Silk Devices for Tympanic Membrane Repair Show No Ototoxicity in a Rat Model.

HYPOTHESIS: Implantation of ClearDrum ® , a biodegradable silk fibroin device, into the middle ear (ME) cavity in a rat model will not affect hearing thresholds as measured by auditory brainstem response. BACKGROUND: Chronic otitis media (COM) is a recurrent middle ear infection often accompanied by tympanic membrane perforation. Our laboratory has developed a biodegradable silk fibroin device (ClearDrum®) designed to treat tympanic membrane perforations in COM. ClearDrum ® acts as a prosthetic eardrum, providing a substrate on which tympanic membrane cells can grow and acts as a long-lasting

Idiopathic incus necrosis: Analysis of 4 cases.

We evaluated ossicular chain reconstruction in patients with idiopathic incus necrosis who have conductive hearing loss and an intact ear drum. The study included four patients (3 women and 1 man; the ages of the patients were 22, 31, 35, and 56 years, respectively) with unilateral conductive hearing loss, no history of chronic serous otitis media, an intact ear drum, normal middle ear mucosa, and necrosis of the long processes of the incus. On preoperative pure tone audiometry, air-bone gaps were 24, 25, 38, and 33 dB. Bilateral tympanometry and temporal bone computed tomography results were

Endoscopic cartilage tympanoplasty: A two-handed technique using an endoscope holder.

OBJECTIVES/HYPOTHESIS: Endoscopic ear surgery provides a minimally invasive approach to the middle ear. The disadvantage of endoscopic ear surgery is that it is a single-handed surgical technique. The nondominant hand of the surgeon is utilized for holding and manipulating the endoscope. This necessitated the need for the development of an endoscope holder that would allow both hands to be free for surgical manipulation. The aim of this article is to report our preliminary experience using our newly designed and developed endoscope holder, which allowed us to perform cartilage tympanoplasty ut

Closed anterior scoring for prominent-ear correction revisited.

The closed-anterior-scoring technique has been used over the past 3 years to correct 56 prominent ears in 32 patients at the West Midlands Regional Plastic Surgery Unit at Wordsley Hospital. A review was carried out to assess the result of this surgical procedure. We briefly discuss the historical development of other surgical techniques for prominent-ear correction, and describe in detail the operative technique for this procedure, which includes closed scoring and suturing of the cartilage. We used this technique to treat 24 patients with bilateral prominent ears and eight patients with unil

Chondrodermatitis nodularis helicis: a transepidermal perforating disorder.

Chondrodermatitis nodularis helicis is a chronic disorder occurring exclusively on the ear. It is most common in the sixth decade, and two thirds of the patients are men. Although it has no single characteristic histologic feature, a diagnosis can be made with certainty with a clinical history and a combination of morphologic changes seen in the cartilage and overlying skin. The natural history and histology are similar to a group of disorders characterized by transepithelial elimination known as the perforating dermatoses.

Developmental disorders of the ear in children and adolescents: conservative and surgical treatment options.

BACKGROUND: Developmental disorders of the ear can impair hearing and cause cosmetic deformities. In recent years, new surgical treatments have become established, above all in audiological rehabilitation. METHOD: We selectively searched the PubMed database up to May 2013 for publications in English and Germanabout the therapeutic options. RESULTS: No randomized trials have been performed, for both ethical and practical reasons (inadmissibility of placebo surgery, specialization of surgeons for individual techniques). To correct prominent ears, cartilage-sparing suture techniques are preferr

Calcification of auricular cartilages in patients with hypopituitarism.

Primary adrenal insufficiency is the most frequent systemic disorder associated with calcification of auricular cartilage. The pathogenesis of this sign has been attributed to ACTH excess or to prolonged desoxycorticosterone acetate therapy. We describe here three patients with long-standing untreated hypopituitarism and calcified auricular cartilages. Replacement hormonal therapy resulted in improvement in their general condition, but did not change the auricular calcification. Analysis of these cases as well as review of the literature suggest that calcification of auricular cartilage in bot

Development of a Method for Scaffold-Free Elastic Cartilage Creation.

