[Place of celio-video-surgery in splenectomy for hematologic diseases].

In order to evaluate the place of the laparoscopic approach in splenectomy for haematological disease, the authors prospectively studied a series of 25 consecutive patients requiring splenectomy. There were 11 cases of thrombocytopenic purpura, 9 lymphomas, 2 cases of herediary spherocytosis, 1 Felty syndrome, 1 idiopathic myelofibrosis and 1 Hodgkin disease. Twelve patients (48%) underwent an immediate conventional procedure for huge splenomegaly (10), obesity (1), unavailability of video-equipment. Thirteen patients (52%) underwent a laparoscopic approach. Five of these operations were conve…

Musculoskeletal syndromes associated with malignancies.

Literature on the association of malignancies with various rheumatic disorders published over the past year is summarized in this review. The possible roles of methotrexate treatment in predisposing to the development of lung cancer and Felty's syndrome in predisposing to non-Hodgkin's lymphoma in rheumatoid arthritis patients are discussed. The increased occurrence of monoclonal gammopathies and non-Hodgkin's lymphoma in patients with Sjögren's syndrome is reported. The possible increased frequency of malignancies among patients with systemic lupus erythematosus (SLE), scleroderma, and polymo

[Case report: rheumatoid arthritis and large granular lymphocytes syndrome].

Felty's syndrome (FS) is a rare complication (less than 1%) of rheumatoid arthritis (RA), with the clinical feature of splenomegaly and neutropenia. Approximately 10-40% of FS patients have an expansion of peripheral blood large granular lymphocytes (LGL). This cell population mainly consists of two subsets: cytotoxic T cells (CD8+, CD57+) and natural killer cells (CD3-,CD8-,CD56+). It has been hypothesised that LGL expansion could be responsible for neutropenia by suppressing neutrophil precursors in the bone marrow, but various mechanisms have been proposed to explain this association. Nodular regenerative hyperplasia of the liver.

Nodular regenerative hyperplasia of the liver, an uncommonly reported and poorly defined clinicopathological entity, obscured clinical diagnosis and was misdiagnosed on hepatic biopsy in a recent case. Approximately 19 cases are recorded in the English literature. Six patients had Felty's syndrome, about 12 patients had congestive heart failure, and the patient under discussion had subacute bacterial endocarditis. Light- and electron-microscopic examination was utilized to define nodular regenerative hyperplasia pathologically. Features common to all reported cases are discussed but elucidatio

ANCA in systemic vasculitides, collagen vascular diseases, rheumatic disorders and inflammatory bowel diseases.

It was the aim of this study to reevaluate the diagnostic significance and clinical implication of ANCA after testing sera from 13,606 patients for the presence of ANCA. Our data confirm the high specificity (97%) and sensitivity (80%) of cANCA for Wegener's granulomatosis. pANCA were found in renal vasculitides (60%), collagen vascular diseases (SLE 20%, Sjögren's syndrome 26%, polymyositis 16%) and rheumatic disorders (Felty's syndrome 50%, rheumatoid arthritis 20%). A third fluorescence pattern in sera of patients with inflammatory bowel disease (ulcerative colitis 28/72, Crohn's disease 6/

Nodular transformation (nodular "regenerative" hyperplasia) of the liver. A clinicopathologic study of 30 cases.

Nodular transformation, a rare hyperplastic condition of the liver, has been reported in patients with rheumatoid arthritis, Felty's syndrome, the CRST syndrome, and myeloproliferative disorders. Associated disorders in the present clinicopathologic study of 30 cases included the foregoing as well as extrahepatic neoplasms, endocrine disorders, and other diseases producing immune dysfunction; some patients had received drug therapy for a prolonged time. Clinically nodular transformation may be confused with cirrhosis; histologic evaluation of liver biopsy material is essential for diagnosis. C

Plasma and serum von Willebrand factor antigen concentrations in connective tissue disorders.

