Case report: Diagnosis of apical hypertrophic cardiomyopathy that escaped clinical and echocardiographic investigations for twenty years: Reasons and clinical implications.

BACKGROUND: Apical hypertrophic cardiomyopathy (ApHCM) is a rare form of hypertrophic cardiomyopathy which predominantly affects the apex of the left ventricle. The diagnosis can be challenging due to several factors, ranging from no typical clinical and electrocardiogram (EKG) findings to potential difficulties in executing and interpreting the echocardiographic examination. CASE PRESENTATION: We report the case of an 84-year-old woman who came to our echo-lab to undergo a routine echocardiogram. She had a history of permanent atrial fibrillation, paced rhythm and previous episodes of heart

Evaluation of the clinical reasoning of GPT-4o, a multimodal generative artificial intelligence model, in 18 public gastroenterology case studies.

INTRODUCTION AND AIM: although generative language models have been extensively studied in the field of digestive diseases, further progress requires addressing underexplored aspects such as linguistic bias, the evaluation of clinical reasoning underlying model responses, and the use of realistic clinical material in non-English-speaking contexts. The aim of this study was to evaluate the accuracy of GPT-4o to answer clinical questions in Spanish and to qualitatively analyze its errors. METHODS: we used the most recent official board examination for specialist in Gastroenterology (Spain, 2023…

Balancing technical rational with professional artistry: a case study in developing clinical reasoning for advanced care practitioners in primary care.

Clinical reasoning is a critical skill for all healthcare practitioners. Learners aspiring to independent practice in primary care are further challenged where diagnostic uncertainty and patient preferences frequently intersect. This case study describes the assessment of a 72-year-old man presenting with unilateral leg swelling following total knee replacement. Although his Wells' score and a raised d-dimer suggested possible deep vein thrombosis (DVT), contextual evidence indicated that d-dimer levels remain elevated for weeks postoperatively. The patient also preferred to avoid unnecessary …

The impact of fellowship and board-certification on diagnostic clinical reasoning in lumbar spine dysfunction using the script concordance test.

BACKGROUND: Clinical reasoning (CR) is essential for effective physical therapy (PT) practice but remains inconsistently defined and under-researched, particularly in terms of its development across varying levels of clinical expertise. Diagnostic reasoning (DxR), a key component of CR, involves making decisions under uncertainty and is fundamental to accurate clinical judgment. The Script Concordance Test (SCT), grounded in Cognitive Script Theory, is a validated tool used to assess DxR by comparing examinee responses to those of experienced clinicians. Although SCT has seen broad use in medi…

Clinical Reasoning: A 70-Year-Old Man With Systemic Illness Related Strokes Refractory to Medical Treatment Managed With Intracranial Stent.

We present the case of a 70-year-old man with a history of embolic stroke, atrial fibrillation, deep vein thrombosis, and polymyalgia rheumatica who presented as a stroke code with transient right-sided focal neurologic deficits (motor and sensory), mild alteration in consciousness, and mild aphasia. His cerebrovascular imaging revealed new multifocal intracranial stenoses. Despite best medical management, this patient continued to have recurrent symptomatic cerebrovascular events. This case illustrates the evaluation approach and key differential diagnoses to consider in patients with stroke-

Clinical Reasoning and Challenges Faced With Onset Psychotic Symptoms in a Case of Combined Huntington's Disease and Gayet-Wernicke Encephalopathy.

Physicians are occasionally confronted with patients presenting psychotic symptoms of organic origin. Therefore, precision in diagnosing the organic basis is pivotal for targeted treatment, addressing the underlying etiology. This case study delineates the nuanced phases of clinical reasoning employed to ascertain a diagnosis of Huntington's disease (HD), notably amidst concurrent alcohol dependence. A comprehensive clinical examination and meticulous review of the patient's medical history served as linchpins in guiding subsequent investigations toward identifying the etiological underpinning…

Case Report and Clinical Reasoning: Fulminant Liver Failure and Invasive Aspergillosis Following Ocrelizumab Treatment in a 21 Year-Old Woman.

We present the case of a 21 year-old woman with newly diagnosed relapsing-remitting multiple sclerosis who is given a single dose of ocrelizumab and placed on moderate-dose steroids with subsequent development of hepatic failure who goes on to develop highly fulminant systemic and central nervous system (CNS) aspergillosis. Ocrelizumab has no documented association with aspergillus infection, and moderate-dose steroids less often lead to such fulminant disease, but liver failure is associated with often-fatal aspergillus infection. We emphasize that liver failure is an underrecognized immune d

NeuroNarrator: A Generalist EEG-to-Text Foundation Model for Clinical Interpretation via Spectro-Spatial Grounding and Temporal State-Space Reasoning.

