Cholesterol granuloma in the petrous apex: case report and review.

Cholesterol granuloma in the petrous apex presents with various symptoms of cranial nerve dysfunction, so the selection for surgical treatment remains controversial. We report a 41-year-old woman with a cholesterol granuloma at the left petrous apex, which was totally resected via a combined middle fossa and posterior transpetrosal approach. In a review of 92 cases, including our case, treated for petrous apex cholesterol granuloma between 1990 and 2001, 38 were men (41.3%) and 54 were women (58.7%). The mean age of these patients was 37.4 years (males were 35.2 years, females were 39.4 years)

The Endoscopic Endonasal Transmaxillary Approach to Meckel's Cave Through the Inferior Orbital Fissure.

BACKGROUND: Surgical access to Meckel's Cave (MC) is challenging due to its deep location and surrounding important neurovascular structures. Currently existing endoscopic endonasal (EE) approaches require dissecting near the internal carotid artery (ICA) or require transposition of the pterygopalatine neurovascular bundle. OBJECTIVE: To describe a novel approach to access the anterolateral aspect of the MC using a minimally invasive EE route. METHODS: The EE transmaxillary transinferior orbital fissure approach was simulated in 10 specimens. The approach included an ethmoidectomy followed b

Adjunctive use of endoscopy during acoustic neuroma surgery.

OBJECTIVE/HYPOTHESIS: In specific clinical situations, endoscopes offer better visualization than the microscope during acoustic neuroma (vestibular schwannoma) surgery and can therefore decrease the incidence of the postoperative complications of cerebrospinal fluid (CSF) leakage and recurrence of tumor. This study was undertaken to determine if the use of adjunctive endoscopy provides complementary information to the operating surgeon during surgery for acoustic neuromas. METHOD: Seventy-eight patients with acoustic neuromas underwent tumor excision by two neurotologists (PAW., D.S.P.), tog

Securing the Puncture Depth via an Endodontic Stopper in Arthrocentesis of the Temporomandibular Joint: A Technical Note.

The reported complications after arthrocentesis of the temporomandibular joint are rare, which makes it to an outpatient and safe oral and maxillofacial surgical procedure. A penetration into the middle cranial fossa is an extremely rare but serious complication for the conventional needle arthrocentesis. The simple technical modification described herein could help to easily retain the puncture depth via an endodontic silicone stopper to avoid this rare complication.

[Urbach-Wiethe syndrome: hyalinosis and lipoidosis of the skin and mucous membranes as a neuroectodermosis].

The author reports a case of Wiethe-Urbach syndrome (dermal and mucosal hyalinosis). The diagnosis was based on characteristic clinical findings in the skin and mucosal membranes and microscopic examination of the skin. Coexistent central nervous system dysplasia was characterized by brain atrophy, especially atrophy of the subcortical layers and two horn-like calcifications on the floor of the middle cranial fossa derived, most likely, from the dura.

Transorbital transnasal endoscopic combined approach to the anterior and middle skull base: a laboratory investigation.

Orbital approaches provide significant trajectory to the skull base and are used with differently designed pathways. The aim of this study is to investigate the feasibility of a combined transorbital and transnasal approach to the anterior and middle cranial fossa. Cadaveric dissection of five silicon-injected heads was used. A total of 10 bilateral transorbital approaches and 5 extended endonasal approaches were performed. Identification of surgical landmarks, main anatomical structures, feasibility of a combined approach and reconstruction of the superior orbital defect were examined. Rod le

Petrosal Approach With Preservation of the Superior Petrosal Sinus (the Graceful Petrosal) for Resection of Giant Trigeminal Schwannoma: 2-Dimensional Operative Video.

Trigeminal schwannomas are benign tumors amendable to curative surgical resection.1 Excellent outcomes, with preservation and improvement of cranial nerve function, including trigeminal nerve function, have been reported with microsurgical resection through skull base approaches.2 Dumbell shaped tumors, involving the middle and posterior fossa, are more challenging.3 They are resected via a middle cranial fossa approach with the expanded Meckel cave providing access to the posterior fossa. However, tumors with a large caudal extension below the internal auditory meatus typically cannot be adeq

One burr-hole craniotomy: Modified presigmoid approach in Helsinki Neurosurgery.

BACKGROUND: In this video abstract, we present a one burr-hole craniotomy for a modified presigmoid approach developed in Helsinki Neurosurgery to access the space extended to both middle and posterior fossa. Thus, indications for this approach are lesions that extend to both middle and posterior fossa, petroclival tumors, basilar tip aneurysms located extremely low below the posterior clinoid process, trunk basilar aneurysms, and bypass procedures from the P2 segment of the posterior cerebral artery. The procedure is composed by three stages: a temporal and presigmoid craniotomy, a partial pe

A quantitative and descriptive approach to morphological variation of the endocranial base in modern humans.

The cranial base is one of the major foci of interest in functional craniology. The evolution and morphogenesis of this structure are still poorly known and rather controversial because of multifactorial influences and polyphasic stages. Endocranial dynamics are associated anteriorly with the upper facial structures, laterally with the mandibular system and midsagittally with brain development. In the present study, we investigated the endocranial morphology of modern humans using 3D landmark-based approaches, i.e. geometric morphometrics and Euclidean distance matrix analysis. The structure o

Zygomatic root approach.

CONCLUSIONS: The zygomatic root (ZR) approach provides improved intraoperative exposure of the key areas around the geniculate ganglion without a craniotomy, combining the advantages of middle cranial fossa (MCF) and transmastoid extralabyrinthine (TMEL) approaches. The ZR approach may be useful in cases of traumatic facial palsy, Bell's palsy, iatrogenic facial palsy, superior semicircular canal dehiscence and primary cholesteatoma. OBJECTIVES: To describe and evaluate the new ZR approach technique in the treatment of traumatic intratemporal facial nerve injuries. PATIENTS AND METHODS: This

[Facial nerve decompression].

Facial nerve palsy is usually managed conservatively, but in some cases may require surgical intervention. In cases in which the continuity of the nerve is not disrupted, decompression is the procedure of choice. No reports were found in the literature addressing this group separately. We report the results of 33 facial nerve decompressions conducted in the Sheba Medical Center through the years 1985-2002. Fifteen of our patients were operated on using the middle cranial fossa approach, 12 underwent mastoidectomy and 6 were treated using the combined middle cranial fossa mastoidectomy approach

Radiological assessment of skull base changes in children with syndromic craniosynostosis: role of "minor" sutures.

INTRODUCTION: This study aims to identify the premature synostosis of "major" and "minor" sutures of the four "sutural arches" of the skull and to perform a morphometric analysis in children with syndromic craniosynostosis in order to evaluate changes in the skull base linked with premature suture synostosis. METHODS: We reviewed multiplanar high-resolution CT images, implemented with 3D reconstructions, from 18 patients with complex syndromic craniosynostosis and compared them with 18 age-matched healthy subjects. We assessed the calvarial sutures and their extension to the skull base, and t

Repair of a large primary subtemporal encephalocele in a 3-year-old child: case report.

Primary encephaloceles (PEs) present only rarely in the temporal region; in the rare instance that they project through the floor of the middle fossa they are secondary. In this case report the authors report on the management of a giant PE extending through the floor of the middle fossa. An 8-month-old boy presented to the authors' service with a large PE projecting into his neck through a missing left middle fossa floor; the lesion was causing significant meta-, dys-, and hypoplasia of the structures of the anterolateral neck on that side. Surgical goals for this patient included the followin
