Is a care pathway for enhanced recovery after colorectal surgery environmentally responsible?

INTRODUCTION: Above and beyond the environmentally responsible operating theater, the environmental impact of the pathways of surgically treated patients seems essential but has seldom been considered in the literature. On a parallel track, enhanced recovery programmes (ERP) programs are presently deemed a standard of care. The objective of this review is to determine the carbon footprint of the ERP approach in colorectal surgery. METHOD: This a narrative review based on articles referenced in PubMed. Our search was centered on the environmental impact of an ERP in the context of colorectal s…

Impact of aspirin discontinuation on thrombotic complications in laparoscopic colorectal cancer surgery.

BACKGROUND: The number of patients taking antiplatelet therapy is increasing. However, there is no definitive guideline for the perioperative management of antiplatelet therapy. Conventionally, the discontinuation of antiplatelet drugs has been the basic treatment as perioperative management. Therefore, we investigated the risk of discontinuing aspirin concerning thrombotic complications in laparoscopic colorectal cancer surgery. METHODS: Between January 2015 and December 2019, a total of 729 patients underwent laparoscopic colorectal cancer surgery in Toyonaka Municipal Hospital. Sixty-four

Creation and Institutional Validation of a Readmission Risk Calculator for Elective Colorectal Surgery.

BACKGROUND: Readmissions reflect adverse patient outcomes, and clinicians currently lack accurate models to predict readmission risk. OBJECTIVE: We sought to create a readmission risk calculator for use in the postoperative setting after elective colon and rectal surgery. DESIGN: Patients were identified from 2012-2014 American College of Surgery-National Surgical Quality Improvement Program data. A model was created with 60% of the National Surgical Quality Improvement Program sample using multivariable logistic regression to stratify patients into low/medium- and high-risk categories. The

The Role of Oral Antibiotic Preparation in Elective Colorectal Surgery: A Meta-analysis.

OBJECTIVES: To compare the impact of the use of oral antibiotics (OAB) with or without mechanical bowel preparation (MBP) on outcome in elective colorectal surgery. SUMMARY BACKGROUND DATA: Meta-analyses have demonstrated that MBP does not impact upon postoperative morbidity or mortality, and as such it should not be prescribed routinely. However, recent evidence from large retrospective cohort and database studies has suggested that there may be a role for combined OAB and MBP, or OAB alone in the prevention of surgical site infection (SSI). METHODS: A meta-analysis of randomized controlled…

Anatomical study of the left colic artery in laparoscopic-assisted colorectal surgery.

BACKGROUND: It is important for lymph node dissection around the inferior mesenteric artery (IMA) with preservation of the left colic artery (LCA) to be aware of the track and the length of the LCA. We aimed to investigate the branching pattern and trajectory of LCA and measure the distances from the root of the IMA to the origin of the LCA (D mm) and from the origin of LCA to intersection of LCA and IMV (d mm) during laparoscopic left-sided colorectal operations. METHODS: We analyzed 106 patients who underwent laparoscope-assisted left-side colorectal surgery during laparoscopic surgery. The…

[CURRENT STATUS OF AND OUTLOOK FOR THE LATEST MINIMALLY INVASIVE SURGERY FOR COLORECTAL DISEASES].

Randomized clinical trials (RCTs) comparing laparoscopic surgery (LS) with open surgery (OS) in patients with colon cancer showed no statistical differences in, safety, and long-term outcomes. LS for colon cancer has become a standard procedure. LS for rectal cancer has also shown advantages, although RCTs showed no significant differences between groups in the long-term outcome. The magnification of laparoscopy enables surgeons to operate precisely in the narrow pelvis while sparing the autonomic nerves and nerves to the anal sphincter. Good visualization of the pelvis offers better education

The treatment of pilonidal disease: guidelines of the Italian Society of Colorectal Surgery (SICCR).

The Italian Society of Colorectal Surgery (SICCR.) has prepared clinical practice guidelines to help its members to optimize the treatment of pilonidal disease, a very common condition, especially among young people, and therefore of great importance on a socioeconomic level. The SICCR committee of experts on pilonidal disease analyzed the international literature and evaluated current evidence. Nonoperative management includes gluteal cleft shaving, laser epilation as well as fibrin glue and phenol injection: reported healing rates and recurrence incidence are satisfactory but the majority of…

What is the role of mechanical bowel preparation in patients undergoing colorectal surgery?

BACKGROUND: Most surgeons use mechanical bowel preparation before performing operations on the colon and rectum. The aim of this study is to … vidence in the literature to support the use of mechanical bowel preparation in patients undergoing colorectal surgery. Hence, there is a need for clinical trials comparing the more traditional, aggressive forms of bowel preparation (e.g., polyethylene glycol solutions, sodium phosphate) with either no preparation or simpler techniques, such as a single phosphate en …
