Ocular infections associated with atypical mycobacteria: A review.

Atypical mycobacteria or non-tuberculous mycobacteria (NTM) are a group of acid-fast bacteria that are pathogenic to different parts of the eye. The organisms can cause a spectrum of ocular infections including keratitis, scleritis, uveitis, endophthalmitis and orbital cellulitis. Trauma, whether surgical or nonsurgical, has the highest correlation with development of this infection. Common surgeries after which these infections have been reported include laser in situ keratomileusis (LASIK) and scleral buckle surgery. The organism is noted to form biofilms with sequestration of the microbe at

[Ocular bacteriosis in a patient with aplastic anemia].

BACKGROUND: For more than a century it has been recognized that bacteria may infect the eye by way of the blood stream. Until fifty years ago, eyes so afflicted nearly always were blinded and a majority of the victims died from overwhelming sepsis. Although effective antibiotics are available today the prognosis is still guarded. CASE REPORT: A 39-year-old physician died 6 years after he was diagnosed for idiopathic aplastic anemia. He had developed pseudomonas aeruginosa bacteremia and later a therapy refractive pneumonia caused by aspergillus which had led to respiratory failure. The patien

Ocular Surface Infection Mediated Molecular Stress Responses: A Review.

Infection mediated ocular surface stress responses are activated as early defense mechanisms in response to host cell damage. Integrated stress responses initiate the host response to different types of infections and modulate the transcription of key genes and translation of proteins. The crosstalk between host and pathogen results in profound alterations in cellular and molecular homeostasis triggering specific stress responses in the infected tissues. The amplitude and variations of such responses are partly responsible for the disease severity and clinical sequelae. Understanding the etiol

Transmission of the eyeworm Thelazia callipaeda: between fantasy and reality.

BACKGROUND: Thelazia callipaeda is transmitted by Phortica variegata, a drosophilid that feeds on lachrymal secretions of mammals. Scientific information on human thelaziosis is still relatively limited, mainly for physicians and ophthalmologists. Indeed, the literature is full of misleading information on the transmission of T. callipaeda to humans. FINDINGS: A recent paper reported a case of human intraocular infestation in a patient from Karnataka. The information presented in that article as well as in other articles in the international literature is outdated and incorrect in several ins

Pediatric infectious endophthalmitis: a review.

Infectious endophthalmitis is a rare but severe complication of septecemia, intraocular surgeries, or penetrating eye trauma. The etiology, prognosis, and management of pediatric endophthalmitis resulting from exogenous and endogenous infections are reviewed. Open-globe trauma and glaucoma surgery are the most frequent causes of endophthalmitis in children, whereas endogenous infection is the least common cause. Streptococcus and Staphylococcus species are common bacterial agents in both posttraumatic and postoperative pediatric endophthalmitis, whereas Candida albicans is a commonly reported

Preseptal and orbital cellulitis.

The patient with a tender, erythematous, swollen eyelid represents a complex clinical challenge to the physician, who must arrive at a correct diagnosis from numerous differential possibilities. Knowledge of the anatomy of the orbit and surrounding structures and proper clinical and radiologic examination are necessary to accurately diagnose these conditions. Proper selection of antibiotic therapy and timely surgical intervention, directed at both the orbital infection and the underlying condition, are essential to avoid serious morbidity from these infections. This article reviews in detail p

[Incidence and pathogenesis of ocular symptoms in HIV infection].

202 patients with HIV infections of different stages [1 (n = 59): symptomless HIV seropositivity, 2 (n = 64): pre-AIDS (LAS = Lymphadenopathy syndrome, ARC = AIDS related complex), 3 (n = 79): AIDS] were prospectively examined. The findings of the first examination were analyzed statistically in a cross-section-study. In 15 AIDS autopsy cases the eyes were examined histopathologically and with immunohistochemical techniques. Patients with stage 1 had only some Sicca syndromes (3%) and neuroophthalmological signs (dyscoria with neurosyphilis) (2%). 14% of the patients with stage 2 had a microva

A rare case of Panophthalmitis due to cobra bite.

PURPOSE: Ocular manifestations of snake bite are rare, ranging from direct injury to the eye from snake venom or indirect injury due to antivenom. We report a rare case of cobra bite causing panophthalmitis due to indirect injury as a result of snake venom toxin related tissue necrosis and susceptibility to secondary infections. METHODS: Observational case report. External photographs, slit lamp photos, ultrasonography of the eye and histopathology of the eviscerated eye were used to characterize and describe the clinical findings. Thirty-nine-years-old male farmer presented with history of c

Ocular-systemic interrelationships in acquired immunodeficiency syndrome.