Electroencephalography (EEG) provides a non-invasive window into neural dynamics at high temporal resolution and plays a pivotal role in clinical neuroscience research. Despite this potential, prevailing computational approaches to EEG analysis remain largely confined to task-specific classification objectives or coarse-grained pattern recognition, offering limited support for clinically meaningful interpretation. To address these limitations, we introduce NeuroNarrator, the first generalist EEG-to-text foundation model designed to translate electrophysiological segments into precise clinical

Integrating Systems and Critical Thinking, Clinical Reasoning, Decision-Making, and Judgement in Dietetic Education: Development of a Conceptual Pilot Model.

INTRODUCTION: Dietetic practice is increasingly characterised by complexity arising from chronic disease, food and nutrition insecurity, demographic shifts, and environmental sustainability concerns. Addressing this complexity requires the effective and interprofessionally aligned use of central cognitive strategies. However, despite frequent reference to systems thinking, critical thinking, clinical reasoning, clinical decision-making, and clinical judgement in health professional education, inconsistent use, and limited shared understanding across professions hinder coherent educational appr…

Evaluation of large language models in supporting autoimmune liver disease diagnosis and clinical decision-making: advantages of reasoning-based models.

INTRODUCTION: Large-language models (LLMs) have demonstrated increasing potential in healthcare, including applications in clinical information retrieval, medical reasoning, and decision support. Autoimmune liver diseases (AILDs), including autoimmune hepatitis, primary biliary cholangitis, primary sclerosing cholangitis, and IgG4-related liver disease, are rare and heterogeneous conditions that often present with nonspecific features and require specialized expertise for accurate diagnosis and management. In resource-limited settings, limited access to hepatology subspecialists may contribute

Lessons in clinical reasoning - pitfalls, myths, and pearls: a case of tarsal tunnel syndrome caused by an intraneural ganglion cyst.

OBJECTIVES: Intraneural ganglionic cysts are non-neoplastic cysts that can cause signs and symptoms of peripheral neuropathy. However, the scarcity of such cases can lead to cognitive biases. Early surgical exploration of space occupying lesions plays an important role in identification and improving the outcomes for intraneural ganglionic cysts. CASE PRESENTATION: This patient presented with loss of sensation on the right sole with tingling numbness for six months. A diagnosis of tarsal tunnel syndrome was made. Nerve conduction study revealed that the mixed nerve action potential (NAP) was

Clinical Reasoning: A 43-Year-Old Man With Subacute Onset of Vision Disturbances, Jaw Spasms, and Balance and Sleep Difficulties.

A brainstem syndrome is recognizable in patients presenting with a combination of visual disturbances, incoordination, gait problems, speech and swallowing difficulties, and new-onset sleep symptomatology. Brainstem disorders of subacute onset (onset and progression with accumulation of disabling deficits in 6-12 weeks) are generally of autoimmune, infectious, inflammatory, or infiltrative neoplastic cause. An autoimmune or infectious brainstem disorder may be referred to as brainstem encephalitis or rhombencephalitis. We describe a patient with paraneoplastic autoimmune rhombencephalitis, in

Adaptation of a clinical reasoning model for use in inflammatory conditions of the lactating breast: a retrospective mixed-methods study.

BACKGROUND: Many potential factors associated with Inflammatory Conditions of the Lactating Breast (ICLB) have been reported in the literature, by lactating mothers and clinicians. Clinicians, including general practitioners, lactation consultants and physiotherapists, require a clinical reasoning model that summarises associated or linked factors, to aid in the assessment, treatment, and prevention of ICLB. Thus, we aimed to adapt the existing Breastfeeding Pain Reasoning Model (BPRM), for use in the management of ICLB, using prior research and clinical audit data to guide adaptation. The exi…

Optimal Load for Managing Low-Risk Tibial and Metatarsal Bone Stress Injuries in Runners: The Science Behind the Clinical Reasoning.

BACKGROUND: Low-risk bone stress injuries (BSIs) of the tibia and metatarsal diaphyses account for more than half of BSIs in runners. They interrupt training and are managed using noninvasive approaches that are designed to achieve a speedy but safe return to running. CLINICAL QUESTION: What is the optimal load to manage low-risk tibial and metatarsal BSIs and safely return to running? KEY RESULTS: Optimal load can be guided by knowledge of the BSI healing process and is symptom driven. At all stages, the optimal load does not produce symptoms during, after, or the day following loading. CL
