Hepatitis C virus (HCV) infection is often associated with abnormal immunological responses. We describe four patients with vasculitic neurological signs and symptoms following HCV infection. A 56-year-old woman with HCV infection developed peripheral neuropathy characterized by asymmetric distal painful hypesthesia, dysesthesia and moderate motor weakness of the lower limbs. A local defense mechanism in bacterial meningitis was evaluated in humans by measuring complement-mediated opsonic activity (CMOA) in normal and infected cerebrospinal fluid (CSF) with a complement-dependent phagocytic bactericidal assay. A patient with renal failure and light-chain proteinuria showed roentgenographic evidence of retroperitoneal fibrosis on further examination. Diffuse hyperglobulinemia, low levels of C3 and C4, and elevated C1q binding were noted. Biopsy specimens of the kidney and retroperitoneal tissue disclosed a dense pleomorphic cellular infiltrate with a preponderance of plasma cells. Renal function improved slowly after ureterolysis and institution of prednisone therapy. The case emphasizes the immunologic features of the early phase of idiopathic retroperitoneal fibrosis.

Fifteen premature newborns with hyaline membrane disease causing acute respiratory distress were evaluated for complement activation. Thirteen patients with concomitant chronic salmonellosis and schistosomiasis and with the nephrotic syndrome were studied. Of the seven patients' sera examined, all had markedly low complement C3 and near normal C4 levels. This indirectly suggests an active role of Salmonella endotoxin in the etiology of the nephrosis.