Concentrations of serum and plasma von Willebrand factor antigen were measured in over 200 patients with a variety of connective tissue diseases, and in control samples from over 200 asymptomatic individuals. This comprehensive study found the highest concentrations of von Willebrand factor antigen in patients with vasculitis, Sjögren's syndrome, Felty's syndrome, giant cell arteritis and polyarteritis nodosum. Raised values were also found in rheumatoid arthritis, systemic lupus erythematosus, polymyalgia rheumatica, systemic sclerosis, Raynaud's syndrome, Takayasu's arteritis and Wegener's g

Factor VIII related antigen in the assessment of vasculitis.

Factor VIII related antigen, an endothelial cell product, was markedly raised in systemic necrotising arteritis, reflecting disease activity, but was not raised in active cutaneous vasculitis. In rheumatoid arthritis high levels were only found in systemic vasculitis or Felty's syndrome, but in other connective tissue diseases increased levels were more frequently detected and usually related to disease activity. It did not correlate with C reactive protein. It was also raised in non-inflammatory peripheral vascular disease and after surgery and was not specific for vasculitic endothelial dama

Nodular regenerative hyperplasia of the liver: report of two cases and review of the literature.

We review the clinical and pathologic features of 39 cases of nodular regenerative hyperplasia of the liver and include two new cases. This is an uncommon but distinct pathological entity characterized by regenerative hepatocytic nodules without fibrosis. Portal hypertension that often leads to hemorrhage from esophageal varices is the most important clinical manifestation. Characteristically, manifestations of hepatic failure are lacking, but a number of patients have elevated alkaline phosphatase of hepatic origin. Felty's syndrome is the entity most often associated with nodular regenerativ

In vivo and in vitro effects of lithium on granulopoiesis in human neutropenic disorders.

Studies have been cazrried out to determine the in vivo and in vitro effects of lithium carbonate in various neutropenic disorders. Addition of lithium to culture of bone marrow from patients with a variety of neutropenic disorders and ANLL in which normal or elevated serum and/or urinary CSA levels were present did not enhance granulocyte colony formation. Administration of lithium carbonate to patients with Felty's syndrome, in which serum and urinary CSA levels are reduced, enhanced peripheral blood neutrophil levels and serum and urinary CSA values. Lithium administration appears to be mos

Leukocyte colony stimulating factors for rheumatologists.

The leukocyte colony stimulating factors (recombinant granulocyte colony stimulating factor and granulocyte macrophage colony stimulating factor) have become widely used in hematology and oncology to raise neutrophil levels in patients with neutropenia. They have been used primarily by rheumatologists to treat neutropenia accompanying infections in patients with Felty's syndrome or systemic lupus erythematosus and in infected patients made neutropenic with drug therapy. These factors have been lifesaving. The drugs generally are well tolerated and adverse effects usually are easily treated. So

The impact of HLA-DRB1 genes on extra-articular disease manifestations in rheumatoid arthritis.

The objective of this study was to examine HLA-DRB1 and HLA-DQB1 genotypes in patients with severe extra-articular rheumatoid arthritis (ExRA) and to compare them with the genotypes of rheumatoid arthritis (RA) patients without extra-articular manifestations. Patients with severe ExRA were recruited from a large research database of patients with RA, from two cohorts of prevalent RA cases, and from a regional multicenter early RA cohort. Cases with ExRA manifestations (n = 159) were classified according to predefined criteria. Controls (n = 178) with RA but no ExRA were selected from the same

Ulcerating rheumatoid nodule of the vulva.

A case of an ulcerating rheumatoid nodule of the vulva in a 76 year old woman with rheumatoid arthritis complicated by Felty's syndrome is reported. The patient presented with a mass in the vulval region. On clinical examination, she had an ulcerated mass associated with inguinal lymphadenopathy. These findings resulted in a clinical diagnosis of invasive carcinoma of the vulva and an excision biopsy was carried out. On microscopic examination, the lesion showed the characteristic features of a rheumatoid nodule with ulceration of overlying epidermis. Adjacent vessels showed inflammation and f

Infection in rheumatoid arthritis.

