Collapsed subchondral fatigue fracture of the femoral head.

Several etiologies can lead to a subchondral fracture of the femoral head, which may result in femoral head collapse and degenerative change. This article evaluates the follow-up results of subchondral fatigue fractures of the femoral head in which femoral head collapse occurred. The study shows that collapsed subchondral fatigue fractures of the femoral head have a benign clinical course quite unlike that of collapsed osteonecrosis of the femoral head.

[Bilateral femoral neck fracture after seizure].

Femoral neck fractures after seizure are rare. This injury can easily be underdiagnosed due to generalised, musculoskeletal pain after seizure. In case of persisting groin pain and limited range of motion X-rays are indicated. Within the first 6 hours after the trauma a joint-preserving therapy is possible. After a delayed diagnosis total hip arthroplasty is necessary. As the result of prolonged intra-articular haematoma the incidence of a femoral head necrosis increases. When choosing the implant, an elevated risk of joint dislocation should be considered.

Proximal femoral fracture in patients with rheumatoid arthritis.

Hip fracture occurrence was examined cross-sectionally in Japanese patients with rheumatoid arthritis (RA). Between January 2005 and June 2006 we studied RA outpatients with a past history of hip fractures. Patients included 1 man and 25 women. As 3 women had bilateral hip fractures, the total number was 29. Age at the time of fracture was 72.1 +/- 4.5 years. Of the 29 fractures, 22 were cervical and 7 were trochanteric. Four fractures were spontaneous while the others occurred in falls. 24 fractures were associated with oral steroid administration. All 5 fractures unassociated with prednisolo

Fractures of the femoral neck: a review and personal statement.

EPIDEMIOLOGY: The number of hip fractures will increase enormously in the decades to come as will the cost of treatment of these patients do. In the USA the annual cost has estimated to be nearly $10 billion. Hip fractures, therefore, represent an enormous socio-economic and medical problem and challenge (orthopaedic) surgeons an anaesthetists to find the cheapest and most effective way to treat them. At the same time the search for preventive measures should be continued. Biphosphonates and hip protectors seem to be able to decrease the risk of suffering a hip fracture with 50%. CLASSIFICATI

Surgical dislocation of the hip for fractures of the femoral head.

Traumatic dislocations of the hip are high-energy injuries that frequently occur with fractures of the femoral head. Controversy exists regarding many aspects of the treatment of these fractures, but following reduction, large displaced head fragments require open reduction and internal fixation. Traditionally, an anterior approach to the hip joint has been used for surgical access to the femoral head, but this incision often results in limited visualization and subsequent difficulty with anatomic reduction. Recently, a surgical hip dislocation technique has been described for acetabular fract

Femoral neck stress fracture in a sanitary worker.

Stress fractures of the femoral neck are uncommon, it is important to have a high index of suspicion in young active people who perform repetitive vigorous activity or have recently had an increase in frequency of such activity. We report a case of femoral neck stress fracture in a sanitary worker which was treated successfully with a dynamic hip screw.

Injury of the femoral nerve associated with acetabular fracture.

BACKGROUND: The purpose of the present study was to document the prevalence of, and recovery from, injuries of the femoral nerve associated with displaced acetabular fractures. METHODS: From 1986 to 2001, 726 acetabular fractures were treated with open reduction and internal fixation. Four patients who had an injury of the femoral nerve associated with a displaced acetabular fracture were identified and were followed for a mean of 3.4 years. The nerve injury was iatrogenic in two patients and traumatic in two patients. RESULTS: Clinically detectable quadriceps femoris motor function returned

[Medial femoral neck fracture. Controversies in treatment].

The treatment of the medial femoral neck fracture remains controversial until today. The goal of this paper is therefore, based on the literature, to show guidelines for optimal treatment: conservative treatment vs. operation, osteosynthesis vs. prosthesis, timing for osteosynthesis, open vs. closed reduction, choice of implant for osteosynthesis, postoperative treatment (weight bearing vs. non weight bearing), Pauwels-Osteotomy vs. prosthesis in cases op pseudarthrosis, femoral head prosthesis vs. total hip arthroplasty, bipolar vs. monopolar femoral head prosthesis, choice of classification.

[Walking recovery in stroke patients with femoral fracture].

