Precision Sedation for Gastrointestinal Endoscopy in Older Adults: Brain Vulnerability and Recovery.

Older adults undergoing gastrointestinal endoscopy are particularly susceptible to oversedation, hypotension, hypoventilation, hypoxemia, and delayed recovery because physiological and cognitive reserve decline with age. Precision sedation provides a practical approach to these problems by tailoring drug selection, dose titration, monitoring, and supportive measures to the individual patient and the requirements of the procedure. Propofol remains widely used because of its rapid onset and predictable recovery, although cardiorespiratory depression remains dose dependent. Remimazolam and ciprof…

Aging as a Continuation of Development: A Hypothesis and Framework.

Prevailing theories of cognitive aging depict late life as a period of compensatory decline-an effort to preserve performance despite progre … ctions across longitudinal, neuroimaging, and computational domains and reinterpret constructs like cognitive reserve as expressions of this developmental reallocation. By reframing aging as purposeful maturation rather than compensation, the adaptive-aging hypothesis positions late-life cognition as a distinct, evolutionarily honed phase of human development.

Adult ADHD as a risk factor for dementia: integrating longitudinal evidence, mechanistic insights, and the role of stimulant treatment.

Adult attention-deficit/hyperactivity disorder (ADHD) is increasingly recognized as a persistent neurodevelopmental condition with long-term … thways linking ADHD to neurodegeneration. Psychiatric comorbidities, lifestyle factors, and reduced cognitive reserve may further amplify vulnerability, decreasing the brain's resilience to neuropathological insults. Preliminary data suggest that stimulant treatments, such as methylphenidate and amphetamine derivatives, may attenuate dementia risk by normalizing do …

Improvement of neurovascular function and cognitive impairment after STA-MCA anastomosis.

OBJECTIVE: In patients with severe steno-occlusive disease of a main cerebral artery, decreased cerebrovascular reserve (CVR) and cognitive impairment without causative focal lesions on magnetic resonance imaging have been reported. We examined ipsilateral and contralateral CVR and cognition before and after superficial temporal artery-middle cerebral artery (MCA) anastomosis operation in patients with internal carotid artery (ICA) or MCA occlusion. METHODS: In 30 patients with ICA or MCA occlusion, cognitive impairment, and decreased CVR, we examined cognition, CVR, leukoaraiosis grades, and

Transient elevation of synaptosomal mitoenergetic proteins and Hsp70 early in a rat model of chronic cerebrovascular hypoperfusion.

Chronic cerebral hypoperfusion (CCH) might account for the cognitive deficits associated with vascular cognitive impairment, but the mechanisms of hypoperfusion insulting to the cognition remain obscure. In the present study, Wistar rats underwent permanent occlusion of bilateral common carotid arteries to induce CCH. 2D-DIGE combined with MALDI-TOF MS was applied to determine the proteins that were differentially expressed in synaptosomes of prefrontal cortex and hippocampus. ATPsynβ, NDUFS1, UQCRC1 and Hsp70 were elevated both in synaptosomes of cortex and hippocampus at week 2 after operati

Thalamic volume alterations mediate glymphatic function and cognitive dysfunction in Alzheimer's disease.

INTRODUCTION: Glymphatic dysfunction is implicated in Alzheimer's disease (AD), but its mechanism remains unclear. This study investigated the associations of the analysis along the perivascular space (ALPS) index, brain reserve, and cognitive outcomes across the AD continuum. METHODS: This study enrolled two cohorts from the local hospital (n = 95) and the Alzheimer's Disease Neuroimaging Initiative (ADNI; n = 178). We calculated the diffusion tensor image ALPS to assess the whole-brain glymphatic function and evaluated its associations with cognition and brain structure. RESULTS: A higher

Regression-based cognitive change norms applied in biochemically defined predementia Alzheimer's disease.

OBJECTIVE: We aim to develop 2-year cognitive change norms for adults ages 41-84 for six cognitive tests, and to evaluate these norms in groups with AD biomarkers. BACKGROUND: Practice effects are common in repeated neuropsychological testing. Not accounting for practice effects may obscure cognitive decline in early Alzheimer's disease (AD). METHOD: We developed standardized regression-based change norms from normative samples consisting of healthy controls from the Dementia Disease Initiation study (n = 125), the Trønderbrain study (n = 57), and the Gothenburg mild cognitive impairment (MC

[Cognitive aging of bipolar patients].

