Disease associations in polymorphous light eruption. A long-term follow-up study of 94 patients.

OBJECTIVES: To examine the long-term outcome of polymorphous light eruption (PLE) in a large patient population and to evaluate associated conditions, especially lupus erythematosus, during the course of the disease. DESIGN: A questionnaire-based follow-up study an average of 32 years after onset of PLE. The study was complemented by clinical examination of the patients with PLE similarly studied 16 years earlier or now reporting equal or worse PLE symptoms compared with the 1978-1979 follow-up or any symptoms suggesting an autoimmune disease. SETTING: A dermatologic clinic in a university h

Quinine and quinidine photoproducts can be identical.

Quinine and its d-isomer quinidine can both cause contact allergy as well as photoallergy. Contact allergic cross-reactions between quinine and quinidine are uncommon. In allergic photosensitization the two isomers cross-react, suggesting the possibility that quinine and quinidine after UV exposure are converted to one or more common sensitizing photoproducts. Solutions of quinine and quinidine at 0.1% in ethanol 99.5% were exposed to UVA for 14 h (total dose 201.6 J/cm2). Using thin-layer chromatography, we identified 8 and 6 photoproducts from irradiated quinine and quinidine, respectively.

The Potential of Natural Compounds in UV Protection Products.

Overexposure to ultraviolet radiation mainly leads to skin disorders (erythema, burns, immunosuppression), skin aging, and skin cancer as the most serious side effect. It has been widely accepted that using sunscreen products is an important way to protect against the harmful effects of UV rays. Although commercial sunscreens have constantly changed and improved over time, there are emerging concerns about the safety of conventional, organic, UV filters due to adverse effects on humans (such as photoallergic dermatitis, contact sensitivity, endocrine-disrupting effects, etc.) as well as accumu

Ketoprofen: experimental overview of dermal toxicity.

Ketoprofen (KP) is a widely used non-steroidal anti-inflammatory drug (NSAID). However, an increasing number of case reports suggest that in broad use, KP can cause allergic dermatitis. Most of these adverse effects have been attributed to the photoallergic potential of KP and photosensitivity. With the exception of a few reports in experimental animals, there is little evidence that KP actually causes dermal toxicity. In this study, in order to investigate the eventual underlying causes of KP dermal toxicity, we conducted primary irritation, skin cumulative, skin sensitization, phototoxicity

Reticulate, patchy and mottled pigmentation of the neck. Acquired forms.

Besides the inherited forms of mottled and reticulate pigmentation, a vast number of diseases and trigger mechanisms can lead to acquired pigmentation of the neck. Nonhereditary variants of reticulate and mottled pigmentation can affect the neck as a typical site and therefore may give a diagnostic clue or it can occur sporadically on the neck as well as on other sites. A well-known and important factor in the pathogenesis is exposure to sunlight. Sun-induced pigmentation often presents on the neck and may result from phototoxic, photoallergic and cumulative actinic damage. Frequent forms comp

Rates of allergic sensitization and irritation to oxybenzone-containing sunscreen products: a quantitative meta-analysis of 64 exaggerated use studies.

BACKGROUND/PURPOSE: Oxybenzone is an active ingredient found in sunscreen products that absorbs a broad spectrum of ultraviolet (UV) light, with absorbance peaking in the UVB region and extending into the UVA region. Although the overall incidence of sensitization and irritation associated with oxybenzone in the general population remains unclear, a few studies have reported on the incidence in specific circumstances. However, the relevance of these studies to the general population is limited, because the sample populations reported in these papers generally have consisted of individuals who

Sesquiterpene lactone allergy.

Sesquiterpene lactones are a large, diverse group of chemicals found in several plant families that cause allergic contact dermatitis. The biological, botanical, allergenic, and structural significance of sesquiterpene lactones is explored as well as the clinical characteristics of cutaneous reactions. The association with UV-light sensitivity and proposed mechanisms for this relationship as well as new developments in patch test methods and treatment are reviewed.

Cross-reactions in patch testing and photopatch testing with ketoprofen, thiaprophenic acid, and cinnamic aldehyde.

In the last 7 years, we have studied 123 patients with allergic reactions to topical arylpropionic anti-inflammatory drugs. We have investigated the rate of sensitization and the irritant potential of one of them, ketoprofen, and its cross-reactivity with such other derivatives as ibuproxam, ibuprofen, naproxen, fenoprofen, flurbiprofen, and thiaprofenic acid. Sensitization was single in most cases, and ketoprofen was the drug most often involved. The combination most frequently found was ketoprofen plus ibuproxam. The most frequent cross-reactions were to fragrance mix, especially cinnamic al

Ultraviolet radiation causes less immunosuppression in patients with polymorphic light eruption than in controls.

