Vascular mechanisms in Meniere's disease.

Normal venous drainage of the vestibular organs through the vein of the paravestibular canaliculus (PVC) may be crucial to inner ear fluid m … uggested mechanism is morphologic change in the microcirculation of the intermediate portion of the endolymphatic sac. Microcirculation changes may be associated with fibrosis of the perisac tissues or shortening of the intermediate sac region or might be physiologically determined. Venous insufficiency may also result from anomalies of the PVC vein.

Measurement of action potential thresholds in experimental endolymphatic hydrops.

Unilateral experimental endolymphatic hydrops was created by endolymphatic sac and duct obstruction in guinea pigs. Studies of action potential (AP) threshold were then performed and compared with histologic findings. Action potential thresholds were significantly increased in the hydrops ears. The close relationship between AP changes and hydrops was sufficient to allow the detection of hydrops by AP threshold measurements alone. The AP threshold changes were positively related to the degree of hydrops and postoperative interval in a manner consistent with hearing changes seen in Meniere's di…

Calcium gradients in inner ear endolymph.

Recent studies suggest that endolymphatic hydrops resulting from the ablation of the endolymphatic duct and sac in guinea pigs may be caused by a disturbance of endolymph calcium homeostasis. A similar disturbance of calcium homeostasis could represent the underlying cause of Ménière's disease. In this study, we mapped the calcium concentrations and electrical potentials throughout the endolymphatic system in normal guinea pigs. Large concentration differences exist between different compartments, including a more than twofold increase along the length of the cochlea. The electrochemical poten

Three-dimensional CT evaluation of anatomical constraints during posterior lip drilling in the retrosigmoid approach to the internal auditory canal.

PURPOSE: To develop a CT-based three-dimensional morphometric framework for evaluating the depth and orientation of posterior lip drilling during the retrosigmoid approach. METHODS: One hundred normal temporal bones from 50 adults were reconstructed from high-resolution CT. Five surgically relevant landmarks were identified at the endolymphatic sac-duct junction, posterior internal auditory canal (IAC) porus, common crus, lateral IAC fundus, and Bill's bar. Six linear distances and four angular relationships were measured to characterise the drilling corridor relative to the IAC, labyrinth, a…

Nervous system involvement in von Hippel-Lindau disease: pathology and mechanisms.

Patients with von Hippel-Lindau disease carry a germline mutation of the Von Hippel-Lindau (VHL) tumor-suppressor gene. We discuss the molec … tem, the origin of which has not yet been entirely clarified, and current hypotheses are discussed. Endolymphatic sac tumors may extend into the brain, but originally arise from proliferation of endolymphatic duct/sac epithelium; the exact nature of the proliferating epithelial cell, however, also has remained unclear, as well as the question why tumors almost cons …

Auditory threshold and inner ear pressure: measurements in experimental endolymphatic hydrops.

HYPOTHESIS: Experimental endolymphatic hydrops leads to an endolymph-perilymph pressure imbalance that is responsible for the loss of auditory sensitivity in the ear. BACKGROUND: This study investigates whether intralabyrinthine pressure is a factor in the auditory dysfunction of experimental endolymphatic hydrops. METHODS: Auditory function was investigated in 10 guinea pigs 90 to 120 days after endolymphatic sac ablation by measuring compound action potentials in response to acoustic stimuli including alternating clicks and tone bursts of 0.5, 1, 2, 4, and 8 kHz. After auditory thresholds …

Pathophysiology of endolymphatic hydrops.

Endolymphatic hydrops of the nonprogressive type occurs in response to a single traumatic or toxic insult of limited duration and although it may result in permanent deficits in sensorineural function, there is total subsidence of vestibular symptoms. Endolymphatic hydrops of the progressive type, on the other hand, appears to be the result of permanent impairment of endolymph resorption and is caused principally by disorders of the endolymphatic sac. It occurs in Menière's disease, syphilitic labyrinthitis and the delayed hydrops syndrome. In addition to deafness of varying extent, it is char…

Meniere's disease: clip ligation of the endolymphatic duct.

AIM: Ménière's disease is characterized by recurrent vertigo attacks and hearing loss, and some patients do not respond well to medical treatment. In such cases, surgical procedures to reduce endolymphatic sac pressure have been considered. In recent years, successful results have been reported after endolymphatic duct blockage in patients with resistant attacks despite sac decompression. This study evaluated hearing function and the severity of dizziness after endolymphatic duct blockage. METHODS: Twenty-one patients were included in the study. Preoperative pure-tone audiometry, the Dizzines…

Meniere's disease. Role of otospongiosis and metabolic disorders.

In summary, the available published information on otospongiosis and metabolic factors in the production of endolymphatic hydrops viz. Meniere's disease was reviewed. By addressing these factors in a therapeutic approach, the effective management of this disorder reveals results as good as the reported surgical results. In addition, it was possible to salvage patients who had previously undergone successful surgery only to deteriorate several years later. At this time, the role of surgery on the endolymphatic sac is reserved for those patients who are failures in this form of management.

Constituents of the endolymphatic tubules as demonstrated by three-dimensional morphometry.

The various constituents of the tubules of the human adult intraosseous endolymphatic sac were stained with lectins and Millon reaction to reveal some of the carbohydrate and protein-containing substances in the endolymph. The amount of each substance was determined by digitizing sequential temporal bone sections. Differing amounts of the various identified substances were found in sacs from patients with Meniere's disease, patients with labyrinthine fibrosis, and controls.

Menière's disease and autoimmunity: clinical study and survey.

In order to clarify whether autoimmune mechanisms form part of the etiopathogenesis of Meniere's disease, sera and endolymphatic sac tissues from 30 patients with Meniere's disease were analyzed according to the clinical criteria for autoimmune diseases stated by Mackay & Burnet. Several cases showed hyper gamma-globulinemia, antibody elevation to Type II collagen in the serum and endolymph, positive staining to immunoglobulins and C3 or infiltration of immuno-competent cells in the endolymphatic sac, good response to prednisolone and combination with systemic immune disorders. Other examinati…

Shunt surgery in Meniere's disease: a follow-up report.

Between December 1969 and May 1986, 140 patients were treated for Menière's disease using the endolymphatic sac subarachnoid shunt. The technique and results are contrasted with an earlier report. The surgical technique described includes modifications of the originally described procedure. A microcomputer was used for the storage and manipulation of data based upon 1972 Academy criteria. Control of vertigo was accomplished in 83% of patients; 51% of patients were graded class A or B, according to the 1972 Academy criteria. No serious complications occurred. Problems included the potential for…
