[A Case of Ascending Colon Cancer Showing Marked Reduction of Ascites by Bevacizumab Combination Chemotherapy].

A 68-year-old woman presented to our hospital with abdominal fullness. Computed tomography(CT)revealed ascites and massive tumors in the abdominal cavity. She was diagnosed with ascending colon cancer with peritoneal dissemination and ovarian metastasis. After ileostomy, panitumumab plus mFOLFOX6 therapy was initiated, but it was discontinued due to adverse events. As the ascites rapidly increased, her chemotherapy was changed to bevacizumab(BV)plus FOLFIRI. BV combination therapy resulted in a dramatic decrease in ascites and improved her quality of life, whereas the therapy did not reduce th

Intramural hematoma of the ascending colon leading to intraperitoneal hemorrhage: a possible post-polypectomy complication.

A 61-year-old male on anticoagulant therapy underwent an endoscopic multiple colon polypectomy in our Department according to established procedures. Several days later he developed an intramural colonic hematoma, which burst into the peritoneal cavity leading to fatal intra-peritoneal hemorrhage. Herein we present the case and discuss the causative role of the anticoagulant therapy but also of the endoscopic procedure per se.

Ascending colon rotation following patient positional change during CT colonography: a potential pitfall in interpretation.

OBJECTIVE: To investigate the degree and pattern of ascending colonic rotation as patients moved from supine to prone positions during CTC. METHODS: A search of our CTC and colonoscopy database found 37 patients (43 eligible lesions) who fulfilled the following criteria: colonoscopy-proven sessile polyps ≥ 6 mm in the straight mid-ascending colon, lesion visualisation in both supine and prone CTC, and optimal colonic distension. A coordinate system was developed to designate the polyp radial location (°) along the luminal circumference, unaffected by rotation of the torso. The degree/directio

The retroperitoneal surface in distal caecal and proximal ascending colon carcinoma: the Cinderella surgical margin?

BACKGROUND: Mesorectal margin tumour involvement is a predictor of local recurrence in rectal carcinoma and an indication for postoperative radiotherapy in suitable patients. However, the prevalence of non-peritonealised surgical margin involvement in ascending colon carcinoma is unknown. AIMS: To test the hypothesis that retroperitoneal surgical margin (RSM) tumour involvement occurs in distal caecal and proximal ascending colon carcinoma. METHODS/RESULTS: One hundred right hemicolectomy specimens, removed for adenocarcinoma of the caecum or proximal ascending colon, were studied. During ro

Advanced mucinous adenocarcinoma of the ascending colon with duodenal invasion: a surgical and diagnostic challenge.

Mucinous adenocarcinoma (MAC) is a distinct subtype of colorectal cancer that is more often diagnosed at an advanced stage and has been associated with greater local invasiveness and a less favourable response to chemotherapy. We report a man in his 80s who presented with progressive weight loss, abdominal pain and altered bowel habits. Diagnostic evaluation revealed a large ascending colon mass extending to the hepatic flexure, with direct duodenal invasion causing high-grade obstruction. The patient underwent an en bloc extended right hemicolectomy with duodenal resection and reconstruction.

Postoperative Pain Due to a Retained Needle in the Mesentery of the Ascending Colon: A Case Report.

A plethora of reports have previously been published concerning the removal of intra-abdominal foreign bodies. However, in most of these cases, an acute inflammatory reaction occurred. In emergency medicine, accidental sewing needle ingestion is a frequent presentation. However, it is extremely uncommon for the needle to be inserted through the abdominal wall and remain silent inside the abdominal cavity. We hereby report a case of incidental finding of a needle retrieved from the antimesenteric intestinal border of the ascending colon in a 41-year-old female patient who presented to our hospi

An unusual case of gallstone ileus within the cecum and ascending colon: a case report.

Gallstone ileus is a rare cause of intestinal obstruction. Due to long-standing inflammation of the gallbladder, fistulization can occur within nearby structures, most commonly to the duodenum or hepatic flexure of the colon. Through these fistulas, a stone can migrate and result in a small bowel obstruction or a large bowel obstruction. This case exemplifies the diagnosis and treatment of gallstone ileus, along with potential complications due to stone migration. Early recognition and treatment of gallstone ileus is important, as stone migration can lead to increased mortality with delayed di

Experience of surgical treatment in a granular cell tumor in the ascending colon: a case report.

We report a case about successful surgical treatment of a granular cell tumor in the ascending colon. A 36-year-old man underwent screening colonoscopy. An endoscopic examination revealed a 10-mm yellowish and hemispheric mass in the ascending colon, and lower endoscopic ultrasonography revealed a hypoechoic-to-isoechoic mass invaded the submucosal layer. The mass was suspected to be a colonic carcinoid tumor. Based on the preoperative evaluation, endoscopic complete resection was considered difficult. Therefore, the lesion was removed via laparoscopic right hemicolectomy. Histological examina

Perforation of the ascending colon during implantation of an indwelling peritoneal catheter: a case report.

