Closed talar body fracture with talonavicular dislocation; a case report.

Closed fracture dislocation of the body of the talus is rare. The dislocation of the subtalar and ankle joints (associated with fracture body) has been reported and already described in the current classifications. We report a case of closed fracture of the body of the talus associated with talonavicular dislocation. Open reduction and internal fixation was performed after failed attempt of closed reduction. The fracture has healed and there is no evidence of avascular necrosis after 2 years follow up. We recommend early evaluation of this injury by CT scan or by proper radiographic views unde

Reduction of anterior glenohumeral dislocations: a new closed reduction technique.

OBJECTIVE: A new closed reduction technique for anterior glenohumeral dislocations and tuberosity fracture dislocations is introduced. METHODS: Forty-one consecutive patients with an acute anterior glenohumeral dislocation or tuberosity fracture dislocation underwent closed reduction by an orthopaedic surgeon employing this new method. RESULTS: Closed reduction was successful in 88% of patients using the reduction maneuver. Associated fracture with glenohumeral dislocation did not influence the success rate of the reduction maneuver. An assistant was needed in 15% of cases. No complications

Long-term postoperative satisfaction and complications in nasal bone fracture patients according to fracture type, site, and severity.

BACKGROUND: It is difficult to completely fix nasal bone fractures with closed reduction, as it is often accompanied by septal cartilage damage, and this often results in postoperative secondary deformities. Thus, patients are often reluctant to undergo closed reduction surgery. The present study aimed to evaluate aesthetic and functional satisfaction, as well as satisfaction with and complications of closed reduction, according to nasal bone fracture type. METHODS: The subjects were patients who underwent closed reduction under general anesthesia from January 2017 to December 2018. Based on

Irreducible Metacarpal Neck Fracture due to Interposition of the Extensor Tendons.

A 15-year-old male presented with multiple right-hand fractures, including a severely angulated small finger metacarpal neck fracture. Multiple closed attempts failed, and open reduction was performed. At surgery, the extensor tendon was found to be interposed within the fracture, thereby preventing closed reduction. The tendon was extricated from the fracture site, adequate reduction was obtained, and the fracture was stabilized using K-wires. The fracture united well, with good return of motion and strength. Extensor tendon interposition is a rare scenario associated with metacarpal neck fra

Optimizing treatment of nasal fractures.

OBJECTIVES/HYPOTHESIS: A graduated protocol involving closed reduction, septoplasty, osteotomies, upper lateral cartilage release, anterior perpendicular plate fracture, and camouflaging cartilage grafts yields a higher percentage of straight noses in cases of acute fracture than treatment with closed reduction alone. STUDY DESIGN: Retrospective chart and literature review followed by a prospective, non-randomized sequential trial. METHODS: Closed reduction was performed on 26 cases of acute nasal fracture and the results were compared with a subsequent series of 79 cases treated using a mor

Primary Radial Nerve Exploration Determined by Ultrasound in Pediatric Supracondylar Humerus Fracture: A Report of Two Cases.

We report two pediatric cases of radial nerve palsy caused by supracondylar fracture of the humerus requiring nerve exploration. The children had comparable conditions, palsy types (complete motor and partial sensory) and radiographic findings. The fracture in the first case was managed conservatively with closed reduction and percutaneous pinning but, while the patient eventually recovered from the partial sensory palsy, her motor palsy remained unchanged. In the second case, ultrasound assessment of the nerve prior to fixation indicated that surgical exploration was needed as it revealed tet

An unusual Monteggia equivalent type 1 lesion: Diaphyseal ulna and radius fractures with a posterior elbow dislocation in a child.

In this report, we describe an extremely unusual Monteggia equivalent type 1 lesion in a 10-year-old boy following a fall from a height of 1 m. On the plain radiographs, our patient had a particular Monteggia equivalent type 1 injury associating a posterior elbow dislocation with diaphyseal radius and ulna fractures. The patient was treated by closed reduction technique. At six months of follow-up, the fractures were consolidated and the elbow was stable. To our knowledge, only 8 adult cases and one paediatric observation with similar lesions had been reported through medical literature. There

Treatment of shoulder dislocation with ipsilateral humeral shaft fracture.

