Total elbow arthroplasty for primary osteoarthritis.

BACKGROUND: Primary osteoarthritis of the elbow is a less common indication for total elbow arthroplasty (TEA). Higher complication rates in younger, active patients may offset short-term improvements in pain and function. The purpose of this study was to determine pain relief, functional outcomes, complications, and survival of TEA in this population. METHODS: Between 1984 and 2011, 20 consecutive TEAs were performed for primary elbow osteoarthritis. Two patients died before the 2-year follow-up. Mean age at surgery was 68 years (range, 51-85 years). Outcome measures included pain, motion, M…

Pediatric elbow arthroscopy: indications and safety.

BACKGROUND: Elbow arthroscopy is a minimally invasive means by which to treat a variety of acute and chronic elbow conditions. Although the safety and efficacy is well documented in the adult population, comparatively little information is available about the role of elbow arthroscopy in the pediatric population. This study reports the indications for and safety of elbow arthroscopy in a series of pediatric patients. METHODS: A retrospective review was performed from 2001 to 2015 of a surgical database at a single institution. All elbow arthroscopies performed in patients aged 18 years and yo…

Bipedicle Flaps for Posterior Elbow Reconstruction.

Chronic posterior elbow soft tissue defects often require soft tissue reconstruction because of exposure of the underlying triceps tendon and proximal ulna. Current options for soft tissue coverage require sacrifice of a local muscle or microsurgery. The purpose of this study is to evaluate patient and surgical outcomes after reconstruction of small-sized to medium-sized (<50 cm) posterior elbow defects with bipedicle advancement flaps. A retrospective chart review was performed for 3 patients who underwent posterior elbow reconstruction with bipedicle flaps. The etiology of the soft tissue el…

Chronic Elbow Dislocation: Evaluation and Management.

Chronic elbow dislocation is defined as a dislocation that has remained unreduced for >2 weeks. The soft-tissue and skeletal changes that develop during this time usually prevent successful closed reduction. These changes include the development of extensive intra-articular fibrotic tissue, as well as contracture of the triceps, collateral ligaments, and elbow capsule. Ulnar nerve involvement and associated fractures may also be present. Because treatment of chronic elbow dislocation is challenging, a stepwise approach is used in the evaluation and management of this condition. No large series…

Management of Adult Elbow Fracture Dislocations.

Elbow fracture dislocations are complicated injuries that are difficult to manage and fraught with complications. A complete series of radiographs is typically complemented with CT scan to evaluate the elbow and assist preoperative planning. Typically, operative intervention is necessary and a systematic approach to the elbow injuries should be chosen. This article addresses the coronoid and proceeds to the radial head, lateral soft tissues, and finally the medial ligaments if elbow instability persists. With a focused, systematic surgical approach, improved outcomes have been demonstrated and…

Long-term outcome of elbow resurfacing.

BACKGROUND: The purpose of this study is to review the long term results the Kudo and instrumented Bone Preserving elbow prostheses. The instrumented Bone Preserving prosthesis is the successor of the Kudo prosthesis, and both of these are nonconstrained elbow resurfacing prostheses. METHODS: Fifty-five nonconstrained elbow prosthesis were implanted in 51 patients. Patients were evaluated with the Mayo Elbow Performance Score (MEPS) and Disabilities of the Arm Shoulder and Hand score. Revision or the necessity of revision was regarded as failure of the prosthesis. RESULTS: Twenty-one prosthe…

Arthroscopic arthrolysis for posttraumatic elbow stiffness.

BACKGROUND: Loss of motion of the elbow joint is a common finding after elbow trauma. Restoration of motion of the posttraumatic stiff elbow can be a difficult, time-consuming, and costly challenge. Arthroscopic capsular release of stiff elbows has recently been introduced as a safe but technically demanding technique. The outcome in 27 patients treated by arthroscopic capsular release was assessed. MATERIALS AND METHODS: We evaluated 27 patients (17 women) who were an average age of 42 years (range, 14-65) at 3, 12, and 24 months after arthroscopic capsular release of a posttraumatic stiff e…

[Open arthrolysis of posttraumatic elbow stiffness].

BACKGROUND: The incidence of relevant posttraumatic functional deficits in the sense of elbow stiffness with less than 30 degrees in extension or flexion less than 120 degrees , is unknown. A differentiation can be made between intraarticular, extraarticular and combined causes. An open procedure is indicated in elbow stiffness after correct analysis of the situation and failure of conservative treatment. MATERIALS AND METHODS: An open procedure was carried out on 35 patients between March 1995 and November 2001, 10 (3-24) months after the trauma. The mean age of the 24 men and 11 women was 4…

Posterolateral elbow impingement in professional boxers.

