Staphylococcal scalded skin syndrome (SSSS) is an exfoliative dermatitis that results from infection with exfoliative toxin-producing Staphylococcus aureus. SSSS is seen primarily in infants and children. Here we ask if there is a specific maturation process that protects healthy adults from this syndrome. For these studies, an active recombinant exfoliative toxin A (rETA) was used in a neonatal mouse model. A time course generated on the susceptibility to the toxin as a function of mouse age indicated that BALB/c mice developed the characteristic symptoms of SSSS until day 7 of life. Between

The bacteriologic, epidemiologic, clinical, and diagnostic findings and management of the scalded skin syndrome (SSS) in 42 children are reported SSS may present in one of three ways: (1) Ritter's disease in infants, characterized by an acute, generalized bullous dermatitis simulating the appearance of scalded skin, followed by exfoliation; (2) Lyell's disease, or toxic epidermal necrolysis (TEN), in children or adults, with scalded skin and bullae followed by extensive exfoliation; or (3) a nonstreptococcal scarlatiniform eruption, or staphylococcal scarlet fever, manifested by a generalized

PRACTICAL RELEVANCE: Cutaneous immune-mediated diseases in cats can present with clinical signs of erythema, hair loss, scaling/crusting and erosions with or without pruritis, mimicking many differential diagnoses such as hypersensitivity dermatitis and infections (eg, parasitic and fungal dermatitis). Veterinarians are encouraged to become familiar with the characteristic clinical features of various immune-driven skin disorders to permit early diagnosis and appropriate treatment.
CLINICAL CHALLENGES: Diagnosis and therapeutic management of feline immune-mediated skin diseases can be challen

Use of medicinal plants is common and widespread throughout Africa, including in Senegal. Because efficacy has been demonstrated, public policies have been instituted that have allowed plant-based therapies to have an important role in general primary care. However, little is known about the cutaneous safety of many plant-based therapies. In this 6-month prospective study all cases of dermatitis induced or aggravated by exclusive use of medicinal plants were evaluated via skin allergy testing. The results were classified and compared with the available literature. Forty-three cases of plant-th

Exfoliative erythroderma is rare but serious condition, which requires close supervision. We report a rare case of 28-year-old man with kissing lesions of Paederus dermatitis at his right side of neck. The rash caused by Paederus beetle was improved after treatments. However, the patient developed generalized erythema with desquamation and scaling. The patient was successfully treated topically with moisturizing liquid soap and topical moisturizer with emollients and humectants, and triamcinolone lotion was applied on the bright red lesion. The patient was also treated with oral replacement so

The skin can be key to early diagnosis of systemic malignancies. In the second part of this review, we present various skin conditions that can, in certain contexts, reveal the presence of malignancy. The skin conditions are presented in groups based on a diverse range of morphological characteristics. Specifically, the following groups are analyzed: erosive and blistering lesions; inflammatory papules and nodules; xerosis, ichthyosis, and generalized exfoliative dermatitis; symptoms such as pruritus; abnormal hair distribution patterns; sweating disorders; benign tumors that can form part of

A patient with Erythema Gyratum Repens (EGR) had a marked increase of his eruption, with uncontrollable pruritus that was unresponsive to steriod therapy. This culminated in an exfoliative dermatitis. A metastatic, undifferentiated adenocarcinoma was removed following a right-sided craniotomy. The patient then had complete cessation of his pruritus, with moderate improvement of his eruption. All the reported cases of EGR were reviewed in terms of the source of the malignant disorder. The relationship between the time of onset of the EGR and the discovery of the malignant disorder, as well as t

Sebaceous adenitis and concurrent meibomian gland dysfunction (MGD) were diagnosed in a two-year-old mongrel dog presenting with hypotrichosis, exfoliative dermatitis and blepharitis. Diagnosis of sebaceous adenitis was based on history, clinical signs, the histological demonstration of multifocal lymphohistiocytic and neutrophilic inflammation targeting the sebaceous glands and sebaceous glands loss. MGD was diagnosed by non-contact infrared meibography followed by tear film lipid layer interferometric evaluation. Ciclosporin and sebolytic shampoos controlled the dermatological condition, whi

An 83-year-old woman developed generalized lymph node hyperplasia together with mycosis fungoides-like skin lesions 11 months after institution of anticonvulsant therapy with phenytoin. The clinicopathological changes disappeared completely three weeks after cessation of therapy. This circumstantial evidence together with the known data on the association of phenytoin with lymphoproliferative disorders suggest that it was the phenytoin that was responsible for the patient's condition. This case had features similar to those associated with the pseudo-mycosis fungoides syndrome except that the

Fever, lymphadenopathy, exfoliative dermatitis, and evidence of drug-induced liver injury developed in a 16-year-old girl three weeks after beginning therapy with phenytoin and phenobarbital. This clinical syndrome can be caused by either of these structurally related drugs but has been more frequently attributed to phenytoin. In vitro studies disclosed marked reactivity of this patient's lymphocytes to concentrations of both drugs, which encompassed their measured serum levels. The demonstration of dual reactivity raises concerns about continuing administration of phenobarbital during an appa

The acute scrotum generates a long list of differential diagnoses. An unusual etiology includes insect envenomation, which typically is an acute process with rapid onset of symptomatologies. Two patients with genital envenomation are reported. We review the reported cases at our institution with all types of bites and stings. Symptoms of pain and pruritus, and signs of ecchymosis and edema preceding exfoliating dermatitis were evident in both cases. Mild analgesics and antihistamines promoted resolution in each instance.

Erythroderma, an inflammatory skin condition characterized by widespread erythema with variable degrees of exfoliation, pustulation, or vesiculobullous formation, is associated with high morbidity and mortality. Determining the underlying cause of erythroderma frequently presents a diagnostic challenge, which may contribute to the condition's relatively poor prognosis. This review covers the clinical presentation, pathophysiology, diagnosis, and treatment of erythroderma. It discusses similarities and differences among the many underlying etiologies of the condition and differences between ery

Bromoderma is a rare hypersensitivity reaction caused by bromide intake. It was relatively frequent in the early years of the previous century because of the common use of bromide-containing solutions in pediatrics due to their antispasmodic, expectorant, sedative, and anticonvulsant effects. Although recently prohibited in many countries, bromides are still used as an adjuvant anticonvulsant drug and still present in some over the counter antispasmodics and analgesics. Bromoderma usually present with pustular and vegetating lesions that may represent a diagnostic challenge for dermatologists.

Infections caused by pyogenic streptococci are commons in pediatric ages. However, in the last decades there has been an increase in the isolation of no A or B Streptococci. We report a case of a 6 years old girl, who presents fever for 5 days and erythroderma. Streptococcus dysgalactiae subspecies equisimilis was isolated from blood cultures. She receives antibiotics for 14 days with ß-lactams with a good clinic evolution with normalization of the inflammatory parameters. This agent ß-hemolítico presents antigens of Lancefield groups A, C and G, and a great similitude regarding virulence fact

A patient who developed exfoliative erythroderma 11 days after the initiation of hydroxychloroquine treatment for psoriatic arthritis is presented. Our experience with her and the continued controversy regarding the incidence of adverse reactions in psoriatic patients to antimalarials prompted a detailed review of the literature. We conclude that the use of antimalarials in patients with psoriasis is associated with a high incidence of severe cutaneous adverse reactions. These medications should not be used in psoriatic patients if other less toxic, effective therapies are available.
