Incidence of elbow involvement in rheumatoid arthritis. A 15 year endpoint study.

OBJECTIVE: To evaluate the incidence of involvement and cause of destruction of humeroulnar (HU) and humeroradial (HR) joints in a prospectively followed cohort of 74 patients with seropositive and erosive rheumatoid arthritis (RA). METHODS: At the 15 year followup standard anteroposterior and lateral radiographs of 148 elbow joints were evaluated, and the grade of destruction was assessed by the Larsen method. RESULTS: Erosive involvement (Larsen grade 2) was observed in 75/148 (51%) elbows in 45/74 (61%) patients; 30 patients had bilateral and 15 unilateral involvement. The incidence of mi

[Classification of elbow stiffness and indications for surgical treatment].

An elbow can become stiff for a variety of reasons, such as intra-articular or extra-articular fractures, soft-tissue trauma, prolonged immobilization, thermal injury, infection, inflammatory arthritis, osteoarthrosis and heterotopic bone formation. Elbow stiffness is usually classified into extrinsic (affecting the capsule and extra-articular soft tissues), intrinsic (affecting the synovial and intra-articular structures) and mixed forms. Indications for operative treatment have to be considered in cases of failed conservative treatment with severe functional deficits. The choice of operative

[Assessment of ligament and tendon injuries of the elbow using magnetic resonance imaging].

An overview about the normal anatomy and frequent pathologies of tendons and ligaments of the elbow using MRI will be provided. The unique joint configuration and articulation, as well as passive ligamentous and active muscle structures as well as the unique configuration of the articulating components of the elbow contribute to joint stabilization. MRI is an essential imaging modality in patients with ligamentous injuries including the sequelae of joint dislocation as well as chronic pathologies such as long-standing and refractory tendinopathies. Ideally, when reporting MRI of the elbow, the

Systematic Review of Elbow Instability in Association With Refractory Lateral Epicondylitis: Myth or Fact?

BACKGROUND: Elbow instability, particularly posterolateral rotatory instability (PLRI), has been reported in patients with refractory lateral epicondylitis (LE). However, evidence of diagnostic approach and surgical outcomes is lacking. PURPOSE: To identify (1) the risk factors, clinical and radiologic-diagnostic characteristics, and (2) the treatment options and clinical outcome of LE with PLRI. STUDY DESIGN: Systematic review. METHODS: We searched the PubMed, Ovid/MEDLINE, Cochrane Library, Google Scholar, Scopus, and EMBASE databases using keywords as well as Medical Subject Headings ter

The results of surgical treatment for posttraumatic heterotopic ossification and ankylosis of the elbow.

OBJECTIVES: Heterotopic ossification which may develop following elbow injuries or elbow surgery may result in complete loss of elbow functions. We evaluated the results of surgical treatment for ankylosis of the elbow due to posttraumatic heterotopic ossification. METHODS: The study included seven patients (6 males, 1 female; mean age 36 years; range 23 to 55 years) who developed heterotopic ossification and ankylosis of the elbow joint following surgical treatment of high-energy fractures in the circumference of the elbow. Two patients had comminuted olecranon fractures and elbow luxation,

Treatment of Unstable Elbow Injuries with a Hinged Elbow Fixator: Subjective and Objective Results.

INTRODUCTION: Injuries around the elbow pose a challenging problem for orthopaedic surgeons. The complex bony architecture of the joint should be restored and the thin soft tissue envelope needs to be handled with meticulous care. Elbow instability is a complication seen after dislocations and fractures of the elbow and remains a treatment challenge. The purpose of this study was to provide subjective and objective results following the surgical treatment of unstable elbow dislocations with an external hinged fixation technique. METHODS: Forty-six consecutive patients with complex trauma of t

[Does simple posterior dislocation of the elbow necessitate strict immobilization?].

OBJECTIVE: To research for the ideal treatment of isolated posterior dislocation with the elbow after reduction. METHODS: 36 patients of isolated posterior dislocation of the elbow after closed reduction (male 26, female 10; left 14, right 22; dominant extremity 21, undominant extremity 15; average age 22) were examined by varus and valgus stress test and push-draw test. Satisfactory stability from 30 degrees to 130 degrees flexion of the elbow was found in all patients. For the 30 patients below 35 years old, long-arm posterior splint was applied to immobilize the elbow joint at 90 degrees f

[Standardised ultrasound scanning of the elbow].

Musculoskeletal ultrasound (US) is noninvasive, nonionisating and cost-effective imaging diagnostic technique. It is a useful imaging modality for the diagnosis of joint and soft tissue pathology and can be considered as an extension of physical examination. Elbow is easily accessed due to its superficial position. A high resolution, multi-frequency (10-15 MHz) linear transducer should be used. US investigation of the elbow includes scanning of the anterior, medial, lateral and posterior part of the joint. Common flexor and extensor origin at the humearal epicondyles, triceps tendon, distal bi

Ulnar Collateral Ligament and Elbow Adaptations in High School Baseball Pitchers.

BACKGROUND: Baseball pitchers have adaptive changes in the soft tissues of the throwing elbow. HYPOTHESIS: High school baseball pitchers would show adaptive changes in the ulnar collateral ligament (UCL), such as calcifications and hypoechoic foci, thickening, and increased ulnohumeral joint laxity, on dynamic ultrasound (DUS). STUDY DESIGN: Cross-sectional study. LEVEL OF EVIDENCE: Level 3. METHODS: Twenty-two asymptomatic high school pitchers, designated as their primary position by their coach, underwent DUS and physical examination of the throwing and nonthrowing elbows prior to the st

Primary Synovial Osteochondromatosis of the Elbow in a Young Adult Complicated by Osteoarthritis: A Case Report.

Primary synovial osteochondromatosis is a benign and uncommon pathology, resulting from metaplasia of the synovial tissue, of unknown cause, leading to the formation of multiple cartilaginous nodules, some of which secondarily detach to form free articular bodies. Its location in the elbow, complicated by osteoarthritis, is poorly described in the literature. We present the case of a 25-year-old man presenting for chronic right elbow pain with joint locking and swelling without a history of trauma. Physical examination revealed an increase in elbow volume compared to the contralateral elbow, w

Ultrasound of the elbow.

Ultrasound examination of the elbow should be performed in a comparative way, exploring joint spaces and fossas. Ultrasound is the method of choice for the detection of joint fluid and loose bodies. Tendinitis and microfissures can be demonstrated in case of tennis and golfer elbow. On the posterior aspect of the elbow, ultrasound shows the different types of bursitis, and allows evaluation of the ulnar nerve in cases of impingement.

[Indications for operation in elbow dislocation].

This paper briefly presents the anatomy of the elbow joint, its pathophysiology, the various types of dislocation of the elbow joint and concomitant injuries, the appropriate diagnostic procedures and the indications for surgery and the operative technique applied for correction of dislocation. The only urgent indications for operative treatment are elbow joint dislocation with concomitant bone injuries, persistent instability or luxation position, open injuries and vessel and/or nerve injuries. For purely ligamentous lesions combined with relative loss of stability an operative procedure does
