internal fixa…

Conservative interventions for treating middle third clavicle fractures in adolescents and adults.

BACKGROUND: Clavicle fractures account for around 4% of all fractures. Treatment of these fractures is usually non-operative. OBJECTIVES: To evaluate the effects of different methods for conservative (non-operative) treatment for acute (treated soon after diagnosis) middle third clavicle fractures in adolescents and adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register, the Cochrane Central Register of Controlled Trials (The Cochrane Library 2008, Issue 4), MEDLINE (from 1966), EMBASE (from 1980), LILACS (from 1982), trial registers and re…

A biomechanical study of plate versus intramedullary devices for midshaft clavicle fixation.

BACKGROUND: Non-operative management of midshaft clavicle fractures is standard; however, surgical management is increasing. The purpose of this study is to compare the biomechanical performance of plate versus intramedullary fixation in cyclic bending for matched pairs of cadaveric clavicles. We hypothesized that the biomechanical properties are similar. METHODS: Eight sets of matched clavicles with vertical, midshaft osteotomies were prepared from fresh, frozen cadavers. A 3.5 mm dynamic compression plate or a 3.8 or 4.5 mm intramedullary device were used for fixation. Clavicles were loaded…

Endovascular treatment of acute subclavian pseudo-aneurysm after fracture of the clavicle.

Pseudo-aneurysms of the subclavian artery remain a rare complication after fracture of the clavicle. Nerve compression has been described with delayed paralysis as a consequence. We report a case of delayed diagnosis of a subclavian pseudo-aneurysm after a closed fracture of the clavicle in a 93-year-old patient. Diagnosis was confirmed by ultrasound and multi-slice-Ct. Two attempts to treat the pseudo-aneurysm with percutaneous thrombin injection failed. Considering the age of the patient, it was decided to exclude the pseudo-aneurysm by endovascular approach. A Hemobahn endoprosthesis was in…

Salvage procedure for chronic acromioclavicular dislocation subsequent to overzealous distal clavicle resection.

This article presents our surgical technique for reestablishing acromioclavicular stability after overzealous distal clavicular resection. Two cases are presented in which the clavicle of the affected acromioclavicular joint was unstable, causing pain with range of motion (ROM). To address the pathology, reconstruction of the coracoclavicular ligament and reattachment of the trapezius and deltoid to the clavicle were required. We reconstructed the coracoclavicular ligaments using an Achilles tendon calcaneal allograft. The bony end of the Achilles tendon allograft was used as an extension to l…

CASE REPORTS: unusual lateral clavicle fracture dislocation: case reports and museum specimens.

We present two clinical cases and four museum specimens of a type of lateral clavicle fracture dislocation that has not been detailed in any of the commonly accepted classification systems. With this injury, the major clavicular element is displaced downward and backward into the supraspinatus outlet and may cause symptomatic impingement. A method for surgical correction is described.

One and Done: A Retrospective Analysis of Radiographic Overuse in Pediatric Clavicle Fractures.

BACKGROUND: Clavicle fractures in children are typically managed nonsurgically due to immense bony remodeling potential. Therefore, repeat radiographs may not be necessary. This study aims to quantify repeat radiographs for pediatric nonsurgical clavicle fractures. METHODS: A retrospective study was conducted of children aged 8 years or younger with clavicle fractures between 2018 and 2022 at a single institution. Children were excluded if the fracture was managed surgically, if initial radiographs were unavailable, or if there was a concomitant ipsilateral shoulder fracture. RESULTS: Medica…

An unusual presentation of a depressed clavicle fracture in a child leading to venous congestion.

Clavicle fractures are common in children, comprising up to 15% of fractures presenting to the emergency department. Typically, mid-shaft clavicle fracture in the paediatric population is managed conservatively, with surgery recommended in a select few. A 12-year-old boy presented with a depressed mid-shaft clavicle fracture with an ipsilateral cervical rib exhibiting features of venous congestion. This case was managed conservatively using a sling and the patient was admitted for overnight observation with early mobilisation encouraged. His recovery was complicated by a re-fracture 4 weeks la…

A Case of Bilateral Congenital Pseudoarthrosis of Clavicle: An Uncommon Variant of a Rare Disorder.

Congenital pseudarthrosis of the clavicle (CPC) is a rare disorder diagnosed at birth or early childhood presenting with a painless, non-tender mass on the clavicle. Its etiology is unknown, caused by failure of fusion of the medial and lateral ossification centers of the clavicle. Left-sided CPC is rare and linked to other pathological abnormalities. Bilateral involvement is extremely rare and it is seen in association with other congenital malformations. A full-term newborn baby girl was examined after a complicated emergency cesarean section delivery. Upon initial pediatric examination, the…

A Novel and Open Classification Emphasizing on Osteoligamentous Complex for Distal Clavicle Fractures.

OBJECTIVE: Current X-ray-based classification methods cannot describe all distal clavicle fracture (DCF) patterns, especially the osteoligamentous injury pattern of DCFs. We aimed to develop a novel classification based on the osteoligamentous injury pattern of the DCFs and investigated its reliability. METHODS: All DCFs from January 2017 to January 2022 were respectively screened and 45 cases (mean age 20-78; male 31, female 14) met the including criteria and were enrolled. Based on their Zanca view X-ray radiograph and three-dimensional CT construction images, we analyzed the osteoligamento…

Congenital Pseudarthrosis of the Clavicle: A Case Report and a Hypothesis for the Right-Side Predominance.

Congenital pseudarthrosis of the clavicle (CPC) is rare. It predominantly affects the right side for an unknown reason. Most of the reported cases are diagnosed outside the neonatal period. Only two CPC cases have been reported in Saudi Arabia, where both were diagnosed during childhood. Here, we present the case of a Saudi male newborn with right-sided CPC. The diagnosis was made shortly after birth because of the uneventful cesarean delivery and painless clavicular lump. Fetuses prefer keeping their head in a right lateral position which may be a plausible explanation for the right-side pred…

Repair of a Crossbow Bolt Injury of the Subclavian Artery Behind the Mid-clavicle.

The management of a rare midclavicular crossbow bolt injury to the subclavian artery is discussed. Important concepts include the initial clinical diagnosis, operative planning, the surgical approach to the retro-clavicular great vessels, the technical aspects of repair, and postoperative course. A discussion of the reasoning behind an operative vs. endovascular approach is also discussed.
