Reducing strains within the tibia and fibula during running may reduce the risk of stress fractures. We examined the effect of reduced flight time during running (i.e., grounded running) on finite-element predicted bone strains within the tibia-fibula complex. Eighty-eight consecutive patients underwent anterior cervical discectomy (ACD) with banked fibula fusion and internal fixation using the locking cervical plate. Recurrent stress fractures rarely affect the same athlete. We present the case of a female triathlete who suffered multiple stress fractures in both tibias, the right fibula, and the left femoral neck. Conservative treatment was instituted in all episodes, with rest, reduced training load, and physical therapy rehabilitation. The relative energy deficiency in sport syndrome, along with an eating disorder, training overload, and osteopenia, was identified as a risk factor. Ankle fracture with involvement of the growth plate is the second most common paediatric fracture after the distal radius. The most common fracture type according to Salter Harris (SH) is type II of the distal tibia combined with green stick of the fibula. Isolated fracture of the distal fibular growth plate is not common and as a rule it does not give any growth arrest. We describe a case of isolated fibular fracture SH type II in a ten year-old girl which ended with symptomatic valgus deformity of the ankle. The patient was operated with good results.

In a series of 16 cases of stress fractures in 15-year-old and younger athletes 8 fractures occurred in boys and 8 in girls. There were no differences between the sexes in the athletes' training habits. Ten of the fractures were located at the tibia, seven at its upper third and three at the lower part of the bone. Three fractures were found in the fibula, in the metatarsal bones two stress fractures and in the femur one stress fracture. Most stress fractures were caused by endurance type sports. The daily training distances were not particularly high at the time of the onset of the symptoms.

Three cases of congenital pseudarthrosis of the fibula are reported, all with prepseudarthrotic involvement of the omolateral tibia. In patients with this condition, correction of valgus deformity should be deferred until puberty, and the occurrence of tibial fractures should be prevented by means of leg splints.

BACKGROUND: Reconstruction after excision of the humeral malignancy is a challenging issue for the reconstructive surgeon. The combined use of a fibular flap and allograft can provide a reliable reconstructive option. This article describes the authors' experience with this technique for the treatment of segmental bone defects after resection of humeral malignancy.
METHODS: From 2005 to 2008, seven patients that had intercalary resection of humeral malignancy underwent reconstruction with an allograft and vascularized fibula construct. Patients were examined clinically and radiographically.

A vascularised fibular strut graft was used for anterior spinal fusion in 16 patients with spinal kyphosis. The procedure was abandoned in three because of difficulty in establishing a vascular anastomosis and in one because the grafted fibula dislodged two days after operation. One patient died after five days. Of the 11 remaining patients, there were seven males and four females. Their ages at the time of operation averaged 30.9 years (12 to 71). An isolated fracture of the middle and lower third of the tibia occurred with marked regularity in a large number of camel-racing jockeys. The mechanism of this fracture was analysed. It was concluded that the compression-rotation of the leg below the knee between the trunks of adjacent racing camels produced this injury, but the sparing of the fibula could not be explained. Simple measures are described such that the incidence of these and other injuries sustained by the jockeys can be markedly reduced.

The juvenile Tillaux fracture is an avulsion fracture (Salter-Harris type-3) of the anterolateral comer of the distal tibial epiphysis.