We analysed the correlation between ophthalmic and systemic findings in 125 subjects with AIDS and 50 subjects with AIDS-related complex (ARC). Positive eye findings were defined as the presence of cotton-wool spots (CWS) or cytomegalovirus (CMV) retinitis. The presence of positive eye findings was significantly more frequent in AIDS than in ARC (P = 0.0001). Both lowest haematocrit and lowest T-helper cell count were significantly lower in AIDS than in ARC, and also lower in subjects with positive eye findings than in those with negative eye findings. No association was found between ocular f

Pseudomonas aeruginosa endophthalmitis in a preterm infant.

Bacterial endophthalmitis is rare in the neonatal period. It occurs in susceptible individuals such as preterm infants. Pseudomonas aeruginosa, a Gram-negative bacillus, has been identified as the causative organism in more than 75% of invasive neonatal eye infections. The source may be endogenous; secondary to septicemia or exogenous, including reports of nosocomial infections and those infections arising from the birth canal. We report the case of a preterm infant who developed pseudomonas endophthalmitis, septicemia, and meningitis after a corneal abrasion.

Disarming Pore-Forming Toxins with Biomimetic Nanosponges in Intraocular Infections.

Intraocular infections are prevalent after traumatic injuries or after common ocular surgeries. Infections cause inflammation that can damage the retina and architecture of the eye, often resulting in poor visual outcomes. Severe cases may result in blindness or require enucleation of the eye. Treatments for intraocular infections include intravitreal antibiotics and corticosteroids or surgical vitrectomy in serious cases. The increase in multidrug-resistant infections calls for novel treatment options. In the present study, a biomimetic erythrocyte-derived nanosponge was tested for the abilit

Detection and prevention of ocular phototoxicity of ciprofloxacin and other fluoroquinolone antibiotics.

Fluoroquinolone (FLQ) drugs are a potent family of antibiotics used to treat infections including ocular infections. To determine if these antibiotics may be phototoxic to the eye, we exposed human lens epithelial cells to 0.125-1 mm FLQs (ciprofloxacin [Cipro], lomefloxacin [Lome], norfloxacin [Nor] and ofloxacin [Ofl]), the precursor quinolone nalidixic acid (Nalid) and UVA radiation (2.5 J cm(-2)). Based on fluorescence confocal microscopy, FLQs are diffused throughout the cytoplasm and preferentially located in the lysosomes of lens epithelial cells. Neither FLQ exposure alone nor UVA expo

Rhodotorula infection. A systematic review of 128 cases from literature.

Rhodotorula is an emerging opportunistic pathogen, particularly in immunocompromised patients. Many cases of fungemia associated with catheters, endocarditis, peritonitis, meningitis, and endophthalmitis are infections incited by this yeast. The main purpose of this study was to review all cases of Rhodotorula infection reported in the literature and to describe risk factors, underlying conditions and outcome. From 128 cases, 79% were fungemia (103 cases), 7% eye infections (nine cases) and 5% (six cases) peritonitis associated with continuous ambulatory peritoneal dialysis. Eighty seven perce…

[Endophthalmitis with keratitis-how to manage severe cases? : A narrative review].

Endophthalmitis secondary to microbial keratitis (MK) is a rare but devastating ophthalmic emergency characterized by diagnostic and therapeutic challenges due to poor corneal clarity. Although it accounts for less than 10% of cases of exogenous endophthalmitis, it often results in poor visual outcomes or enucleation. The most common causative organisms include Staphylococcus spp., Pseudomonas spp., and Fusarium spp., with rising concerns about antimicrobial resistance. Several risk factors have been identified, including the use of topical corticosteroids, systemic immunosuppression, and fung

Observation of Retrocorneal Plaques in Patients With Infectious Keratitis Using Anterior Segment Optical Coherence Tomography.

PURPOSE: Endothelial plaques are a typical characteristic in patients with fungal keratitis. However, bacterial keratitis and herpetic keratouveitis are rarely associated with fibrin formation on the retrocorneal surface. This study was conducted to examine plaques attached to the endothelium in patients with infectious keratitis using anterior segment optical coherence tomography (AS-OCT). METHODS: Seventeen patients (10 women and 7 men; mean age, 75 ± 15.5 years) suspected to have infectious keratitis with retrocorneal plaques were included. AS-OCT was used to acquire a scan of the retrocor