BACKGROUND: Tunneled peritoneal drainage catheters are described as an effective and relatively safe method in the management of malignant and non-malignant refractory ascites. Therapeutic advantages, linked to their use, are self-management of ascites and palliative care at home. Complications occur rarely. We describe an ascending colon perforation after implantation of a peritoneal drainage in a patient with refractory ascites due to liver cirrhosis. CASE PRESENTATION: The 68-year-old male was admitted to the intensive care unit due to severe community acquired pneumonia. The ascites drain

Volvulus of the ascending colon due to failure of zygosis: A case report and review of the literature.

INTRODUCTION: Volvulus of the mobile ascending colon is a serious complication of a subtle embryological abnormality that frequently goes unrecognised. There has been renewed interest in the development of the mesentery because of its relevance to the contemporary management of surgical diseases. This case is presented to illustrate the dire consequences of missing this diagnosis and to review the fascinating embryology of the condition as well as its clinical implications. PRESENTATION OF CASE: We report on a 23-year-old male who presented with signs and symptoms of distal small bowel obstru

Rare Skin Fistulas Relating to Ascending Colonic Carcinoma: An Unusual Mode of Revelation.

Chronic or nonhealing wounds is a complex disease influenced by a multitude of factors, such as infection, ischemia, malnutrition, and diabetes and infrequently relates to retroperitoneal carcinoma. We present a case of an adenocarcinoma of ascending colon in a 68-year-old male who had lumbago and waist fistulas with retroperitoneal abscesses preceding other signs or symptoms of colonic malignancy. Supplemental information regarding the diagnosis and treatment of nonhealing wounds and colon carcinoma has also been included in the report. Adenocarcinoma of ascending colon is rarely associated w

Adenocarcinoma of the ascending colon in a 31-year-old pregnant woman: A case report.

RATIONALE: Colorectal cancer (CRC) is the 2nd most common type of cancer in females and the 3rd in males, worldwide. It occurs rarely during pregnancy and is often associated with poor prognosis, due to the unspecific manifestations until advanced stage. Majority of CRC are localized in the rectum (63%) and the sigmoid colon (20%) during pregnancy. PATIENT CONCERNS: In thisstudy, we report the case of a pregnant woman who was diagnosed with adenocarcinoma of the ascending colon at her 33rd gestational week. She was referred to our department from local hospital with low fever and right-sided

Anomalous inferior mesenteric artery supplying the ascending, transverse, descending, and sigmoid colons.

We have encountered in our anatomical practice the first case and an extremely rare second case in which the ascending, transverse, descending, and sigmoid colons were supplied by the inferior mesenteric artery. The causes of colic artery anomalies are generally explained in conjunction with the development of the superior mesenteric artery, which is intimately related to embryonic elongation and midgut rotation. However, this embryological model was inapplicable to both cases. This difficulty motivated us to seek possible relationships with reported anomalous inferior mesenteric arteries in a

Metastatic osteosarcoma to the stomach and ascending colon in a pediatric patient causing gastrointestinal hemorrhage.

Osteosarcoma metastasis to the gastrointestinal tract is a rare phenomenon (Horiuchi A, Watanabe Y, Yoshida M, et al.: Metastatic osteosarcoma in the jejunum with intussusception: report of a case. Surg Today 2007;37:440-2). Gastrointestinal metastases may cause intussusception, bowel obstruction, or hemorrhage (Horiuchi A, Watanabe Y, Yoshida M, et al.: Metastatic osteosarcoma in the jejunum with intussusception: report of a case. Surg Today 2007;37:440-2; Chondramohan K, Somanathan T, Kusamakumary P: Metastatic osteosarcoma causing intussusception. J Pediatr Surg 2003;38(E44):1-3; Hung GY, C

A case of benign schwannoma of the ascending colon treated with laparoscopic-assisted wedge resection.

Isolated colonic schwannomas are rare gastrointestinal mesenchymal tumors. Only a small number of cases have been reported. Occurrence of these tumors is more common in the stomach than in the large intestine. These spindle cell lesions are distinct from leiomyoma, leiomyosarcoma, and gastrointestinal stromal tumors because the tumor cells have a distinct immunophenotype, with strong diffuse positivity for S-100 and vimentin, as well as corroborative negative staining of CD117 and smooth muscle markers. We present a case of colonic schwannoma in a 70-year-old woman who had no specific symptoms
