Chelating drugs and zinc.

Essential trace metals participate in a wide range of biochemical processes. Administration of drugs containing nucleophilic atoms, primarily N-, O- and S-ligands, may result in formation of organometallic non-ionic complexes or chelates. The stability of the complex depends on the position of the metal in the first transition series, except for Cu2+, which forms stronger bindings than Zn2+ does. In nature, a wide range of chelates occur, e.g. as vitamin-metal complexes (Zn-thiamine, Zn-pyridoxamine and Zn-biotin). Phytate in unleavened grains forms unsoluble Zn-complexes. In the body, complex

Approvals and Timing of New Formulations of Novel Drugs Approved by the US Food and Drug Administration Between 1995 and 2010 and Followed Through 2021.

IMPORTANCE: New formulations of prescription drugs can improve convenience and tolerability for patients, but they also constitute manufacturer strategies to extend brand-name drug market exclusivity periods. OBJECTIVE: To examine whether new formulations of brand-name novel drugs were associated with novel drugs' sales and/or therapeutic value, as well as characterize first new formulations' approval timing relative to the novel drug's generic approval. DESIGN SETTING AND PARTICIPANTS: This cross-sectional study used the Drugs@FDA database to identify all novel tablet and capsule drugs appr

Malaria medicines to address drug resistance and support malaria elimination efforts.

Antimalarial drugs are essential weapons to fight malaria and have been used effectively since the 17th century. However, P.falciparum resistance has been reported to almost all available antimalarial drugs, including artemisinin derivatives, raising concerns that this could jeopardize malaria elimination. Areas covered: In this article, we present a historical perspective of antimalarial drug resistance, review current evidence of resistance to available antimalarial drugs and discuss possible mitigating strategies to address this challenge. Expert commentary: The historical approach to drug

Psychotic and Bipolar Disorders: Antipsychotic Drugs.

Antipsychotic drugs block dopamine receptors and are used to manage psychosis as well as other mental illnesses that may or may not have psychotic features, such as bipolar disorders and major depressive disorder. First-generation antipsychotic drugs are more likely to cause adverse effects such as extrapyramidal symptoms and tardive dyskinesia. Adverse effects of second-generation antipsychotic drugs typically are related to metabolic abnormalities such as weight gain, abnormal blood glucose levels, and elevated lipid levels. Neuroleptic malignant syndrome is a rare but serious adverse effect

Time to inclusion of selected medicines for priority diseases in National Essential Medicines Lists compared with the WHO Model List.

INTRODUCTION: The WHO maintains a Model List of Essential Medicines, which guides countries in developing their National Essential Medicines List (NEML) to improve access to medicines. We aimed to assess the time it took for countries to adjust their NEMLs when medicines were added or deleted from the WHO Model List and if this differed between priority diseases. METHODS: We extracted medicines added or deleted from the WHO Model List (2007-2021) for selected priority diseases: diabetes, hepatitis C, HIV, oncology and tuberculosis. These medicines were compared with NEMLs from 20 countries; r

Thalamic Deep Brain Stimulation.

The use of deep brain stimulation (DBS) of the thalamus has been proven to be a safe and efficacious treatment for the management of many diseases. The most common indication for thalamic DBS remains essential tremor (ET), one of the most common movement disorders in the world. ET patients should be considered for surgical intervention when their tremor has demonstrated to be refractory to medication, a characteristic estimated to be present in roughly 50% of ET cases. Advantages of DBS over thalamotomy include its reversibility, the ability to adjust stimulation settings to optimize efficacy

Generation and Validation of the iKp1289 Metabolic Model for Klebsiella pneumoniae KPPR1.

Klebsiella pneumoniae has a reputation for causing a wide range of infectious conditions, with numerous highly virulent and antibiotic-resistant strains. Metabolic models have the potential to provide insights into the growth behavior, nutrient requirements, essential genes, and candidate drug targets in these strains. Here we develop a metabolic model for KPPR1, a highly virulent strain of K. pneumoniae. We apply a combination of Biolog phenotype data and fitness data to validate and refine our KPPR1 model. The final model displays a predictive accuracy of 75% in identifying potential carbon

Zonisamide for essential tremor: an evaluator-blinded study.

In this evaluator-blinded open-treatment trial, subjects with moderate/severe upper limb essential tremor were titrated to 300 mg/day zonisamide, or adjusted to a lesser dose if symptoms warranted, as monotherapy or as adjunct to stable antitremor medication, followed by a 12-week extension phase. The primary efficacy outcome variables were blinded rater videotaped/drawing tremor score changes at the Treatment and Extension visits compared to Baseline, based on Fahn-Tolosa-Marin and Postural Tremor Scales. Subjects also rated Functional Disabilities. Primary outcomes showed reduced tremor scor

[Cerebral venous thrombosis secondary to essential thrombocythemia].

