Caseating cutaneous granulomas in a child with common variable immunodeficiency.

Cutaneous, noninfectious, granulomatous lesions have been reported occasionally in different types of immunodeficiencies, including common variable immunodeficiency (CVD). We present a child with CVID and cutaneous granulomas with a strikingly prominent caseating necrosis. We think that such granulomatous lesions constitute a distinctive manifestation of immunodeficiency, and may reflect a altered immune response. Corticosteroids have been of benefit to our patient, as well as in similar cases.

Low-dose weekly methotrexate for unusual neutrophilic vascular reactions: cutaneous polyarteritis nodosa and Behçet's disease.

Low-dose weekly methotrexate therapy has been used to treat patients with psoriasis for more than 20 years. This regimen has also been used to treat rheumatoid arthritis, inflammatory bowel disease, primary sclerosing cholangitis, and corticosteroid-dependent asthma. We report two patients with Behçet's disease with cutaneous neutrophilic vascular reactions and three with cutaneous polyarteritis nodosa who responded dramatically to low-dose weekly methotrexate therapy.

Patch testing with the "sesquiterpene lactone mix": a marker for contact allergy to Compositae and other sesquiterpene-lactone-containing plants. A multicentre study of the EECDRG.

6278 patients were patch tested with a sesquiterpene lactone mix (SL-mix) in 10 European clinics. 4011 patients were tested only with 0.1% SL-mix, 63 (approximately 1.5%) of whom were positive, with 26 (41%) of these cases being considered clinically relevant. There were no cases of active sensitization, though 5 cases of irritation were reported. 22 irritant reactions and 22 cases of active sensitization occurred when testing also with 1% and 0.33% concentrations of SL-mix. SL-mix 0.1% pet. is shown to be an important patch test and many relevant sensitizations will be missed without routine

Porokeratosis of Mibelli. A report of two unusual cases.

2 cases are reported with unusual manifestations of porokeratosis of Mibelli. In 1 case multiple atrophic facial lesions resembled plaques of chronic discoid lupus erythematosus. The second case exhibited extensive cutaneous involvement in the form of hyperkeratotic lesions in linear configuration.

Lichenoid pseudovesicular papular eruption of the nose (LIPEN): clinicodermoscopic and histopathological correlation.

Lichenoid pseudovesicular papular eruption of the nose (LIPEN) is an uncommon and often overlooked facial dermatosis characterised by monomorphic papules with a pseudovesicular appearance, predominantly affecting the nasal dorsum and adjacent centrofacial areas. Owing to its clinical resemblance to several inflammatory and granulomatous dermatoses, it is frequently misdiagnosed, leading to delayed recognition and inappropriate management for a long term. We report a patient presenting with chronic, asymptomatic centrofacial papules unresponsive to conventional therapies. Dermoscopic examinatio

Pellagra associated with esophageal carcinoma and alcoholism.

Pellagra is a nutritional disease caused by the deficiency of niacin. It presents with a photodistributed rash, gastrointestinal symptoms, and neuropsychiatric disturbances. In the Western world, this disease is mostly confined to alcoholics or the impoverished. However, this condition must be recognized in other clinical settings because it is easily treated and can be fatal if not identified. We describe a case of pellagra caused by esophageal carcinoma and alcoholism; we also review the literature.

Lupus miliaris disseminatus faciei with madarosis: A novel presentation.

RATIONALE: Lupus miliaris disseminatus faciei (LMDF) is a chronic, self-limiting inflammatory skin disease of unknown etiology that can lead to disfiguring scars. PATIENT CONCERNS: A 51-year-old male presented with a 1-month history of facial papules and eyebrow exfoliation. DIAGNOSES: Skin biopsy confirmed that this case was LMDF. INTERVENTIONS: After the diagnosis was confirmed, the patient was treated with oral doxycycline (100 mg) and prednisone (12 mg) once daily. OUTCOMES: Follow-up at 1 month showed significant improvement in facial rash and regrowth of both eyebrows. LESSONS: This

[Vitiligo and Koebner phenomenon following platelet-rich plasma injections].

Vitiligo is a common and acquired skin pigmentation disorder characterized by a selective and often continuous loss of epidermal melanocytes. We report the case of a female patient who developed facial vitiligo following platelet-rich plasma injections. The diagnosis of Koebner phenomenon was retained in order to explain the occurrence of vitiligo on the areas where platelet-rich plasma (PRP) had been injected.

Rosacea causing unilateral Morbihan syndrome.

Morbihan syndrome is a rare entity causing woody induration of the face. There are numerous case reports of bilateral Morbihan syndrome. We present a case of a 46-year-old man with right infra-orbital cheek swelling and symptoms of rosacea who had histology consistent with granulomatous rosacea following debulking surgery. His clinical presentation and investigation findings support a diagnosis of rosacea causing unilateral Morbihan syndrome.