Sepsis is an unusually common cause of illness and death in RA. All sorts of infections occur, but pyarthrosis produces exceptional problems. Clinically, pyarthrosis, empyema, and purulent pericarditis mimic bland rheumatoid effusions. Aspiration of the attendant effusions is the only reliable diagnostic procedure. Subcutaneous nodules on the sacrum and back are easily overlooked. Necrosis and ulceration of these nodules may provoke septicemia. Those with Felty's syndrome do not uniformly have problems with recurrent infection. Splenectomy may not benefit such patients. The belief that cortico

A Rare Case of T-Large Granular Lymphocytic (T-LGL) Leukemia in a Patient With Rheumatoid Arthritis With Neutropenia and Low LGL Level.

T-large granular lymphocytic (T-LGL) leukemia is a rare hematological malignancy characterized by clonal expansion of cytotoxic T-cells resulting in cytopenias. The diagnostic criteria for T-LGL leukemia necessitated a sustained peripheral blood elevation of LGLs exceeding 2 × 109/L for a minimum duration of 6 months, in the absence of an identifiable etiology. In most of the cases, it is associated with autoimmune disorders such as rheumatoid arthritis. As cytopenias, including neutropenia, can be an early manifestation of the disease, they may get confused with Felty's syndrome, resulting in

The emerging role of Rituximab in the treatment of large granular lymphocytic leukemia associated with rheumatoid arthritis: a single center experience.

Large Granular Lymphocytic (LGL) leukemia is a rare lymphoproliferative disorder with a peculiar association with Rheumatoid Arthritis (RA). The most common feature is neutropenia and patients can have splenomegaly, resembling Felty's Syndrome. These diseases have similar clinical and laboratory abnormalities, but the diagnosis of T-cell LGL (T-LGL) leukemia requires evidence of clonality. Even though T-LGL leukemia is indolent in most cases, inadequate treatment when it is indicated can lead to significant morbidity and mortality, mainly associated with recurrent infections. We present two cl

Clinical liver disease in patients with rheumatoid arthritis taking methotrexate.

Between 1981 and 1989, 3 of 134 patients with rheumatoid arthritis (RA) treated with methotrexate (MTX) developed clinically significant hepatic dysfunction and showed histologic evidence of severe liver disease (fibrosis and cirrhosis). Factors identified in these patients that may have been linked to liver toxicity included diabetes, congestive heart failure and Felty's syndrome. In the patient group that received a post-MTX liver biopsy, pulmonary fibrosis and obesity were significantly associated with hepatic fibrosis/cirrhosis. Severe liver disease may occur in patients with RA treated wi

The coexistence of rheumatoid arthritis with Reiter's syndrome and/or ankylosing spondylitis: a model of dual HLA-associated disease susceptibility and expression.

In a prospective study of 184 patients with ankylosing spondylitis and/or Reiter's disease, we have examined the frequency of concomitant rheumatoid arthritis (RA) and determined the HLA profile of these patients. Five men (2.7%), 3 with ankylosing spondylitis and 2 with Reiter's disease, had coexistent classical RA and 2 of these also had Felty's syndrome. Immunogenetic analysis demonstrated that all 5 patients had either HLA-R27 or another B-7-cross-reactive antigen and that 3 also possessed HLA-DR4. Thus, 2 or more HLA-associated genetic predispositions may occur in the same individual, and

A common anti-cardiolipin antibody idiotype in autoimmune disease: identification using a mouse monoclonal antibody directed against a naturally-occurring anti-phospholipid antibody.

We have recently produced a series of human monoclonal antibodies reacting with cardiolipin. One of these, H3, a polyspecific IgM/k derived from a normal individual, was used to raise mouse monoclonal antibody to its idiotype. Two anti-idiotypic antibodies, S2.9 (IgG2b) and S2.10 (IgM) were selected for their specific reaction with H3.S2.9 did not react with five other human monoclonal antibodies of IgM/k class despite the fact that these shared some antigen-binding characteristics with H3.S2.9 was able to block the binding of H3 to all of its cross-reactive antigens including cardiolipin, whi
