Metamizole is an old analgesic used frequently in some countries. Active metabolites of metamizole are the non-enzymatically generated N-methyl-4-aminoantipyrine (4-MAA) and its demethylation product 4-aminoantipyrine (4-AA). Previous studies suggested that 4-MAA demethylation can be performed by hepatic cytochrome P450 (CYP) 3A4, but the possible contribution of other CYPs remains unclear. Using human liver microsomes (HLM), liver homogenate and HepaRG cells, we could confirm 4-MAA demethylation by CYPs. The influence of non-narcotic analgesics analgin and pentalgin in the basic pharmacological effects of diazepam and mexidol has been studied in outbred male albino rats. Drug-induced agranulocytosis (DIA) is a potentially fatal disorder. Hematopoietic growth factors have been used in the treatment of DIA. We report nine cases of DIA treated with granulocyte macrophage - colony stimulating factor (GM-CSF) in a dose of 300 microg/day. All the patients had evidence of systemic infection. Mean time to reach an absolute neutrophil count of 0.5 x 10(9)/L was three days. One patient succumbed to the disease. The cause of death was multiorgan failure. To evaluate the tolerance to piroxicam in patients with urticaria induced by analgesic and/or nonsteroidal antiinflammatory drugs (NSAIDs), we carried out a 2-year study in an outpatient clinic. All the patients referred to the clinic for study entered a protocol for evaluation of intolerance to one or more drugs. If patients were allergic to at least two different NSAIDs they were allocated to group A, but if patients were allergic to only one they were considered as having selective intolerance (group B). The analgesic efficacy of 50 and 100 mg tramadol and 500 and 1,000 mg metamizole was compared using a randomized double-blind design on 10 volunteers; drugs were given orally as solutions.
