[Necrotizing fasciitis caused by group A streptococcus].

The first case of the confirmed necrotizing fasciitis caused by Group A Streptococcus in Yugoslavia was presented. Male patient, aged 28, in good health, suddenly developed symptoms and signs of severe infective syndrome and intensive pain in the axillary region. Parenteral antibiotic, substitutional and supportive therapy was conducted along with the radical surgical excision of the necrotizing tissue. The patient did not develop streptococcal toxic shock syndrome thanks to the early established diagnosis and timely applied aggressive treatment. He was released from the hospital as completely

[Hyperbaric oxygen for the treatment of necrotizing fasciitis].

Necrotizing fasciitis (NF) is a severe often life threatening bacterial infection. There are 2 main reasons to use hyperbaric oxygen (HBO2): the polymorphism of the bacterial flora with a predominance of anaerobes, either strict or aerotolerant; and the tissular necrosis due to an extensive disseminated microvascular obstruction within the infected area. Association of HBO2 to antibiotics and surgery is based on strong pathophysiological findings as well as on evidences from animal studies. Clinical evidence in human is still lacking even if published data supports its use in severe cases. Con

[Necrotizing fasciitis. A diagnostic and therapeutic challenge].

A case of necrotizing fasciitis of the upper limb caused by group A streptococcal infection and a case with an abscess of the hand caused by staphylococcus aureus, are presented. The anatomo-pathological and clinical differences between the two different types of infection are underlined and stress is laid on the excruciating pain, the high temperature and the rapidly spreading inflammatory-necrotizing evolution of the first case. More than the anatomo-pathological findings of a necrotic area the authors consider more important, the rapidity and the severity of the infection, that can evolve i

A fatal case of craniofacial necrotizing fasciitis.

A case of fatal craniofacial necrotizing fasciitis is described in a 72-year-old diabetic woman and management is discussed. Progressive infection of the eyelids occurred with involvement of the right side of the face. Computed tomography revealed soft tissue swelling. Antibiotic treatment was started and debridement performed; histopathology showed acute inflammation and thrombosis of the epidermis and dermis. Despite treatment, scepticemia occurred, resulting in death less than 48 h after presentation. At this time extensive necrosis had developed in the superficial fascia with undermining a

Prompt recognition of necrotizing fasciitis in a newborn.

We report a 13-day-old female neonate with necrotizing fasciitis, who presented with fever and erythema on her back. Rapidly progressive extension of the erythema to the abdominal wall occurred within 12 hours. Sonography for the lesion showed extensive fascial thickening with fluid accumulation. An abscess was also demonstrated. Necrotizing fasciitis was highly suspected. Under sonographic guidance, pus was aspirated from the abscess. A smear for the aspirated pus revealed a large number of neutrophils and abundant gram-positive cocci. Immediate surgical debridement was done. Cultures from th

Necrotizing fasciitis and gangrene of the upper extremity.

Necrotizing fasciitis is a severe, fulminant infection most commonly encountered in patients with diabetes mellitus, alcohol abuse, and intravenous drug abuse. The infection can spread-unrecognized along fascial planes beneath seemingly normal skin. The relatively benign appearance of the extremity is misleading and often results in delay in diagnosis and increased morbidity or death. Immediate aggressive surgical debridement through extensile incisions in combination with antibiotic therapy is necessary for control of these limb- and life-threatening, soft-tissue infections. Gas gangrene, or

Extensive necrotizing fasciitis in a diabetic patient.

Necrotizing fasciitis is a rare but life-threatening infection of the subcutaneous tissue. In addition to Group A streptococci, polymicrobial infection with various aerobic and anaerobic gram positive or gram negative bacteria is frequently observed. The present case illustrates that a high level of clinical suspicion is necessary for early diagnosis of this disease and confirms the value of computed tomography in patients with sepsis of unknown origin. Because of rapid progression of the disease early debridement of necrotic tissue is mandatory for survival.

[Hyperbaric oxygenation in the treatment of necrotizing fasciitis].

The authors present the results of treatment provided to 11 patients with necrotizing fasciitis who were after a surgical operation and after administration of antibiotics treated in a hyperbaric chamber. A total of 8 patients (82%) recovered completely. The authors discuss the theory of action of hyperbaric oxygen on microorganisms and draw attention to the possible reduction of mortality of this serious disease when using hyperbaric oxygenation. The latter is considered an important auxiliary method which supplements surgical treatment, rational antibiotic therapy and in particular careful i

Necrotizing fasciitis of the vulva during chemotherapy.

A case of necrotizing fasciitis of the vulva arising in a leukemic patient during a chemotherapy nadir is presented. The nature of the process was not recognized initially, and the patient was treated with intravenous antibiotics. After clinically evident necrosis developed, prompt surgical debridement was carried out. Clinical improvement followed and the wound healed well. This case suggests that chemotherapy with its attendant immunosuppression is a predisposing factor in the development of necrotizing fasciitis and warrants a high index of suspicion.

Necrotizing fasciitis. A report of 3 cases.

Necrotizing fasciitis is a serious infectious disease characterized by a necrotizing cellulitis of the subcutaneous tissue. The mortality has not changed over the last 50 years, probably because the condition is not recognized early enough to permit effective treatment by debridement. We report 3 cases illustrating different clinical courses.

Necrotizing fasciitis. A report of five patients.

Five patients with necrotizing fasciitis are reported. Streptococci were isolated in all but one patient, in whom Staphylococcus albus was responsible. Other organisms were also isolated in culture. Surgical debridement and skin grafting were needed in all our patients, in addition to systemic antibiotics and topical treatment, comprising Eusol paraffin soaks. One patient developed this condition after a skin biopsy; another had chronic venous leg ulcer. There was no predisposing cause in the remaining three patients.

Medicolegal aspects of necrotizing fasciitis of the neck.

Necrotizing fasciitis of the neck (NFN) is a relatively rare, fulminating infectious process of the cervicofacial tissues which may cause sudden and unexpected death. Although often the result of a dental infection, injuries of the soft tissues of the neck may also initiate rampant cellulitis, and recognition of the underlying etiology of such cases is necessary to determine properly the manner of death. Five cases of NFN are presented with a review of the causative factors and usual bacteriology, and specific factors of medicolegal interest are addressed.
