Comminuted fractures of the basilar joint of the thumb: combined treatment by external fixation, limited internal fixation, and bone grafting.

Displaced, comminuted fractures of the thumb carpometacarpal joint were treated by intermetacarpal external fixation, anatomic reduction of the joint surfaces, bone grafting and adjunctive internal fixation in thirteen patients. Retrospectively, at an average follow-up time of 35 months, nine patients showed a good result and one patient had a fair result. All fractures had united without secondary displacement, but focal irregularities of the joint surface were commonly seen. As compared with the uninvolved side, axial rotation averaged 79%, radial abduction 89%, key pinch 88%, and grip stren

External Fixation combined with Limited Internal Fixation versus Open Reduction Internal Fixation for Treating Ruedi-Allgower Type III Pilon Fractures.

BACKGROUND: The optimal treatment of type III pilon fractures remains controversial. Hence, we performed this study to investigate whether open reduction and internal fixation (ORIF) is superior to external fixations combined with limited internal fixations (EFLIF). MATERIAL AND METHODS: From January 2012 to October 2013, a total of 78 patients were included. Twenty-six patients underwent EFLIF and 52 patients underwent ORIF. All subjects were followed up at 1, 3, 6, and 12 months postoperatively. All outcomes and complications were recorded. RESULTS: No statistical differences were observed

A Meta-Analysis for Postoperative Complications in Tibial Plafond Fracture: Open Reduction and Internal Fixation Versus Limited Internal Fixation Combined With External Fixator.

The treatment of tibial plafond fractures is challenging to foot and ankle surgeons. Open reduction and internal fixation and limited internal fixation combined with an external fixator are 2 of the most commonly used methods of tibial plafond fracture repair. However, conclusions regarding the superior choice remain controversial. The present meta-analysis aimed to quantitatively compare the postoperative complications between open reduction and internal fixation and limited internal fixation combined with an external fixator for tibial plafond fractures. Nine studies with 498 fractures in 49

External fixation for displaced intra-articular calcaneal fractures: A case series.

Intra-articular calcaneus fractures (CF) remain challenging. Surgery aims to restore subtalar joint congruence and calcaneus width, height, shape, and alignment, thereby avoiding medial and lateral impingement and enabling patients to resume a normal lifestyle. We retrospectively evaluated 32 patients (35 fractures) with a minimum displacement of 2 mm of the posterior facet and disruption of the calcaneus length, width, and height. Treatment involved a minimally invasive sinus tarsi approach and definitive stabilization with an external fixator (EF). The mean time to partial weight-bearing was

External Fixation for Ballistic Mandibular Trauma: An Old Method for a Modern Problem.

BACKGROUND: Ballistic injuries to the mandible produce comminuted fractures with compromised vascularity and contaminated wound beds that challenge the assumptions of rigid internal fixation. External fixation, the earliest form of mandibular stabilization, preserves periosteal blood supply by avoiding open dissection, yet direct comparisons between the 2 approaches in the ballistic setting are scarce. METHODS: A retrospective review was conducted of consecutive patients with gunshot-related mandibular fractures treated at a single level 1 trauma center from January 2020 to August 2024. Patie

Management of Severe Cavovarus and Equinocavovarus Deformities Using Hexapod External Fixation.

Traditional reconstructive surgery for severe equinocavovarus or cavovarus deformities has disadvantages, especially in patients with complex multiaxial deformities or scar tissue, including neurologic and vascular injury after acute correction, soft tissue complications, deep infection, over or undercorrection, and shortening of the foot. Moreover, newer software-guided hexapod multiplanar systems have allowed precise 3 dimensional corrections. Distraction osteogenesis and joint angular correction are the main methods of correction associated with additional limited soft tissue procedures. Th

Safety Corridors and Anatomic Considerations for Circular External Fixation in the Foot and Ankle.