UNLABELLED: The aims of this study were: 1) to evaluated the degree of walking of hemiplegic patients after femur fracture, 2) to evaluate if the neurological damage interferes with the recovery of walking. We studied 51 hemiplegic patients with femur fracture. All patients could walk without any help before the fracture. At the end of the rehabilitation period we had the following results: 5.8% of patients regained full walking independence; 37.2% walked with a cane; 43.1% with a quadripod; 9.8% walked with assistance; 3.9% remained unable to walk. 50.9% of them regained their prefracture fun

[Management of ipsilateral femoral shaft and neck fracture].

From Oct. 1987 to Oct. 1993, 704 patients with femoral shaft fracture were managed at our hospital. Among these, 12 patients were with combined femoral shaft and neck fracture (2%). The age ranged from 17 years to 58 years (average 38.2 years). Eleven patients were victims of traffic accidents and another 1 fell from a height. Most of them had associated injuries, including fractures elsewhere, head injury, chest and abdominal trauma. Five of them were treated with open reduction and internal fixation with Huckstep intramedullary nail, 2 with Kuntscher nail and Knowles' pin, 2 with plate and c

Osteoclast cytomorphometry in patients with femoral neck fracture.

In patients with femoral neck fracture, nutritional deficiencies have been shown to be common. A low calcium diet and/or a reduced vitamin D intake have been suspected to cause secondary hyperparathyroidism responsible for increased bone turn over and bone loss. Parathyroid hormone (PTH) levels are increased in these patients, data which are in accordance with the pronounced changes observed on bone biopsies reflecting a true hyperparathyroidism. We have used a cytomorphometrical approach to characterize PTH-induced changes on the osteoclastic population. Osteoclasts were detected histochemica

Nonunion of a distal femoral epiphyseal fracture-separation.

Distal femoral physeal fractures account for approximately 1% of all epiphyseal injuries. Complications include growth arrest and leg length discrepancy, malunion and deformity, and stiffness and knee instability. However, nonunion of such a fracture is exceedingly rare and has only been reported in patients with spinal dysraphism. We report a case of distal femoral physeal nonunion in a neurologically intact adolescent athlete. Initially, there was a delay in the diagnosis of this nondisplaced fracture, as the patient had normal roentgenograms. Additional imaging modalities (magnetic resonanc

[Intramedullary nailing of old femoral fracture in children].

Intramedullary nailing was performed for 40 old femoral fractures in children. Open intramedullary nailing was used in all fractures. The results showed that intramedullary nailing was suitable for upper and middle third femoral fractures, including transverse or short oblique fractures, multiple segmental fractures, malunited fractures and fractures failed to unite after internal fixation with plate. The authors consider that fresh long oblique or spiral fractures, severe comminuted fractures and lower third femoral fractures are not suitable for intramedullary nailing. In this series, 34 pat…

[Elastic osteosynthesis in intracapsular fracture of the femoral neck].

The authors present a method of treatment for transcervical fractures of the femoral neck using implants with mechanical characteristics--curved shape, smooth surfaces and elastic resistance to deformity--which respect the biomechanical properties of the normal bone. These implants maintain a close contact between the bony fragments during convalescence and preserve the blood supply of the bone. In this way, bone healing is encouraged and the development of ischaemic necrosis of the femoral head is prevented. Between 1983 and 1985, 126 fractures, of the femoral neck, 120 recent and 6 old, were

Fracture of the femoral neck after internal fixation.

A case of femoral neck fracture despite the presence of a previously inserted sliding screw and sideplate is reported. Internal fixation in itself did not prevent subsequent fracture. A critical factor in the mechanism of this fracture was the patient's underlying disease, felt to be alcoholic osteomalacia.

Relevance of osteoporosis in women with fracture of the femoral neck.

A prospective study of fractures of the femoral neck was conducted over 12 months in order to ascertain the relevance of generalised osteoporosis as determined by metacarpal morphometry. A series of some 200 women sustaining a fracture of the femoral neck after minor trauma had bone mass measurements similar to those of a control population of normal women, and 16% were not osteoporotic. A history of previous fractures was documented in one third of the women, but this was unrelated to the presence or severity of osteoporosis, although over half of the fractures had occurred within the previou

Treatment of femoral fracture in the child with head injury.

The outcome of various types of treatment for femoral fracture in children with head injury was studied retrospectively in 51 patients with 56 fractures. Of these, 36 patients (71 per cent) were in deep coma and scored 5 to 7 on the Glasgow scale. Forty-three children (84 per cent) were eventually able to walk freely. Open reduction and internal fixation proved an attractive solution for femoral fractures in children with head injury who could tolerate general anaesthesia: intramedullary nailing was safe and gave satisfactory results in 16 fractures but infection complicated three of the five …
