High-resolution sonography is a rapidly evolving technique that is gaining an increasing success in the assessment of crystalline arthropathies. In calcium pyrophosphate dihydrate crystal deposition disease, the sonographic features of crystal deposition include hyperchoic spots within hyaline cartilage and/or fibrocartilage and soft tissue calcifications. The aim of this pictorial essay was to present the main findings evocative of crystal deposition in patients with pyrophosphate arthropathy.

Background: Microfracture is a common procedure for cartilage repair, but it often produces inferior fibrocartilage. We previously reported that a super positively charged SOX9 (scSOX9) promoted hyaline-like cartilage regeneration by inducing bone marrow derived mesenchymal stem cell differentiation into chondrocytes in vivo. Here we examined the long-term efficacy of cartilage repair induced by microfracture with scSOX9 by assessing the biomechanical property of the repaired cartilage. Methods: A cartilage defect was created at the right femoral trochlear groove in New Zealand female rabbits …

Both the technique and the technology involved in knee arthroscopy have undergone considerable development over the last 10 years. Diagnostically, the procedure allows visualization of synovium, hyaline cartilage, fibrocartilage (menisci), and ligaments (cruciate, patellofemoral alignment). Synovitis of rheumatoid arthritis is suspected by boggy, hyperemic, and edematous hypervilli that may undergo necrosis. In osteoarthritis, the synovium is often mildly inflamed and the cartilage demonstrates ulcerations, yellowing, softening, and areas of denuded bone. Synovectomy for rheumatoid arthritis w…

This review examines whether MRI can reliably replace wrist arthroscopy in diagnosing TFCC injuries of the ulnar wrist, based on 18 articles selected through PubMed, Embase, and Cochrane Library. MRI shows improvement in diagnosing TFCC injuries, and the predictive values of MRI get significantly better by using tools like post-processing techniques, contrast, and pathological markers. However, MRI is unreliable as an independent diagnostic tool for TFCC lesions that need surgical treatment. More well-designed studies are required to determine this diagnostic modality's clinical relevance.

INTRODUCTION: the forearm is considered a joint in itself where the ulna and radius interact. It is composed of the proximal radioulnar joint (PRUJ), the interosseous membrane (IOM), with the central band as the main component, and the distal radioulnar joint (DRUJ), which includes the triangular fibrocartilage complex (TFCC). Essex-Lopresti lesion (ELL) is a complex injury caused by axial forearm loading, leading to longitudinal radioulnar dissociation and stability loss due to IOM rupture, PRUJ injury with radial head involvement, and DRUJ injury with TFCC involvement. Early diagnosis is cru…

The ulnar shortening osteotomy (USO) is a common procedure used to treat ulnar impaction syndrome secondary to static or dynamic ulnar-positive variance. There are many described techniques for the USO. The distal metaphyseal ulnar shortening osteotomy (DMUSO) with retrograde cannulated screw fixation was described to reduce complications seen with other techniques. Biomechanical analysis of fixation constructs demonstrates 2-screw constructs are significantly stiffer than 1 screw and antegrade constructs have similar or greater stiffness when compared with retrograde constructs. Here, we desc

Common indications for surgical procedures of the wrist and hand include acute fractures or fracture-dislocations; nonunited fractures; posttraumatic, degenerative, and inflammatory arthritides and tendinopathies; injuries to tendons, ligaments, and the triangular fibrocartilage complex; and entrapment neuropathies. Soft tissue or osseous infections or masses may also need surgical treatment. Several of these procedures require surgical hardware placement, and most entail clinical follow-up with periodic imaging. Radiography should be the first imaging modality in the evaluation of the postope…

Triangular fibrocartilage complex tears are a common source of ulnar-sided wrist pain and distal radioulnar joint instability. Symptoms recalcitrant to conservative management or injuries in high-demand athletes may indicate surgical management. Both open and arthroscopic techniques offer improvements in objective measures, patient-centered outcome scores, and return to work, but may be complicated by nerve irritation, persistent wrist instability, and pain. Recently developed knotless arthroscopic techniques are not well studied but may limit morbidity.

OBJECTIVE: The aim of the procedure is to visualize the proximal pouch of the DRUJ, the joint surfaces of the sigmoid notch and the ulnar head, the convexity of the ulnar head and the proximal ulnar side surface of the triangular fibrocartilage complex (TFCC).
INDICATIONS: Arthroscopy of the distal radioulnar joint is applied for the evaluation of joint pathologies in ulnar-sided wrist pain, especially in cases without diagnostic findings in standard X‑rays and MRIs and arthroscopically assisted procedures.
SURGICAL TECHNIQUE: In vertical extension, two portals of the wrist are created on th…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2011eng
Arthroscopy of the wrist is a frequently performed procedure. Its role in diagnosis and treatment is significant. The complications of arthroscopy are well known and are described in the literature. We describe a case of accidental section of the ulnar nerve during repair of the triangular fibrocartilage complex during arthroscopy. The nerve section was caused by the trocar used for drainage in the 6U portal. We propose to establish the injury mechanism and describe a safe procedure for this examination.

