A Randomized Controlled Trial of Early versus Late Surgical Decompression for Thoracic and Thoracolumbar Spinal Cord Injury in 73 Patients.

Convincing clinical evidence exists to support early surgical decompression in the setting of cervical spinal cord injury (SCI). However, clinical evidence on the effect of early surgery in patients with thoracic and thoracolumbar (from T1 to L1 [T1-L1]) SCI is lacking and a critical knowledge gap remains. This randomized controlled trial (RCT) sought to evaluate the safety and efficacy of early (<24 h) compared with late (24-72 h) decompressive surgery after T1-L1 SCI. From 2010 to 2018, patients (≥16 years of age) with acute T1-L1 SCI presenting to a single trauma center were randomized to r

Arthroscopic subacromial decompression for subacromial shoulder pain (CSAW): a multicentre, pragmatic, parallel group, placebo-controlled, three-group, randomised surgical trial.

BACKGROUND: Arthroscopic sub-acromial decompression (decompressing the sub-acromial space by removing bone spurs and soft tissue arthroscopically) is a common surgery for subacromial shoulder pain, but its effectiveness is uncertain. We did a study to assess its effectiveness and to investigate the mechanism for surgical decompression. METHODS: We did a multicentre, randomised, pragmatic, parallel group, placebo-controlled, three-group trial at 32 hospitals in the UK with 51 surgeons. Participants were patients who had subacromial pain for at least 3 months with intact rotator cuff tendons, w

Decompression and coflex interlaminar stabilisation compared with conventional surgical procedures for lumbar spinal stenosis: A systematic review and meta-analysis.

BACKGROUND: Decompression plus spinal fusion is one of the most common surgeries for the treatment of degenerative spine disease in older adults. However, complications caused by fusion surgery have been reported in some studies. Recently published studies have reported that coflex is a safe and viable option in the selection of instrumentation for spinal stabilisation. Our meta-analysis was conducted to investigate whether decompression and coflex results in better performance for lumbar spinal stenosis (LSS) patients when compared with decompression and fusion surgery. Early Surgical Decompression Restores Neurovascular Blood Flow and Ischemic Parameters in an in Vivo Animal Model of Nerve Compression Injury.

BACKGROUND: Chronic nerve compression neuropathies result in decreased blood flow at the site of compression. Surgical decompression of the nerve often has variable postoperative results. The current study examines whether the timing of surgical intervention is an important variable in reversing the compression-induced ischemia and associated changes in biochemical markers. METHODS: An established model of chronic nerve compression injury was created in 100 C57BL/6 mice, and serial electrophysiological examinations were used to confirm the creation of a chronic nerve compression injury. [Acute carpal tunnel syndrome after distal radius fractures--long term results of surgical treatment with decompression and external fixator application].

Acute Carpal Tunnel Syndrome (ACTS) is a relatively rare sequel of distal fractures of the radius. This paper is an analysis of the causes of ACTS and of long-term results of treatment by surgical decompression combined with use of external fixators. Out of 128 patients with distal radius fractures treated in our department 11 cases required surgical treatment because of ACTS. Fractures were classified according to the AO classification as C2 and C3. CTSA was diagnosed basing on typical clinical signs: pain numbness over the radial three and a half fingers. All patients were treated surgically

Intracranial microvascular decompression for "cryptogenic" hemifacial spasm, trigeminal and glossopharyngeal neuralgia, paroxysmal vertigo and tinnitus: I. Surgical technique and results.

Intracranial microvascular decompression was performed in 21 out of 24 patients with hyperactive dysfunction of cranial nerves: 8 cases of hemifacial spasm, 12 of trigeminal neuralgia, 3 of glossopharyngeal neuralgia and 1 case of paroxysmal vertigo and tinnitus. In 21 cases an abnormal vascular loop was found to impinge on the root entry zone of the nerve in the brainstem. Dissection of this loop with decompression of the nerve resulted in long-lasting relief of symptoms in all but two patients who presented early recurrence; in one of these a second procedure was eventually successful. Surgical results of nonambulatory patients caused by ossification of the posterior longitudinal ligaments in the thoracic spine: retrospective comparative study between posterior decompression and instrumented spinal fusion versus anterior decompression through a posterior approach.

OBJECTIVE: Thoracic myelopathy caused by ossification of the posterior longitudinal ligament (OPLL) remains one of the most difficult-to-treat disorders for spine surgeons. In Japan, approximately 75% of patients with this condition are treated using posterior decompression with instrumented spinal fusion (PDF). In contrast, anterior decompression is the most effective method for relieving spinal cord compression. The authors treated nonambulatory patients with thoracic OPLL by either PDF or by their technique using anterior decompression through a posterior approach. In this study the surgica

Surgical outcomes and prognostic factors in traumatic optic neuropathy: a systematic review and meta-analysis of the endoscopic optic nerve decompression.

BACKGROUND AND AIM: Traumatic optic neuropathy (TON) is a vision-threatening condition with no consensus on optimal management. Corticosteroids and surgical decompression of the optic canal are widely used, but their relative efficacy remains debated. This meta-analysis aimed to evaluate the effectiveness of Endoscopic transnasal optic canal decompression (ETOCD) and compared with or alongside medical therapy, with a focus on visual acuity (VA) outcomes and the influence of timing of intervention. METHODS: A systematic literature search was conducted across PubMed, Embase, Scopus, and Web of

The time-dependent efficacy of early surgical decompression in patients with traumatic spinal cord injury: A systematic review and meta-analysis.

OBJECTIVE: Numerous studies have demonstrated that early surgical intervention in patients with traumatic spinal cord injury leads to superior recovery of various indicators, including neurological function, compared to late surgical intervention. However, some relevant clinical randomized controlled trials have indicated that the motor function recovery of patients undergoing early surgical decompression does not show a significant advantage during initial follow-up periods. The benefits of early surgical decompression become more apparent as the follow-up duration extends. Therefore, this st

Decompressive craniotomy in split-technique (DCST) for TBI in infants: introducing a new surgical technique to prevent long-term complications.

INTRODUCTION: Decompressive craniectomy (DC) is rarely required in infants. These youngest patients are vulnerable to blood loss, and cranial reconstruction can be challenging due to skull growth and bone flap resorption. On the other hand, infants have thin and flexible bone and osteogenic potential. MATERIAL AND METHODS: We propose a new technique called DCST, which makes use of these unique aspects by achieving decompression using the circumstance of the thin and flexible bone. We describe the surgical technique and the follow-up course over a period of 13 months. RESULTS AND CONCLUSION: I

Endoscopic Transnasal Odontoidectomy for Ventral Decompression of the Craniovertebral Junction: Surgical Technique and Clinical Outcome in a Case Series of 19 Patients.

BACKGROUND: Abnormalities and pathologies of the craniovertebral junction as well as space-occupying lesions of the odontoid process can result in myelopathy symptoms. A staged procedure with posterior stabilization and anterior transnasal endoscopic decompression is recently considered a less invasive alternative to the transoral approach. We present a considerably large case series focused on the operative technique and the long-term neurological clinical outcome. OBJECTIVE: To determine the safety and efficacy of odontoidectomy performed via an endoscopic transnasal approach. METHODS: We
