Propionibacterium (Propionimicrobium) lymphophilum is a Gram-positive anaerobic rod involved in few human diseases. We report a rare case of bacteremia due to this microorganism in an elderly patient. A 95-year-old woman without a remarkable medical history presented with dyspnea, chest pain and fever for seven days. Blood cultures resulted in isolation of P. lymphophilum. Resistance only to metronidazole was found. Treatment with amoxicillin-clavulanic acid was established, and the patient was discharged and improvement of her general condition was documented.

A 63-year-old woman presented with a 5-day history of high-grade fever, petechiae on the lower legs and severe thrombocytopenia. Clinical findings included left inguinal lymphadenopathy and an encrusted wound on the left tibia, which the patient had sustained from a rotten piece of wood. Her pet dog had sporadically licked the wound. After more than 2 days of blood culture incubation, Capnocytophaga canimorsus was finally detected. Antibiotic treatment with amoxicillin/clavulanic acid led to a complete recovery.

Mycetomas are localized chronic inflammatory infectious diseases involving subcutaneous tissues, skin and bones. We report a case of mycetoma in a 40-year-old farmer from the north of Senegal. The clinical appearance of the lesions suggested primarily the diagnosis of a soft tissue malignant tumor. Mycetoma diagnosis was confirmed by histopathologic evidences and growth of Actinomadura pelletieri in cultured samples. The evolution was good under a triple antibiotic therapy combining cotrimoxazole, amoxicillin-clavulanic acid and streptomycin for 12 months.

A 21-year survey conducted in northeast Thailand of antimicrobial resistance to parenteral antimicrobial drugs used to treat melioidosis identified 24/4,021 (0.6%) patients with one or more isolates resistant to ceftazidime (n = 8), amoxicillin-clavulanic acid (n = 4), or both drugs (n = 12). Two cases were identified at admission, and the remainder were detected a median of 15 days after starting antimicrobial therapy. Resistance to carbapenem drugs was not detected. These findings support the current prescribing recommendations for melioidosis.
