Duodenal villous adenoma.

Duodenal villous adenomas are extremely rare (1/5th of small bowel tumors). In view of their strategic position and risk of malignancy, they require definitive management in specialized care units. We report a case of a 35 year old female patient with a large duodenal (5 cm) villous adenoma causing intussusception of duodenum (a phenomenon rare because of its anatomic location & fixation).

Duodenal ulcer disease.

An overview is given of the pathogenic mechanisms involved in Helicobacter pylori-associated duodenal and gastric ulceration. Special attention is given to the role of microbial virulence factors, the effects on gastric acid secretion and the development of 'gastric type' metaplasia in the duodenal bulb. Cure of the infection essentially eliminates ulcer recurrence. Cure also prevents ulcer-related bleeding. Antibiotic regimens providing a reliable high cure rate are briefly discussed. Finally the questions about ulcer disease which remain unanswered are summarized.

Refractory duodenal ulcer.

Refractory or intractable ulcer is defined as an ulcer that fails to heal completely after eight to twelve weeks, despite appropriate treatment with a modern antiulcer therapy in a compliant patient. Refractory ulcer should be suspected in individuals diagnosed to have peptic ulcer if their symptoms persist longer than usual: occurrence of complications or simply their ulcers fail to heal, since up to 25% of such patients remain asymptomatic. Conditions associated with refractory ulcer include noncompliance, continuous consumption of nonsteroidal anti-inflammatory drugs, acid hypersecretion, s

Management of duodenal trauma.

The surgical management of fifteen cases of duodenal trauma is reported. Blunt injury of the abdomen was the commonest mode of injury. Thirteen patients (86.6%) had associated intra-abdominal and/or extra-abdominal injuries. Diagnosis of duodenal injury was made at laparotomy in all patients. In the majority of patients (n = 11), either duodenal repair, or anastomosis with the pylorus or jejunum was done. Duodenal diverticulization or pancreaticoduodenectomy were undertaken in 2 patients each. Feeding jejunostomy was an important adjunct for nutritional support. Septicemia with multisystem org

Developments in duodenal ulceration.

Fiberoptic endoscopy is now the standard technique for the precise diagnosis of active duodenal ulceration. Most doctors choose to treat acute ulcer patients using a full dose of a histamine H2-receptor antagonist at the time of ulcer relapse, selecting long-term maintenance treatment for those patients with either aggressive ulceration, old age, or serious coincidental medical illnesses. Helicobacter pylori appears to be an important factor in the chronicity of duodenal ulceration, and those patients with difficult or aggressive ulceration may benefit from an attempt to eradicate the organism

[Duodenal atresia and heterotaxia].

We report two cases of duodenal atresia associated with corporal symmetry changes. The one of them presented a "situs inversus total" and the other one a Ivermak syndrome. The association between duodenal atresia and Ivermak syndrome has a very low incidence. We have not found references with respect to the association between duodenal atresia and "situs inversus totalis". The embryo-genetic hypothesis that could relate both anomalies are discussed. Perhaps are not caused by chance.

Congenital duodenal obstruction.

Over the period 1980-1989, 49 patients with congenital duodenal obstruction due to duodenal atresia, duodenal stenosis or annular pancreas were treated. There were three deaths, all related to associated severe congenital anomalies and none related to surgery for duodenal obstruction; 78 per cent of the patients had associated congenital anomalies which, in some instances, complicated their management. Prenatal diagnosis by ultrasonography was made in 18 per cent of the patients and these patients had operations relatively earlier.

Duodenal webs in the adult.

Duodenal webs and diaphragms in the adult are congenital in origin and are rare. They are explained embryologically by the process of duodenal epithelial plugging and subsequent revacuolization. The symptoms may be insidious and appear late in life and consist of partial gastric outlet complaints. Two thirds of the patients will have other upper GI problems, some of which may be related to the antral distension and stasis produced by the web. Peptic ulcer disease is common but may resolve when the web is excised. Diagnosis can be made by contrast duodenography but is more definitive with endos

Duodenal ulcer in the elderly.

Of 333 duodenal ulcer (DU) patients 75 (22.5%) were aged 65-93 years (study group). Ninety-two percent (306 patients) of the entire group were diagnosed endoscopically, and all were followed prospectively. In the study group of the older patients there were fewer smokers, but more patients used steroids and other nonsteroidal antiinflammatory drugs (NSAIDs) and had more arteriosclerotic heart disease than the younger control group. Presenting signs and symptoms were similar in both age groups, although painless upper gastrointestinal bleeding was more common in the elderly, and pain, when pres

Prostaglandins for duodenal ulcer.

Studies to date indicate that certain synthetic prostaglandin E derivatives, notably misoprostol and enprostil, possess duodenal ulcer healing efficacy that is equivalent to that of the existing H2-receptor antagonists. Whether healing is achieved by virtue of their antisecretory action alone or in combination with their cytoprotective properties cannot be ascertained. Earlier reports that non-antisecretory prostaglandin E2 was effective for duodenal-ulcer healing suggested that cytoprotective mechanisms may contribute to ulcer healing, but this needs be confirmed. Duodenal patch grafting.

Loss of duodenal tissue around the pancreatic and common bile ducts presents a challenge to abdominal surgeons. Available methods to correct the problem include duodenojejunostomy, diamond duodenoplasty, serosal patching, mucosal pedicle patching from the ileum or jejunum, and gastric island patching. The choice of procedure must be tailored to the nature of the defect, the amount of tissue lost and the amount of peritoneal contamination. We believe that jejunal mucosal pedicle patching is a reasonable choice in most instances.