BACKGROUND: Elbow injury is common in boxing, but it has not been reported in the literature. The onset is often a hyperextension trauma caused by a missed hit. Clinically the boxers complain of pain, stiffness, and an extension deficit. PURPOSE: To evaluate the pathogenesis, diagnostic approach, and arthroscopic treatment of elbow injury in boxers, and to compare these with other sports-related elbow injuries. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Between 2003 and 2005, a group of 5 professional boxers received a diagnosis of posterior elbow impingement. An arthroscopic…

Elbow arthroscopy: where are we now?

Elbow arthroscopy has developed over the past several years. Indications have evolved from simple loose body removal to the treatment of rheumatoid arthritis. There have been few published reports of elbow arthroscopy compared to reports of knee or shoulder arthroscopy. Complications are more frequently reported after elbow arthroscopy than after arthroscopy of larger joints; therefore, careful attention to detail is necessary to help prevent neurovascular complications. Elbow arthroscopy can be a useful technique for the orthopaedic surgeon. This review will describe the operative setup and a…

Ulnar nerve palsy after elbow arthroscopy.

Reports of ulnar nerve injury as a result of elbow arthroscopy are rare in the literature. We report a case of ulnar nerve injury following arthroscopic debridement and retrograde drilling of the capitulum in a patient with symptomatic osteochondritis dissecans. The standard location of proximal medial portal placement is 2 cm proximal to the medial epicondyle at the level of the medial intermuscular septum. In this location, the ulnar nerve is protected from injury by the medial intermuscular septum. Extending this placement more proximally may negate this protection, leaving the nerve more s…

Elbow arthroscopy: surgical techniques and rehabilitation.

Elbow arthroscopy allows for direct visualization into the elbow joint, minimizes the potential for postoperative edema and discomfort, as well as protects the ligamentous structures. Arthroscopic procedures for the elbow and postoperative management are described for patients who have undergone loose body removal, synovectomy, and capsulectomy. The effect of early mobilization on the elbow complex and the role that splinting may play, as well as the controversies surrounding the use of continuous passive motion are discussed. Arthroscopy can significantly reduce the time frame, as well as imp…

[Arthroscopic treatment of acute elbow fractures.].

Surgical management of the rare thrue articular fractures of the elbow remain difficult: the surgeon must achieve anatomical reconstruction with a reliable fixation allowing mobilisation of the joint for early rehabilitation. Arthroscopic treatment of elbow fractures (capitellum, coronoid, olecranous and radial head) gives an excellent intra articular view with few soft tissue damage. Cartilage damage, intra articular loose bodies and capsulolabral injuries are then well adressed. Arthroscopic management of elbow fracture is potentially dangerous for the neuro-vascular structures and requiere …

Polyethylene wear after total elbow arthroplasty.

BACKGROUND: Articular wear is considered to be a possible long-term complication of the use of stemmed, coupled elbow replacements with the capacity to correct deformity and restore function. There have been no reports on this topic, to our knowledge. METHODS: A review of the results of 919 replacements with the semi-constrained linked Coonrad-Morrey total elbow implant, performed between 1981 and 2000, revealed that twelve patients (1.3%) had undergone an isolated exchange of the articular bushings as a result of polyethylene wear. The status of these patients was assessed clinically and rad…

Anterior elbow dislocation with recurrent instability.

Anterior elbow dislocations are often associated with severe soft tissue lesions. The mechanism of dislocation is usually an anteriorly directed blow on the proximal ulna after a fall on a flexed elbow. Simple anterior elbow dislocations are traditionally treated by closed reduction followed by a short period of immobilisation. Fracture-dislocations are usually unstable after reduction and necessitate surgical management. The authors report a case of acute anterior dislocation with a concomitant radial head fracture that was complicated by recurrent instability. The literature is reviewed to d…

Traumatic nerve injuries about the elbow.

Although not common, nerve injuries about the elbow occur because of the proximity of the three upper extremity nerves or because of the relationship of the median, ulnar, and radial nerves to the bony and soft-tissue structures about the elbow joint. Nerve injuries at and about the elbow joint occur more frequently with fractures than with any other kind of trauma. Nerve injuries may be found with periarticular fractures, dislocations, gunshot wounds, lacerations, and other iatrogenic causes.
