[Remissions in affective disorders: epidemiology, psychopathology, clinical and social prognosis, treatment].

OBJECTIVE: Although there is some evidence on heterogeneity of remissions in affective disorders (partial and full), many aspects of their comparative epidemiology, psychopathology, comorbidity and treatment are still unclear. According to our hypothesis, partial/symptomatic and full/syndromal remissions are not just consecutive stages of disease with spontaneous or treatment related interchange, but independent clinical entities with different essence. The distinction reflects a predominant pattern of disease development, based on interaction of two comorbid axes -- recurrent depression and p

Brain fluorodeoxyglucose PET in adrenoleukodystrophy.

OBJECTIVE: To investigate the cerebral glucose metabolism in subjects with X-linked adrenoleukodystrophy (X-ALD) by using brain [(18)F]-fluo … s. With respect to personality, we found that patients with X-ALD usually present with an obsessive-compulsive personality disorder on the MCMI-III, with significant (p < 0.05) correlation between glucose uptake in ventral striatum and severity of score on the obsessive-compulsive subscale. CONCLUSIONS: We examined cerebral glucose metabolism using FDG-PET in a cohort of patien …

Knowledge and competency standards for specialized cognitive behavior therapy for adult obsessive-compulsive disorder.

Obsessive-Compulsive Disorder (OCD) is a leading cause of disability world-wide (World Health Organization, 2008). Treatment of OCD is a specialized field whose aim is recovery from illness for as many patients as possible. The evidence-based psychotherapeutic treatment for OCD is specialized cognitive behavior therapy (CBT, NICE, 2005, Koran and Simpson, 2013). However, these treatments are not accessible to many sufferers around the world. Currently available guidelines for care are deemed to be essential but insufficient because of highly variable clinician knowledge and competencies specif

A study of the Repeated Actions Diary in patients suffering from obsessive compulsive disorder.

The structured Repeated Action Diary (RAD) collects in vivo data on compulsions and their various characteristics. Certain compulsions (i.e., those ending because the patient feels certain that it is safe to stop) are then compared with uncertain compulsions. The compulsion profile in patients with obsessive compulsive disorder (OCD) was assessed by using the RAD. Thirty-two patients from two sources participated in the study. Before pooling the two subgroups, we checked that they did not differ significantly with regard to demographic and clinical variables. Patients reported several categori

Trait and behavioral prototypes of personality disorder.

The author reports a survey of the clinical features used by psychiatrists to diagnose personality disorders. The purpose was to develop definitions of these disorders and to identify behavioral features that could be used as diagnostic criteria. Psychiatrists rated trait and behavioral items describing each disorder for the degree to which they were prototypical of the disorder. Judgments of the prototypical quality of traits and behaviors were found to be equally reliable. Given that behavioral criteria require less inferential judgment, subsequent revisions of criteria to assess personality

Childhood maltreatment and personality disorders in adolescents and adults with psychotic or non-psychotic disorders.

INTRODUCTION: While the attention to personality disorders (PD) and childhood maltreatment (CM) has grown in recent years, there remains limited understanding of the prevalence and distinctions of PD and CM in clinical populations of Chinese adolescents in comparison to adults. METHODS: A total of 1,417 participants were consecutively sampled from patients diagnosed with either psychotic or non-psychotic disorders in the psychiatric and psycho-counseling clinics at Shanghai Mental Health Center. The participants were categorized into two groups based on their age: adolescents (aged 15-21 year

The NEO Five-Factor Inventory: latent structure and relationships with dimensions of anxiety and depressive disorders in a large clinical sample.

The present study evaluated the latent structure of the NEO Five-Factor Inventory (NEO FFI) and relations between the five-factor model (FFM) of personality and dimensions of DSM-IV anxiety and depressive disorders (panic disorder, generalized anxiety disorder [GAD], obsessive-compulsive disorder, social phobia [SOC], major depressive disorder [MDD]) in a large sample of outpatients (N = 1,980). Exploratory structural equation modeling (ESEM) was used to show that a five-factor solution provided acceptable model fit, albeit with some poorly functioning items. Neuroticism demonstrated significa

Laxative abuse among women with eating disorders: an indication of psychopathology?

OBJECTIVE: The results of the scant research on laxative abuse among women with eating disorders suggest that laxative abuse is a diagnostic indicator of greater psychopathology. We further investigated the relationship of history of laxative abuse to eating and related attitudes, impulsivity, and personality pathology. METHOD: Women assessed in an outpatient clinical setting and diagnosed with anorexia nervosa, binge-eating/purging type (n = 51) or bulimia nervosa, purging type (n = 280) completed measures of laxative abuse, eating and related attitudes, and personality psychopathology at in

Adaptation and validation of the Swedish Five Factor Obsessive-Compulsive Inventory-Short Form in adults with and without eating disorder psychopathology.

