Application of the microwave digestion-vacuum filtration-automated scanning electron microscopy method for diatom detection in the diagnosis of drowning.

The cause of death for the decomposed corpses recovered from water is still a difficult issue in current forensic practice. In this article, we present two cases of bodies recovered from water with no positive findings to indicate the cause of their death. We apply both conventional acid digestion method as well as the microwave digestion-vacuum filtration-automated scanning electron microscopy method (MD-VF-Auto SEM) for diatom detection in different organs of both bodies. Our results indicate that MD-VF-Auto SEM method provide more accurate information and match further police investigation.

Therapeutic bronchoscopy in a child with sand aspiration and respiratory failure from near drowning--case report and literature review.

Foreign matter aspiration occurs relatively commonly in drowning and near-drowning events. In most cases, stomach contents are aspirated. Sand aspiration rarely occurs and there are no reported cases in children with near drowning. Limited data are available on clinical presentation and management of sand aspiration with accidental burial. We report a 3-year-old boy who nearly drowned while swimming in brackish waters and was found face down in sand. Sand aspiration was suspected when the child continued to have persistent wheezing and high ventilatory requirement despite intensive bronchodila…

Infection of the CNS by Scedosporium apiospermum after near drowning. Report of a fatal case and analysis of its confounding factors.

This report describes a fatal case of central nervous system pseudallescheriasis. A 32 year old white man presented with headache and meningismus 15 days after nearly drowning in a swine sewage reservoir. Computerised tomography and magnetic resonance imaging of the head revealed multiple brain granulomata, which vanished when steroid and broad spectrum antimicrobial and antifungal agents, in addition to dexamethasone, were started. Cerebrospinal fluid analysis disclosed a neutrophilic meningitis. Treatment with antibiotics and amphotericin B, together with fluconazole and later itraconazole, …

A Height-Normalised Revised Drowning Index for Thoracic Fluid Burden: Associations with Water Type, Sex, Toxicology and Postmortem Interval.

Background: The forensic diagnosis of drowning lacks pathognomonic signs, and visceral weight-based indices normalised to spleen or heart weight have shown variable discriminant performance. We assessed a height-normalised revised Drowning Index (rDI) as a measure of thoracic fluid burden in confirmed drowning cases, not as a case-control test of drowning versus non-drowning deaths. Methods: Fifty-five institutional drowning cases (Ancona and Turin Province, Italy, 2008-2020) were analysed. The rDI and the original spleen-normalised Drowning Index (DI) were calculated. A multivariable linear r…

Rewarming Young Children After Drowning-Associated Hypothermia and Out-of-Hospital Cardiac Arrest: Analysis Using the CAse REport Guideline.

OBJECTIVES: Extracorporeal membrane oxygenation (ECMO) is recommended in adults with drowning-associated hypothermia and out-of-hospital cardiac arrest (OHCA). Our experience of managing a drowned 2-year-old girl with hypothermia (23°C) and cardiac arrest (58 min) prompted this summary using the CAse REport (CARE) guideline to address the question of optimal rewarming procedure in such patients. DESIGN/PATIENTS: Following the CARE guideline, we identified 24 reports in the "PubMed database" describing children less than or equal to 6 years old with a temperature less than or equal to 28°C who…

Multiple Scedosporium apiospermum brain abscesses after near-drowning successfully treated with surgery and long-term voriconazole: a case report.

Scedosporium apiospermum is an emerging fungal pathogen, increasingly recognised as a cause of infection not only in immunocompromised, but also in non-immunodeficient patients. We present the case of an immunocompetent 21-year-old Thai man who experienced recurrent ruptured mycotic aneurysms and developed multiple S. apiospermum brain abscesses following aspiration of polluted water. A combination of surgery and voriconazole was effective in this difficult-to-treat case. Voriconazole penetrated the central nervous system (CNS) well and had limited toxicity, despite prolonged treatment. Vorico

Drowning among personal watercraft passengers: the ability of personal flotation devices to preserve life on Arkansas waterways, 1994-1997.