Distal radius fractures with severe displacement or concomitant triangular fibrocartilage complex tears may be accompanied by distal radioulnar joint instability. Clinicians should examine the distal radioulnar joint closely when managing wrist fractures and treat coexisting instability appropriately. Chronic instability from distal radius malunion may require osteotomy or radioulnar ligament reconstruction. With proper management, most patients recover forearm stability and rotational motion after distal radius fracture.

Osteoarthritis (OA) is a condition that affects the quality of life of millions of patients worldwide. Current clinical treatments, in most cases, lead to cartilage repair with deposition of fibrocartilage tissue, which is mechanically inferior and not as durable as hyaline cartilage tissue. We designed an mRNA delivery strategy to enhance the natural healing potential of autologous bone marrow aspirate concentrate (BMAC) for articular cartilage repair. We used mineral-coated microparticles to deliver TGF-β1 mRNA to autologous BMAC. mRNA-activated BMAC was suspended in peripheral blood to gene…

We report that 3-D printed scaffold channel size can direct bone marrow derived stem cell differentiation. Treatment of articular cartilage trauma injuries, such as microfracture surgery, have limited success because durability is limited as fibrocartilage forms. A scaffold-assisted approach, combining microfracture with biomaterials has potential if the scaffold can promote articular cartilage production and share load with cartilage. Here, we investigated human bone marrow derived stromal cell (hBMSC) differentiation in vitro in 3-D printed silica/poly(tetrahydrofuran)/poly(ε-caprolactone) h…

The distal radioulnar joint (DRUJ) is the distal articulation between the radius and ulna, acting as a major weight-bearing joint at the wrist and distributing forces across the forearm bones. The articulating surfaces are the radial sigmoid notch and ulnar head, while the ulnar fovea serves as a critical attachment site for multiple capsuloligamentous structures. The DRUJ is an inherently unstable joint, relying heavily on intrinsic and extrinsic soft-tissue stabilizers. The triangular fibrocartilage complex (TFCC) is the chief stabilizer, composed of the central disk, distal radioulnar ligam…

PURPOSE: The triangular fibrocartilage complex (TFCC) is composed of dorsal and palmar radioulnar ligaments (RULs). A common injury pattern of the RUL is the avulsion of the ulnar insertion, which can be treated by arthroscopic or open repair. Although the general method of TFCC reconstruction is a tendon graft with a bone tunnel, detailed information regarding the radial attachment of the RUL is unclear. This study aimed to clarify the morphology of the radial attachments of the palmar radioulnar ligament (PRUL), dorsal radioulnar ligament (DRUL), and short radiolunate ligament (SRL) using th…

Congenital external auditory canal stenosis (EACS) is a spectrum of abnormalities affecting the external and middle ear. We report a 6 year-old patient with EACS affecting the lateral fibrocartilaginous canal that was successfully repaired. This patient highlights a variant of EACS characterized by lateral soft tissue narrowing with normal osseous development. Most previous studies of CAA have described severe forms associated with complete atresia, bony stenosis, and middle ear malformations. Stenosis affecting only the fibrocartilaginous canal is a milder form resulting from premature arrest

With the evolution of dry wrist arthroscopy, there is an ever-increasing role for its use in the treatment of disorders of the wrist including ligament repair or reconstruction, bony procedures such as distal radius corrective osteotomies or fracture fixation, and partial arthrodesis. We describe some of the tips and tricks that can be used to manage ulnar-sided wrist pain. We particularly emphasize the different technical points to perform dry wrist arthroscopy compared with previously described wet arthroscopic procedures.

Ulnocarpal impaction (UCI) syndrome is a well-recognized and relatively frequent cause of ulnar-sided wrist pain and limitation of motion. In the setting of negative or questionable negative radiographs and a strong clinical suspicion for UCI, magnetic resonance imaging (MRI) is helpful in detecting occult disease. Current MRI technology is capable of providing high-spatial-resolution images on multiple planes while manipulating contrast to highlight different tissue types. However, imaging of the triangular fibrocartilage complex has been challenging because of the small and complex component…

Osteoarthritis is the most common joint disorder and a major cause of disability with a major socio-economic impact. In these circumstances is very important to understand its pathogenesis. Although previous research focused primarily on changes in the articular cartilage, more recent studies have highlighted the importance of the subchondral bone, synovium, menisci, ligaments, periarticular muscles and nerves. Now osteoarthritis is viewed as a multifactorial disease affecting the whole joint.