Circular external fixation of the foot and ankle possesses inherent stability based on circumferential fixation or transfixion in the axial plane. Nonetheless, frame installation can potentially compromise or damage several soft tissue structures. Safety corridors allow the positioning of these fixation elements while diminishing the risk of injury to the neurovascular elements. Surgeons must possess a comprehensive understanding of the anatomic structures at risk and the safety corridors located in the distal leg, ankle, hindfoot, and midfoot regions. This review encompasses a contemporary an

The effect of spanning external fixation on entrapped structures in tibial pilon fractures.

PURPOSE: Pilon fractures are often complex injuries involving severe soft tissue injury. Studies have shown pilon fractures may entrap soft tissue structures between fracture fragments. Staged fixation of pilon fractures with spanning external fixation (SEF) is important for soft tissue rest and plays an important role in the management of these injuries. While SEF has been shown to promote soft tissue rest prior to definitive fixation, no studies have shown the effect SEF has on entrapped structures (ES). The purpose of this study was to evaluate how SEF effects ES in pilon fractures. METHOD

Facilitated trailing haptic externalization technique for intrascleral intraocular lens fixation.

PURPOSE: To describe the facilitated trailing haptic externalization technique for intrascleral intraocular lens (IOL) fixation. METHODS: In this technique, first the direction of the 4-5 mm tip of the trailing haptic is straightened with the help of forceps. After the second needle enters the posterior chamber, the edge of the IOL optic is pushed towards the opposite scleral tunnel with the needle. The trailing haptic approaching the center from the periphery and the straightened distal part of the haptic towards the direction of the needle facilitate the relatively easily and gently placing

Intra-articular Calcaneal Fracture Treatment With Staged Medial External Fixation.

BACKGROUND: To compare the clinical and radiographic outcomes between the conventional delayed and staged approaches for intra-articular calcaneus fractures in which early definite fixation could not be performed because of severe soft tissue injury. METHODS: From January 2015 to May 2019, a total of 32 cases with acute intra-articular calcaneal fractures met criteria and were enrolled in the study. We compared the outcomes of intra-articular calcaneal fractures that underwent delayed internal fixation between groups treated with a conventional delayed approach (non-EF group) vs a temporary m

The Use of Hexapod External Fixation in the Management of Charcot Foot and Ankle Deformities.

Charcot neuroarthropathy (CN) and its sequela is a disabling pathology in the foot and ankle. The 2-stage computer hexapod-assisted technique is an effective tool to address midfoot Charcot and ankle-hindfoot deformities to restore function and decrease the risk of amputation secondary to ulceration and infection. Although this is not the only technique available, it is an excellent option in cases with significant angular deformity or subluxation, need to reduce shortening of the foot, and in the presence of soft tissue defects, with or without concurrent soft tissue or bone infection.

External fixation of airway stents for upper tracheal stenosis and tracheoesophageal fistula.

OBJECTIVES: Stent migration is a common complication of airway stent placement for upper tracheal stenosis and tracheoesophageal fistula. Although several researchers have reported that external fixation is effective in preventing stent migration, the usefulness and safety of external fixation have not been proved because their cohorts were small. We therefore investigated the efficacy and safety of external fixation during upper tracheal stenting. METHODS: Records of patients who underwent airway stent placement from May 2007 to August 2018 in a single centre were retrospectively reviewed. W

SUCCESSFUL REPAIR OF ANTEBRACHIAL FRACTURES WITH EXTERNAL SKELETAL FIXATION IN THREE MACROPODS.

Three captive macropods comprising three different species sustained unilateral antebrachial fractures. All fractures were assumed to be trauma related, although the specific circumstances surrounding each case was unknown. Each fracture was surgically repaired with type Ia (unilateral, monoplanar) external skeletal fixators, which were all removed approximately 3 mo postoperatively. Although each animal experienced at least one complication, all animals showed adequate bridging and remodeling at the fracture sites and had good-to-excellent return to normal function after fixator removal.