Personal flotation devices (PFD) are promoted and in many states are required for safe boating practices and to prevent drowning. Primary use of PFDs is associated with water sports (water skiing, tubing, etc.), boating and other water activities. Their purpose is to preserve life and prevent drowning. However, their effectiveness to prevent drowning and near-drowning has not been well established. The purpose of this investigation was to determine the effectiveness of PFDs to prevent drowning and near-drowning of individuals involved in personal watercraft (PWC) crashes. Methods include the 4…

Impact of liquid dosing and associated cell 'drowning' in a 3D human bronchial epithelial model used for protease exposure studies.

INTRODUCTION: As part of a programme of work developing human-relevant New Approach Methodologies (NAMs) for next-generation risk assessment (NGRA), use of a human air-liquid interface (ALI) bronchial epithelial model (MucilAir™) to investigate the impact of exposure to protease was assessed. METHODS: To guide in vitro dosing, scenarios representing low-high risk of sensitisation after 8 h of protease exposure in an occupational setting were modelled by performing simulations with the Multiple Path Particle Dosimetry (MPPD) model with refinement of the outputs using a tracheobronchial/alveola

Subnational inequalities and preventable gaps in drowning premature mortality and non-fatal health loss among population younger than 20 years.

BACKGROUND: Drowning remains a preventable cause of premature mortality among children and adolescents. National estimates can monitor progress, but may not identify subnational locations where risk remains high. We examined drowning burden in the population younger than 20 years in 2023, focusing on age patterns, national differences, subnational variation, and frontier gaps. METHODS: We analyzed 1990-2023 population health estimates for drowning. Deaths, disability-adjusted life-years (DALYs), death rates, and DALY rates were summarized globally and across 204 countries and territories. Sub…

Association between early radiographic chest findings and clinical outcomes in pediatric drowning: a retrospective study in a tertiary Italian hospital.

UNLABELLED: Pediatric drowning incidents, both fatal and non-fatal, represent a significant challenge in emergency medicine, particularly for children under 14 years of age. Drowning is a leading cause of unintentional injury-related deaths, with children aged one to four years being especially vulnerable. Accurate and timely assessment, particularly through chest imaging, such as chest radiography (CXR) and lung point-of-care ultrasound (LUS), is crucial for evaluating pulmonary complications and guiding treatment decisions. This retrospective study analyzed 64 pediatric drowning cases in Ist…

Out-of-Hospital Cardiac Arrest due to Drowning in North America: Comparison of Patient Characteristics between Survival and Mortality Groups.

Out-of-hospital cardiac arrest (OHCA) due to drowning carries high morbidity and mortality. There are a few studies on drowning-related out-of-hospital cardiac arrest (OHCA), in which patients are followed from the scene to hospital discharge. This study aims to compare patient characteristics between the survival group and mortality group of OHCA due to drowning. OHCA due to drowning cases were selected from the North America Termination of Resuscitation Association database between 2011 and 2015. The retrospective analysis of epidemiological characteristics and clinical features of all OHCA …

[Acute respiratory failure (ARDS) in a young child after drowning accident: therapy with exogenous surfactant and high frequency oscillatory ventilation].

The adult respiratory distress syndrome (ARDS) in children has a very poor prognosis with a mortality risk of between 55 and 85%, in spite of improvements due to the introduction of positive endexpiratory pressure ventilation. We describe the clinical course of a not yet 3 year-old boy with severe ARDS following near-drowing. Treatment with exogenous surfactant and high frequency oscillatory ventilation, a well-established procedure in neonatology, was responsible for the favorable outcome. The high cost of surfactant therapy, however, is the main limiting factor for this kind of treatment in

Evaluation of the WAVE Drowning Detection SystemTM for use with children's summer camp groups in swimming pools: A prospective observational study.

BACKGROUND: Groups of children swimming during summer camp or child care are generally monitored by a small number of lifeguards and staff. The high child-to-staff ratio can make pool monitoring less effective, increasing drowning risk. The aim of this study is to evaluate novel drowning detection technology that could supplement pool monitoring. METHODS: The WAVE™ Drowning Detection System was deployed at a camp pool for 8 weeks. The WAVE™ Drowning Detection System entails headbands worn by swimmers that send alerts to vibrating staff bracelets and audible alarms when submerged for a period …
