Direct oral anticoagulant use and drug-drug interactions in transplant recipients receiving calcineurin inhibitors.

BACKGROUND: Direct oral anticoagulants (DOACs) are metabolized by CYP3A4 and/or transported by P-glycoprotein. Cyclosporine and tacrolimus can inhibit these pathways, potentially increasing DOAC exposure and bleeding risk. However, data on the relevance of these interactions in transplant recipients are limited. OBJECTIVES: We aimed to study DOAC doses, plasma level monitoring, and interactions with calcineurin inhibitors in transplant recipients. METHODS: We retrospectively (2013-2024) included all adults treated at the University Hospitals Leuven, Belgium, who received both a DOAC and tacr

Denosumab-induced hypocalcemia: detection of drug-drug interactions using disproportionality analysis in VigiBase®.

BACKGROUND: Denosumab-induced hypocalcemia has been reported more frequently in real-world studies than in pivotal clinical trials; this is probably due to a closer monitoring of medication regimens in trials. Here, we sought to detect signals for drug-drug interactions (DDIs) between denosumab and other hypocalcemia-inducing drugs. METHODS: We performed a disproportionality analysis of data in VigiBase® between January 1st, 2011, and February 7th, 2024. The frequency of hypocalcemia reports for all drugs known to induce hypocalcemia was compared with that for all other drugs and was quoted a…

Scoring drug efficacy by integrating drug-induced protein-protein interactions into cellular phenotypic information.

Precision oncology aims to match cancer patients with the most effective therapeutic drugs. Although cancer genomics has been widely applied, its low matching rate limits its clinical value. Image-based functional precision oncology, which directly characterizes the responses of patient-derived cells to drugs, is expected to achieve higher matching rates. However, acquiring only phenotypic information from images cannot distinguish specific target engagement from non-selective cytotoxicity, which usually leads to some false-positive results. In this study, using live-cell FRET microscopy, we c

Thrombocheck: a practical pocket guide to pharmacokinetic drug-drug interactions in oral antithrombotic therapy.

Pharmacokinetic drug-drug interactions (DDIs) can compromise the efficacy or safety of oral antithrombotic therapies (defined as oral anticoagulants or antiplatelet agents) due to under- or overexposure. Currently, there is limited published data on DDIs involving direct oral anticoagulants, and there is an alarming lack of clear data on DDIs involving antiplatelet agents. In this paper, we elaborated a comprehensive, up-to-date pocket guide on pharmacokinetic DDIs showing five possible levels of DDI (major, moderate or weak increase and major or moderate/weak decrease in antithrombotic drug e…

Potential drug-drug interactions analysis in Polish pediatric pneumonology units, including cystic fibrosis patients.

The lack of data on drug-drug interactions in pediatrics represents a relevant problem in making appropriate therapeutic decisions. Our study aimed to investigate the incidence and risk factors for potential drug-drug interactions (pDDIs) in pediatric pneumonology units, including cystic fibrosis patients. We performed a 6-month prospective observational study during which clinical pharmacists, using the Lexicomp Drug Interactions checker, screened medical records to identify pDDIs. Spearman's rank coefficient, logistic regression, and the Mann-Whitney U test were used to identify correlations…

Identifying Drug-Drug Interactions in Spontaneous Reports Utilizing Signal Detection and Biological Plausibility Aspects.

Translational approaches can benefit post-marketing drug safety surveillance through the growing availability of systems pharmacology data. Here, we propose a novel Bayesian framework for identifying drug-drug interaction (DDI) signals and differentiating between individual drug and drug combination signals. This framework is coupled with a systems pharmacology approach for automated biological plausibility assessment. Integrating statistical and biological evidence, our method achieves a 16.5% improvement (AUC: from 0.620 to 0.722) with drug-target-adverse event associations, 16.0% (AUC: from

Drug-drug interactions involving CFTR modulators: a review of the evidence and clinical implications.

INTRODUCTION: Cystic fibrosis (CF) is characterized by mucus accumulation impairing the lungs, gastrointestinal tract, and other organs. Cystic fibrosis transmembrane conductance regulator (CFTR) modulators (ivacaftor, tezacaftor, elexacaftor, and lumacaftor) significantly improve lung function and nutritional status; however, they are substrates, inhibitors, and/or inducers of certain CYP enzymes and transporters, raising the risk of drug-drug interactions (DDI) with common CF medications. AREAS COVERED: A literature search was conducted for DDIs involving CFTR modulators by reviewing new dr…

Physiologically Based Precision Dosing Approach for Drug-Drug-Gene Interactions: A Simvastatin Network Analysis.

Drug-drug interactions (DDIs) and drug-gene interactions (DGIs) are well known mediators for adverse drug reactions (ADRs), which are among the leading causes of death in many countries. Because physiologically based pharmacokinetic (PBPK) modeling has demonstrated to be a valuable tool to improve pharmacotherapy affected by DDIs or DGIs, it might also be useful for precision dosing in extensive interaction network scenarios. The presented work proposes a novel approach to extend the prediction capabilities of PBPK modeling to complex drug-drug-gene interaction (DDGI) scenarios. Here, a whole-…
