Indoor air pollution in developing countries: a major environmental and public health challenge.

Around 50% of people, almost all in developing countries, rely on coal and biomass in the form of wood, dung and crop residues for domestic energy. These materials are typically burnt in simple stoves with very incomplete combustion. Consequently, women and young children are exposed to high levels of indoor air pollution every day. There is consistent evidence that indoor air pollution increases the risk of chronic obstructive pulmonary disease and of acute respiratory infections in childhood, the most important cause of death among children under 5 years of age in developing countries. Evide…

Potential interventions for the prevention of childhood pneumonia in developing countries: improving nutrition.

Acute respiratory infections are the leading cause of childhood death in developing countries. Current efforts at mortality control focus on case management and immunization, but other preventive strategies may have a broader and more sustainable effect. This review, commissioned by the World Health Organization, examines the relations between pneumonia and nutritional factors and estimates the potential effect of nutritional interventions. Low birth weight, malnutrition (as assessed through anthropometry), and lack of breast-feeding appear to be important risk factors for childhood pneumonia,…

Hypoxia in childhood pneumonia: better detection and more oxygen needed in developing countries.

Even though hypoxia is a major risk factor for death in children with acute respiratory infection in developing countries, oxygen is not part of first line treatment. Because oxygen is not readily available in developing countries it tends to be given to the most seriously ill children, whose outcome is poor. Oxygen might be useful if given earlier in the course of the disease. Clinical signs are not clear cut, however, though the presence of cyanosis and grunting together with a raised respiratory rate can significantly increase the detection of hypoxaemia. A simple oximeter would make detect…

Evaluation of two staging systems for HIV infection for use in developing countries.

OBJECTIVE: To evaluate the clinical axis of the World Health Organization (WHO) clinical staging system and the modified WHO staging system … or a modified version of this system using lymphocytes stratification may be a good alternative in developing countries to the CD4 lymphocyte count-based HIV staging system used in the developed world. Cohort studies in developing countries are needed to assess their prognostic value.

A questionnaire to screen for cognitive impairment among elderly people in developing countries.

Most questionnaires designed to assess cognitive impairment among elderly people are constructed in the West, where literacy is high. However, such questionnaires may not be applicable in developing countries because of cultural differences or low literacy. The Elderly Cognitive Assessment Questionnaire (ECAQ) is derived from items in the Mini-Mental State Examination and Geriatric Mental State Schedule. It is a satisfactory scale for quantitative assessment of cognitive impairment among elderly people living in developing countries. This 10-item questionnaire shows a sensitivity of 85.3%, spe…

Effects of agricultural pesticides on humans, animals, and higher plants in developing countries.

Undesirable side effects that result from the indiscriminate use of agricultural pesticides in developing countries are widespread. This has captured the attention of health workers, governments, and environmental protection agencies. In fact, the potential health effects of long-term exposure to humans, animals, and higher plants are of great concern. This study examines the effects of agricultural pesticides on such living forms and explores modes of action, and presents strategies to minimize the deleterious effects of pesticides to living forms in developing countries.

The incidence of the respiratory distress syndrome: with particular reference to developing countries.

The incidence of RDS varies in various parts of the world and appears to be lower in negroes than caucasian and mongolians. The incidence in mongolians is as high if not higher than in caucasians. The difference between negroes and other races may be due to the ability of the negro fetus to produce surfactant earlier. Variation within the black ethnic groups may be due to genetic differences between them. Studies on the incidence of RDS will be more accurate if surfactant deficiency is documented and other conditions, particularly transient respiratory distress and early neonatal sepsis which

Methadone maintenance therapy as evidence based drug abuse planning in developed countries: can developing countries afford and learn from this experience?

Use of illicit substances of abuse is a major public health problem in developed countries like the US. However, this problem of illicit substance use has spread like a tumor to include currently developing countries where most of its youths and adolescents are actively engaged in this illegal practice. This problem is even more worse in poor resource countries, as use of these substances is accompanied with a lot of HIV- risk behaviours, and for cocaine and heroin drug injectors often share injecting equipments hence increasing the chances of contracting and spreading HIV infection. Apart fro…

[Comparative analysis of mortality due to violence in developed countries and in a few developing countries during the 1985-1989 period].

A comparative analysis of crude death rates and specifically of deaths from violence was conducted for a large number of developed and developing countries which provide data to the World Health Organization's data bank. For these countries, the analysis shows, first of all, that violent deaths rank third among the major causes of death, after diseases of the circulatory system and malignant tumours, in most developed countries and in some developing countries with reliable data. The comparative analysis also reveals substantial variations in the level and structure of death rates, both among …

Pregnant women with epilepsy in a developing country.

This is the first prospective study carried out in Argentina and Latin America to provide the impact of epilepsy throughout the childbearing years life of women, and pregnancy outcome in a population of pregnant women with diagnosis of epilepsy and antiepileptics drugs (AEDs). Ninety-four women were studied prospectively at the Epilepsy Center, the largest in Argentina. Of the 94 women examined in this study only 10% planned their pregnancy and received folic acid before conception.More than half of women in our study were on monotherapy, with the most frequently prescribed drugs being carbama

Phrynoderma: a forgotten entity in a developed country.

Phrynoderma is a type of follicular hyperkeratosis attributed to various nutritional deficiencies, most notably vitamin A. We report a case of a 31-year-old mentally-deficient man who was a resident of a voluntary welfare nursing home. He presented with characteristic hyperkeratotic follicular papules on his trunk in a setting of low serum level of vitamin A and malnutrition. Although commonly seen in South and East Asia, it is rarely reported in Singapore. However, there is still a population at risk here--patients with malabsorption and eating disorders, and institutionalised persons. Phryno

Congenital diaphragmatic hernia in a developing country.

INTRODUCTION: Outcome of neonates with congenital diaphragmatic hernia (CDH) varies widely and the data from developing countries is scanty. We aimed to study the management and outcome of CDH. We also aimed to ascertain prenatal and postnatal factors affecting the outcome. METHODS: A retrospective review of neonates with CDH admitted to a teaching hospital was conducted. Demographical data, prenatal and postnatal factors, birth details, management and outcomes were studied. Survival was the primary outcome. RESULTS: 16 live-born neonates with diaphragmatic hernia were admitted during the st…

Preseptal and orbital cellulitis in a developing country.

PURPOSE: To report the clinical features, etiology, management practices, and outcomes of preseptal (PC) and orbital cellulitis (OC) in a de … tions such as: visual loss (29%), neurological deficits (17%), and mortality (3%). CONCLUSIONS: In developing countries, eyelid infection may be the most important predisposing factor for periocular infection. Staphylococcus is the predominant causative agent. OC can be associated with serious complications. Aggressive management of OC may improve patient outcomes.