A case presentation of shoulder dislocation with ipsilateral humeral shaft fracture is presented along with a review of the literature regarding nine reported cases. In the current case, closed treatment of the humeral shaft fracture was undertaken with an unsuccessful attempt at closed reduction of the anterior shoulder dislocation, resulting in a radial nerve deficit. Successful treatment required open reduction of the humeral fracture with compression plating followed by closed reduction of the shoulder dislocation. We compare this treatment (with outcome) to other methods of treatment.

Current Management of Subcondylar Fractures of the Mandible, Including Endoscopic Repair.

Treatment of subcondylar fractures has been the subject of debate for many years. Options for treatment include physical therapy, elastic maxillomandibular fixation, and open repair. Proper imaging and clinical evaluation are imperative when deciding on the best management option. In the past, most subcondylar fractures were treated with a closed approach. Recent data support open repair, when feasible. Studies show increased interincisal opening, lateral excursion, and protrusion with less mandibular shortening, jaw deviation, and pain. There are serious side effects that may be associated wi

Entrapment of Extensor Pollicis Longus Tendon after Volar Plating of a Smith Type Pediatric Distal Forearm Fracture.

Extensor pollicis longus tendon entrapment is a rare complication of volarly displaced pediatric distal radius fractures. The few pediatric case reports have described tendon entrapment associated with conservative fracture treatment, or have been recognized at subsequent revision surgery after failure to achieve closed fracture reduction. A case of extensor pollicis longus tendon entrapment after open reduction and volar plate fixation of a pediatric distal forearm fracture is reported. This complication may also occur secondarily after open reduction and volar plating of the distal radius.

The management of pink pulseless hands in paediatric supracondylar humerus fractures: A systematic review and meta-analysis.

INTRODUCTION: The management of the pink pulseless hand (PPH) following paediatric supracondylar humerus fractures remains controversial, with no clear consensus between orthopaedic management and early neurovascular surgical exploration. This study aimed to compare outcomes of these strategies through systematic review and meta-analysis. METHODS: A systematic review was conducted in accordance with PRISMA guidelines. Multiple electronic databases were searched up to December 2024. Retrospective cohort studies evaluating management of PPH in paediatric supracondylar fractures were included. P

Posttraumatic Nasal Deformities: Correcting the Crooked and Saddle Nose.

The nose is frequently traumatized in facial injuries and this often results from motor vehicle accidents, sports-related injuries, and altercations. Subsequently, posttraumatic nasal deformity is one of the most common reasons that patients seek consultation in the doctor's office. Depending on the type of nasal deformities, this can result in functional impairment and aesthetic problems. Two challenging problems to be addressed in the posttraumatic nose include the crooked nose deformity and the saddle nose deformity. The numerous publications on these two topics attest to the exacting surgi

Percutaneous Reduction of Displaced Radial Neck Fractures Achieves Better Results Compared With Fractures Treated by Open Reduction.

There is uniform agreement in the literature that radial neck fracture with <30 degrees of angulation and minimal translation in younger children can be managed by casting alone, without the need for closed reduction. For more severe fractures, closed manipulation techniques followed by percutaneous reduction technique (in case closed reduction fails) should be performed before switching to an open reduction technique. Debate still exists regarding the best treatment protocol for severely displaced fractures. Although some surgeons favor open reduction with the argument that repeat manipulatio

A Bundled Payment Model for Pediatric Distal Radius Fractures: Defining an Episode of Care.

BACKGROUND: Distal radius fractures are the most common fracture of childhood, occurring in ∼1 per 100 children annually. Given the high incidence of these fractures, we explored feasibility of a bundled payment model. We determined the total treatment costs for each child and identified components of fracture management that contributed to variations in cost. METHODS: We retrospectively reviewed all hospital and physician costs related to the treatment of closed distal radius fractures at a large academic children's hospital. We included all children age 2 to 15 years treated by an orthopaed

Transcondylar traction as a closed reduction technique in vertically unstable pelvic ring disruption.

Little information is provided in the literature describing an efficient reduction technique for pelvic ring disruption. The aim of this study is to assess the use of the transcondylar traction as a closed reduction technique for vertically unstable fracture-dislocations of the sacro-iliac joint. Twenty-four pelvic ring disruptions were treated with attempted closed reduction followed by percutaneous screw fixation. Transcondylar traction was used as a closed reduction technique. Closed reduction to within 1 cm of residual displacement was obtained in all cases. No incidence of infection, dige