It is hypothesized that polymorphic light eruption is characterized by a partial failure of ultraviolet radiation-induced immunosuppression, resulting in a delayed-type hypersensitivity response to photo-induced antigens. We aimed to study the susceptibility of PLE patients to UVR-induced immunosuppression, by measuring the strength of sensitization to 2,4-dinitrochlorobenzene after UVR exposure, and to diphenylcyclopropenone without UVR exposure, in subjects with PLE and controls. Thirteen PLE patients and 11 controls were exposed to 1 minimum erythema dose (MED) of UVR delivered from Waldman

Skin reactions to sunscreens.

Sunscreen reactions are said not to be uncommon. A population referred to a patch testing clinic was evaluated for reactions to sunscreen by questionnaire initially and then, if relevant, by patch testing to sunscreen products and their components. Irritant reactions were more common than allergic contact dermatitis. Allergic reactions to sunscreens were less common than to non-sunscreen chemicals present in sunscreen products.

Clinical and experimental photosensitivity reaction to tilisolol hydrochloride.

This report describes a patient who developed a photosensitivity reaction during the treatment of hypertension with tilisolol hydrochloride, which is a beta-blocker agent Although the data are insufficient to distinguish photoallergy and phototoxicity, clinical and histologic features suggested that the mechanism involved was photoallergic in nature. The action spectrum was mainly the ultraviolet A range. The reaction was reproducible on oral readministration of the drug and exposure to a low dose of UVA. Photopatch testing with the drug was also positive only in the patient. Although the expe

Photopatch testing of 1155 patients: results of the U.K. multicentre photopatch study group.

BACKGROUND: Photoallergic contact dermatitis can be difficult to diagnose if not appropriately investigated. Currently, the most common U.K. photoallergens appear to be sunscreen chemicals. The investigation of choice is photopatch testing (PPT), which is probably underused. In part, this is due to differences in methodology and results interpretation. OBJECTIVES: To conduct PPT using a group of sunscreen chemicals, defined indications and a standardized methodology including interpretation and relevance of reactions in patients attending for investigation at 17 centres across the U.K., Irela

16S rRNA Sequencing Reveals Dysbiosis of Skin Microbiome Associated With Disease Severity in Chronic Actinic Dermatitis.

BACKGROUND: Chronic actinic dermatitis (CAD) is a refractory photoallergic skin disease characterized by inflammatory infiltration and UV sensitivity. While the role of microbiome dysbiosis has been established in various inflammatory skin conditions, its contribution to CAD pathogenesis remains unexplored. OBJECTIVE: To characterize the skin microbiome alterations in CAD patients and investigate their potential associations with disease severity and UV sensitivity. METHODS: Skin microbiome samples were collected from 15 CAD patients and 14 matched family controls, covering both photoexposed

In chemico sequential testing strategy for assessing the photoallegic potential.

Several non-animal testing methods to assess photoallergic potential have been developed so far, while none of them have yet to be validated and regulatory accepted. Currently, some photoreactivity assays such as UV-VIS spectral analysis and ROS assay are generally used for initial photosafety assessments because of their high sensitivity. However, they have a low specificity, generating a high percentage of false positive results, and the development of a follow-up assessment method is desired. Therefore, this study aimed to develop an in chemico photoallergy testing method, photo-direct pept

Sunscreen photopatch testing: a series of 157 children.

BACKGROUND: Photoprotection including sunscreen use in children is encouraged by health campaigns. While sunscreen chemicals are common causes of photoallergic (PA) contact reactions in adults, limited data are available in children. OBJECTIVES: To assess the frequency of PA and contact allergy (CA) to sunscreens in children aged < 18 years undergoing investigation for suspected photosensitivity. METHODS: Retrospective analysis of data on children who underwent photopatch testing to a standard series of nine ultraviolet (UV) filters and to sunscreen products in a single photoinvestigation ce

Quinolone photoallergy: photosensitivity dermatitis induced by systemic administration of photohaptenic drugs.

Quinolone antibacterial agents are well known to elicit photosensitivity as a side effect. The photoallergenicity of fluoroquinolones, the representative quinolone derivatives, is mainly derived from their photohaptenic moiety. When epidermal cells are irradiated with ultraviolet A light in the presence of fluoroquinolones, quinolone photoadducts are formed in the treated cells. This photomodification is thought to be an initial step for sensitization and elicitation of this photoallergy, and quinolone-photoderivatized Langerhans cells are capable of stimulating immune T cells in mice.