Microtia is a congenital aplasia of the auricular cartilage. Conventionally, autologous costal cartilage grafts are collected and shaped for … ng scaffold materials. Chondrocyte progenitor cells were collected from perichondrial tissue of the ear cartilage. By using a multilayer culture and a three-dimensional rotating suspension culture vessel system, we succeeded in creating scaffold-free elastic cartilage from cartilage progenitor cells.

Changes in the Transcriptome and Proteome of Cartilage in Microtia.

Congenital microtia is a severe physiological defect and is among the most common craniofacial defects. It is characterized by severe auricle dysplasia, external auditory canal atresia or stenosis, and middle ear malformation, though inner ear development is mostly normal with some hearing occurring through bone conduction. Auricular reconstruction is the only treatment for congenital microtia. In this study, the authors integrated messenger ribonucleic acid and mass spectrometry data of cartilage obtained from the affected and unaffected sides of 16 unilateral microtia patients who had underg

Preauricular flap for post burn ear lobe reconstruction - a case report.

Burns of the face often affect the ear causing deformation of the pinna or ear lobe due to perichondritis or loss of cartilage and excess scarring. The reconstruction of the ear lobe may be limited due to scarring of the surrounding tissues. We describe a preauricular flap for reconstruction of the anterior and posterior surface of the ear lobe following burn injury. This two stage technique of ear lobe reconstruction by a preauricular flap is not described in the available literature. An inferiorly based preauricular flap was harvested in a young lady who had post burn loss of the right ear l

Minimally invasive technique of auricular cartilage harvest for tissue engineering.

Tissue engineered human cartilage is presently being utilized in clinical research programs in a variety of medical disciplines including otolaryngology, urology, and orthopedics. In this study, we present a new methodology for auricular cartilage harvest that can be applied to tissue engineering. Eight 16-week-old pigs were subjected to a traditional open cartilage harvest technique involving suture closure, while the other ear was subjected to the closed stitchless cartilage harvest, using a 12-gauge core biopsy needle. Surgical time was significantly (p < 0.0001) shorter (3.5 +/- 2.8 min fo

A combined technique for correction of the prominent ear.

PURPOSE: The literature illustrates a large number of different techniques for the correction of the prominent ear, and the great variety available is indicative of how difficult it is to achieve satisfactory results in all cases. This report describes a procedure for successfully treating such patients. PATIENTS AND METHODS: Forty patients were treated with a surgical procedure creating a new anthelical plica based on the Stenström and Mustardé method and producing an anatomic cavity in the mastoid region where the concha can be repositioned according to the Furnas method. Bilateral same-day endoscopic transcanal cartilage tympanoplasty: initial results.

INTRODUCTION: Same-day closure of bilateral tympanic membrane perforations is a quick and more comfortable procedure for the patients. However, conventional bilateral same-day tympanoplasty or myringoplasty has been rarely performed because of the theoretical risk of postoperative complications. OBJECTIVE: To evaluate the advantages and outcomes of bilateral simultaneous endoscopic cartilage tympanoplasty in patients with bilateral tympanic membrane perforations. METHODS: From February 2012 to March 2013, patients with bilateral dry tympanic membrane perforations who had some degree of heari

CT manifestations of tracheobronchial involvement in relapsing polychondritis.

Two patients with relapsing polychondritis presented with tracheobronchial stenosis without clinical manifestation of nasal septum or ear lobe cartilage involvement. The CT findings included diffuse, smooth tracheobronchial wall thickening with narrowing and deformity of the lumen. Areas of dense calcium deposition within the thickened tracheal cartilage were seen. On follow-up after steroid medication, tracheobronchial wall thickening was decreased and the configuration of the tracheal lumen returned to normal.