Bipolar disorder is a severe, recurrent mood disorder, associated with a higher rate of morbidity and mortality. A new population of aging b … eless this cognitive decline will be variable from one patient to another. This can be explained by cognitive reserve which reflects the brain's capacity to endure neuropathology, and minimize clinical manifestations. The second hypothesis is the neuroprogression paradigm that offers an explanation on how a cyclic disorder such as bipolar disorder could evolve prog …

Compensatory cerebellar activation during fluid intelligence processing following mild traumatic brain injury.

Cognitive outcome following mild traumatic brain injury (mTBI) vary widely, with many individuals experiencing long-term impairments associated with frontoparietal network dysfunction. Mild TBI patients have demonstrated functional reorganization, suggesting an expansion of activation to cerebellar regions during specific executive functions. In this study, we investigated cerebellar involvement in fluid intelligence processing using a novel fMRI paradigm based on Raven's Progressive Matrices in 51 acute mTBI patients and 61 healthy controls. Despite comparable task accuracy, mTBI patients exh

Long-Term Neurologic Consequences following Fetal Growth Restriction: The Impact on Brain Reserve.

BACKGROUND: Fetal growth restriction (FGR) corresponds to the fetus's inability to achieve an adequate weight gain based on genetic potential and gestational age. It is an important cause of morbidity and mortality. SUMMARY: In this review, we address the challenges of diagnosis and classification of FGR. We review how chronic fetal hypoxia impacts brain development. We describe recent advances on placental and fetal brain imaging using magnetic resonance imaging and how they offer new noninvasive means to study growth restriction in humans. We go on to review the impact of FGR on brain integ

Trait characteristics of midfrontal theta connectivity as a neurocognitive measure of cognitive control and its relation to general cognitive abilities.

Understanding the neurocognitive basis of cognitive control and its relationship with general cognitive ability is a key challenge in individual differences research. This study investigates midfrontal theta connectivity as a neurocognitive marker for individual differences in cognitive control. Using electroencephalography, we examined midfrontal global theta connectivity across three distinct cognitive control tasks in 148 participants. Our findings reveal that midfrontal theta connectivity can be modeled as a trait-like latent variable, indicating its consistency across tasks and stability

Management of older adult emergency general surgery patients in the intensive care unit: What you need to know.

Older adults undergoing emergency general surgery represent a rapidly growing and clinically vulnerable population frequently requiring perioperative management in the surgical intensive care unit. Age-associated physiologic decline, multimorbidity, frailty, and reduced cognitive and functional reserve contribute to the increased susceptibility to surgical and nonsurgical complications, prolonged surgical intensive care unit length of stay, cognitive and functional decline, and mortality of the critically ill older adult. Despite the high acuity and complexity of this population, geriatric-spe

Consensus Paper: Cerebellum and Ageing.

Given the key roles of the cerebellum in motor, cognitive, and affective operations and given the decline of brain functions with aging, cerebellar circuitry is attracting the attention of the scientific community. The cerebellum plays a key role in timing aspects of both motor and cognitive operations, including for complex tasks such as spatial navigation. Anatomically, the cerebellum is connected with the basal ganglia via disynaptic loops, and it receives inputs from nearly every region in the cerebral cortex. The current leading hypothesis is that the cerebellum builds internal models and

Role of Polygenic Risk Scores in Predicting Cognitive Functioning after Mild Traumatic Brain Injury: A TRACK-TBI Study.

Patients with traumatic brain injury (TBI) and Glasgow Coma Scale scores of 13-15 (historically called mild TBI [mTBI]) commonly experience … dependent contributions to outcome prediction. The mediation model findings may support theories of cognitive reserve, which propose that individuals with stronger preinjury cognitive processing abilities (often estimated by educational history) can better compensate for TBI. Moreover, findings indicate that PRS may contribute modestly to multivariable models predi …
