Deformation behaviour and damage accumulation of cortical bone specimens from the equine tibia under cyclic loading.

Despite its clinical importance, the fatigue behaviour of cortical bone has not been examined as widely as its static behaviour. In the present study, specimens from the tibiae of horses have been subjected to load-controlled single step tests. The cyclic deformation behaviour was described by the development of stress-strain hysteresis parameters over the lifetime. The fatigue behaviour of bone is characterised by cyclic softening which is most distinctive towards the end of the lifetime. The microstructural damage accumulated during cyclic loading results in a loss of stiffness, asymmetrical…

Relationships between material properties and CT scan data of cortical bone with and without metastatic lesions.

Breast, prostate, lung, and other cancers can metastasize to bone and lead to pathological fracture. To lay the groundwork for new clinical techniques for assessing the risk of pathological fracture, we identified relationships between density measured using quantitative computed tomography (rhoQCT), longitudinal mechanical properties, and ash density (rhoAsh) of cortical bone from femoral diaphyses with and without metastatic lesions from breast, prostate, and lung cancer (bone with metastases from six donors; bone without metastases from one donor with cancer and two donors without cancer). …

Vitamin D receptor genotype is associated with cortical bone loss in Japanese patients with primary hyperparathyroidism.

Vitamin D receptor (VDR) genotype was determined in 66 Japanese patients with primary hyperparathyroidism (pHPT). In contrast to previous report showing an association between VDR genotype bb and the development of pHPT in a Swedish population, we did not find any differences in the frequencies of the VDR genotypes between pHPT and the control. The bone mineral density in the radius was significantly lower (Z score, -1.723 +/- 1.819) in bb genotype than in BB/Bb genotype (+0.255 +/- 2.344) in pHPT patients. Based on the fact that PTH requires vitamin D3 to take effect on calcium mobilization f

A murine skeletal adaptation that significantly increases cortical bone mechanical properties. Implications for human skeletal fragility.

Mov13 mice carry a provirus that prevents transcription initiation of the alpha 1(I) collagen gene. Mutant mice homozygous for the null mutation produce no type I collagen and die at mid-gestation, whereas heterozygotes survive to adulthood. Dermal fibroblasts from heterozygous mice produce approximately 50% less type I collagen than normal littermates, and the partial deficiency in collagen production results in a phenotype similar to osteogenesis imperfecta type I (an inherited form of skeletal fragility). In this study, we have identified an adaptation of Mov13 skeletal tissue that signific

Arthroscopic Posterior Glenoid Bone Block Using an Adapted Anterior Glenoid Guide and Independent Cortical Button Fixation.

Recurrent posterior shoulder instability associated with posterior glenoid bone loss remains challenging. Arthroscopic bone block procedures have gained popularity due to their minimally invasive nature and their ability to restore glenoid concavity while addressing associated intra-articular pathology. We describe a fully arthroscopic posterior glenoid bone block technique using a tricortical iliac crest autograft fixed with independent cortical buttons from the Glenoid Bone Loss-Advanced Instability System (Smith & Nephew Andover, MA, USA). A commonly available anterior glenoid guide is used

In vivo mechanical assessment of cortical bone rigidity enhances fracture discrimination beyond DXA in postmenopausal women.

DXA-derived areal BMD remains the clinical standard for assessing osteoporosis risk, yet it fails to identify over 75% of individuals who sustain fragility fractures. Direct in vivo mechanical assessment of cortical bone strength may address this diagnostic gap by capturing structural and material properties that govern whole-bone strength but are not reflected by BMD. We conducted a multicenter case-control study with cross-sectional exposure assessment to compare ulna flexural rigidity (EI), a biomechanical property correlated with whole-bone strength (R2 ≈ 0.99), estimated using cortical bo…

Retrospective cohort study analyzing temporal bone cortical thickness and perioperative complication rate, in pediatric cochlear implantation.

BACKGROUND: Cochlear implant fixation in pediatric patients can be challenging due to the thin cranial bone. The dura matter can be exposed by drilling a bony recess leading to possible complications. A minimally invasive newer fixation method might avoid such risks. OBJECTIVES: The study focus is to assess the feasibility of drilling a bony well adequate for cochlear implant receiver/stimulator device embedment in pediatric patients of different age groups. We also aim report the occurred complications and device failure rates using different surgical techniques for cochlear approach and fix

Multiscale dynamics analysis of lumbar vertebral cortical bone based on the Abaqus submodel finite element method.

The direct effect of macroscopic loads on the microstructure of bone tissue in a vibration environment is not yet known. Therefore, this study aims to investigate the macro- and micro-biomechanical properties of the lumbar spine system under dynamic loading in such an environment. We analyzed the dynamic characteristics of osteon by establishing a macro- and micro-scale model of the lumbar spine, using a submodel-specific boundary displacement method based on St. Venant's principle. Utilizing the results from the transient dynamic analysis of the entire lumbar spine as boundary conditions, thi

Sequential irradiation does not improve fatigue crack propagation resistance of human cortical bone at 15 kGy.