BACKGROUND: Obsessive-compulsive personality disorder (OCPD) traits are highly prevalent in eating disorder populations and linked to greater symptom severity and poorer treatment outcomes, yet no validated dimensional measure of these traits exists for use in Swedish-speaking populations or in eating disorder contexts. The Five-Factor Obsessive-Compulsive Inventory-Short Form (FFOCI-SF) is a brief, dimensional instrument designed to assess OCPD-related traits within the five-factor model framework. This study aimed to adapt and validate the Swedish FFOCI-SF and to examine its psychometric fun…

Self-injurious behavior and attempted suicide in purging bulimia nervosa: associations with psychiatric comorbidity.

BACKGROUND: Few studies, to date, have investigated the relationship between self-damaging behavior and the presence of comorbid psychiatric diagnoses in eating disorders. The aim of the present study was to investigate the axis I and II comorbidity in subjects with bulimia nervosa who report self-injurious behavior and/or suicide attempt. METHODS: The subjects were 95 patients with purging type bulimia nervosa who underwent a clinical evaluation assessing the presence of self-injurious behavior and suicide attempts, comorbidity for axis I and II psychiatric disorders and temperament. Impulsive and compulsive behaviors among Danish patients with Parkinson's disease: prevalence, depression, and personality.

INTRODUCTION: Dopaminergic medication administered to ameliorate motor symptoms of Parkinson's disease is associated with impulse control disorders, such as pathological gambling, hypersexuality, compulsive buying, and binge eating. Studies indicate a prevalence of impulse control disorders in Parkinson's disease of 6-16%. OBJECTIVE: To estimate the prevalence of impulsive and compulsive behaviors among Danish patients with Parkinson's disease and to explore the relation of such behavioral disorders to depression and personality. METHODS: 490 patients with Parkinson's disease (303 males), id

Psychopathological factors and personality dimensions on dysfunctional eating behaviors in a sample of individuals with obesity.

INTRODUCTION: Obesity and eating disorders are considered to be part of a broad spectrum of disorders associated with weight issues and maladaptive eating styles. Several studies show that psychopathological and personality characteristics contribute to the development and maintenance of obesity and dysfunctional eating behaviors, showing a bidirectional relationship. The purpose of this study was to understand the role of psychopathological factors and personality dimensions on dysfunctional eating behaviors in a sample of individuals with obesity. METHODS: The study was conducted with 96 in

'Autism is the Arena and OCD is the Lion': Autistic adults' experiences of co-occurring obsessive-compulsive disorder and repetitive restricted behaviours and interests.

Repetitive behaviours and interests are a hallmark feature of autism. It is very common for autistic people to experience mental health difficulties, such as obsessive-compulsive disorder. Previous research has investigated similarities and differences between obsessive-compulsive disorder symptoms and repetitive behaviours in autism through questionnaires and observation studies. This is the first study to interview autistic adults about their personal experiences of differentiating between obsessive-compulsive disorder symptoms and repetitive behaviours related to autism. We interviewed 15 a

Overlapping Spectrum of Impulsivity and Compulsivity Across Psychiatric Disorders: A Narrative Review on Dimensional Perspectives.

Impulsivity and compulsivity are complex constructs that underpin a wide range of psychiatric disorders. While traditionally viewed as opposing dimensions, emerging evidence suggests they exist on an overlapping spectrum, influencing diagnosis and treatment. Impulsivity is characterized by poorly planned, premature responses aimed at achieving gratification, while compulsivity involves repetitive behaviors driven by anxiety relief. Both constructs share common neurobiological substrates, such as impaired response inhibition and urgency, but differ in their executive dysfunction patterns. Psych

Feeling unreal: 30 cases of DSM-III-R depersonalization disorder.

OBJECTIVE: In contrast to the recent surge of interest in other dissociative disorders, DSM-III-R depersonalization disorder has not been thoroughly investigated and characterized. The authors systematically elucidated its phenomenology, comorbidity, traumatic antecedents, and treatment history. METHOD: Thirty adult subjects (19 women and 11 men) were consecutively recruited and administered various structured and semistructured interviews as well as the self-rated Dissociative Experiences Scale. An age- and sex-matched normal comparison group was also recruited. RESULTS: The mean age at ons