INTRODUCTION: Essential thrombocythemia (ET) is a myeloproliferative syndrome; cerebral venous thrombosis (CVT) is a rare complication. OBSERVATION: We report the case of a 20-year-old woman with an uneventful history who was admitted with intracranial hypertension syndrome which had developed over the last four months in association with bilateral decline of visual acuity. Physical examination at admission revealed stage II papilloedema and absence of any focal neurological signs. The brain MRI and the venous MRA showed a thrombosis involving the superior longitudinal sinus and the lateral s

Robust Salmonella metabolism limits possibilities for new antimicrobials.

New antibiotics are urgently needed to control infectious diseases. Metabolic enzymes could represent attractive targets for such antibiotics, but in vivo target validation is largely lacking. Here we have obtained in vivo information about over 700 Salmonella enterica enzymes from network analysis of mutant phenotypes, genome comparisons and Salmonella proteomes from infected mice. Over 400 of these enzymes are non-essential for Salmonella virulence, reflecting extensive metabolic redundancies and access to surprisingly diverse host nutrients. The essential enzymes identified were almost excl

Indications for lowering platelet numbers in essential thrombocythemia.

Essential thrombocythemia (ET), one of the chronic myeloproliferative disorders, exposes individuals to significantly increased risk for thrombohemorrhagic complications. Epidemiologic data indicate that the two most prominent risk factors for thrombosis are age greater than 60 years or a history of or presentation with thrombosis at any age. Age is an important factor in selecting among therapeutic options, as the agents used to treat ET may contribute to acute leukemic transformation and other secondary malignancies. Whether or not hydroxyurea (HU) carries these risks is controversial and un

Therapy of essential thrombocythemia with thiotepa and chlorambucil.

Twenty-seven patients with essential thrombocythemia were treated with combination chemotherapy consisting of weekly intravenous thiotepa (until the platelet count fell below 1,000 X 10(3)/cu mm) and daily chlorambucil (until a sustained remission, platelet count less than 400 X 10(3)/cu mm). All patients responded promptly, platelet counts fell to below 1,000 X 10(3)/cu mm by 1-3 wk, and were less than 400 X 10(3)/cu mm by 2-6 wk. Remission of thrombocytosis was accompanied by an improvement of the patients' symptoms and resolution of the splenomegaly that was present in 21 of the 27 patients

[Epileptic encephalopathies in infancy. How do we treat them? Does the aetiology influence the response to treatment?].

INTRODUCTION: Resistance to treatments is a common feature of Ohtahara, Aicardi, West and Dravet syndromes, as well as malignant migrating epilepsy in infancy. AIMS: To update the therapeutic management and to analyse whether the aetiology somehow determines the treatment. DEVELOPMENT: Convulsive seizures in the first year of life may be due to a potentially treatable aetiology, which makes it essential to carry out a complete evaluation so as to be able to begin, as early as possible, the most suitable and the non-specific symptomatic treatments to control the seizures, which prevents or mi

Botanicals in Dermatology: Essential Oils, Botanical Allergens, and Current Regulatory Practices.

Largely because of their perceived safety, the use of essential oils and other botanically derived products has become increasingly popular. Recent evidence raises concern about the safety of these products, frequently found in cosmetics and sought as an alternative to standard medical treatments. Essential oils are challenging to standardize because of the variable growing conditions, genetics, and harvesting of botanicals. There exists a potential for adverse reactions, in particular allergic contact dermatitis. Furthermore, these products are often sold without prior Food and Drug Administr

Spironolactone prevents Na+/H+ exchange enhancement in primary aldosteronism.

The study was undertaken to determine the possible effect of an aldosterone antagonist, spironolactone (SP), on red blood cell sodium-hydrogen exchange (NHE) enhancement in primary aldosteronism (PA) and essential hypertension (EH). NHE was measured as the amiloride-inhibited fraction of H+ efflux (V max) from erythrocytes (pHi 6.40 +/- 0.05) into a Na+-containing medium (pHo 8.00 +/- 0.05). Subjects were 12 hypertensive patients with aldosterone-producing adrenal adenoma (six treated with 200 mg/day spironolactone for an least 5 days and six drug-free), 20 essential hypertensives (10 treated

A computational subtractive genome analysis for the characterization of novel drug targets in Klebsiella pneumonia strain PittNDM01.

The emergence of carbapenem-resistant Klebsiella Pneumoniae had been reported previously, which needs rapid attention. Currently, Pittsburgh University Hospital reported a new strain of carbapenem-resistant Klebsiella pneumoniae that was co-producing OXA-232 and NDM-1 named as PittNDM01. This strain is resistant to almost all beta-lactam antibiotics such as Carbapenem as well as to fluoroquinolones and aminoglycosides. Globally, failure to the wide-spread pathogenic strains had been observed due to the increased and antibiotic resistance, which leads to less antimicrobial drug efficacy. Since