Sequential irradiation has been advocated for mitigating the reduction in fatigue properties of tendon compared to a single dose. However, to our knowledge, its capability of mitigating fatigue losses in bone is unknown. Recently, we reported that sequential irradiation did not mitigate losses in high-cycle S-N fatigue life of cortical bone at 15 kGy; however, it is unclear if sequential irradiation provides a benefit to fatigue crack propagation resistance. Our previous study also showed that radiation-induced collagen chain fragmentation and crosslinking increased from 0 to 15 kGy, suggestin…

Chronic kidney disease compromises structural and mechanical properties of maxillary cortical bone in a rat model.

PURPOSE: This study aimed to investigate the effects of chronic kidney disease (CKD) on the structural and mechanical properties of the maxillary and mandibular cortical bone. METHODS: The maxillary and mandibular cortical bones from CKD model rats were used in this study. CKD-induced histological, structural, and micro-mechanical alterations were assessed using histological analyses, micro-computed tomography (CT), bone mineral density (BMD) measurements, and nanoindentation tests. RESULTS: Histological analyses indicated that CKD caused an increase in the number of osteoclasts and a decrea…

Management of Adrenal Cortical Adenomas: Assessment of Bone Status in Patients with (Non-Functioning) Adrenal Incidentalomas.

Our aim is to analyse the bone profile in adults with (non-functioning) adrenal incidentalomas (AIs), specifically addressing the impact of autonomous cortisol secretion (ACS). This narrative review, based on a PubMed search from inception to February 2023 (case reports, non-ACS, and other secondary causes of osteoporosis were excluded), included 40 original studies, a total of 3046 patients with female prevalence (female:male ratio of 1921:1125), aged between 20.5 and 95.5 years old. This three decade-based analysis showed that 37 studies provided dual-energy X-ray absorptiometry (DXA) inform

Vertebral Fractures Occur Despite Control of Acromegaly and Are Predicted by Cortical Volumetric Bone Mineral Density.

CONTEXT: Recent studies suggest that cortical bone could also play a role in vertebral fracture (VF) development in acromegaly. OBJECTIVE: Evaluate the occurrence of VFs and their relationship to dual energy x-ray absorptiometry-derived bone parameters. METHODS: A single-center 2-year prospective study of acromegaly patients was conducted. Each subject had L1-4 spine, femoral neck and total hip (TH) areal BMD measured using DXA, and trabecular bone score (TBS) measurement performed. 3D Shaper™ was used to assess proximal femur trabecular and cortical volumetric (v)BMD, cortical surface (s)BM…

Material heterogeneity, microstructure, and microcracks demonstrate differential influence on crack initiation and propagation in cortical bone.

The recent studies have shown that long-term bisphosphonate use may result in a number of mechanical alterations in the bone tissue including a reduction in compositional heterogeneity and an increase in microcrack density. There are limited number of experimental and computational studies in the literature that evaluated how these modifications affect crack initiation and propagation in cortical bone. Therefore, in this study, the entire crack growth process including initiation and propagation was simulated at the microscale by using the cohesive extended finite element method. Models with h…

High incidence of early subtrochanteric lateral cortical atrophy after hip arthroplasty using bone-conserving short stem.

PURPOSE: Load transfer to the bone is believed to be more physiological around the short stem in total hip arthroplasty (THA). However, we found unusual bony remodeling around the shortened tapered stem. This study was performed to investigate the extent and frequency of this phenomenon and to find the possible risk factors of it. METHODS: Among 121 consecutive THA using the same short stem, 80 THAs were enrolled. Radiographic measurements were made using anteroposterior (AP) radiographs taken immediately and at two years after surgery. The thickness of the lateral cortex at the level of the

Mixed-mode I+II fracture characterization of human cortical bone using the Single Leg Bending test.

Mixed-mode I+II fracture characterization of human cortical bone was analyzed in this work. A miniaturized version of the Single Leg Bending test (SLB) was used owing to its simplicity. A power law criterion was verified to accurately describe the material fracture envelop under mixed-mode I+II loading. The crack tip opening displacements measured by digital image correlation were used in a direct method to determine the cohesive law mimicking fracture behavior of cortical bone. Cohesive zone modeling was used for the sake of validation. Several fracture quantities were compared with the exper…

Artificially low cortical bone mineral density in Turner syndrome is due to the partial volume effect.

UNLABELLED: We aimed to show that the decrease in the cortical bone mineral density (BMD) in the radius in Turner syndrome (TS) is artificially caused by the partial volume effect. We confirmed that the partial volume effect-corrected cortical BMD is not decreased in TS compared to in the healthy controls. Other factors are responsible for the increased fracture rate in TS. INTRODUCTION: Decreased cortical bone mineral density (BMD) has been reported in Turner syndrome (TS), using peripheral quantitative computerised tomography, and it is perceived as one of the major factors leading to incre…

Mechanical evaluation of a 4-mm cancellous "rescue" screw in osteoporotic cortical bone: a cadaveric study.

OBJECTIVES: Obtaining sufficient fracture fixation in osteoporotic bone is challenging. The purposes of the current study were (1) to compare the pullout strength of a 4-mm cancellous screw (cancellous screw group) with that of a 3.5-mm cortical screw (cortical screw group), and (2) to measure the pullout strength of a 4-mm cancellous screw placed as a rescue screw (rescue screw group) in a stripped 3.5-mm cortical screw (stripped screw group) hole while controlling for bone density and cortical thickness. METHODS: We inserted 4 screws, one from each experimental group, into 11 osteoporotic c
