The phenomenology of posttraumatic stress disorders following a natural disaster.

This study examined the utility of the DSM-III diagnostic criteria for posttraumatic stress disorder (PTSD) in a high-risk group of 50 firefighters who had had an intense exposure to a natural disaster 8 months before being interviewed. Follow-up over the next 3 years allowed examination of the ability of these diagnostic criteria to predict a pattern of chronic posttraumatic morbidity. They predicted a pattern of chronic disorder, demonstrated by the finding that eight of the 15 subjects who had definite or borderline PTSD at 8 months remained symptomatic 3 years later. A disturbance of atten…

Development of a perceptual hyperthermia index to evaluate heat strain during treadmill exercise.

Fire suppression and rescue is a physiologically demanding occupation due to extreme external heat as well as the physical and thermal burden of the protective garments. These conditions challenge body temperature homeostasis and results in heat stress. Accurate field assessment of core temperature is complex and unreliable. The present investigation developed a perceptually based hyperthermia metric to measure physiologic exertional heat strain during treadmill exercise. Sixty-five (28.9 ± 6.8 years) female (n = 11) and male (n = 54) firefighters and non-firefighting volunteers participated i…

Comparisons between Shikoro-type helmet with no hood and typical fire protective helmets with hood in a hot and humid environment.

The purpose of this study was to evaluate physiological and subjective responses while wearing the Shikoro-type helmet for firefighters when compared to typical helmets. Eight firefighters conducted a 30-min exercise at a 5 km h-1 in three helmet conditions at an air temperature of 32 °C with 70%RH. The results showed that no significant differences in rectal, mean skin temperature and physiological strain index among the three conditions were found during exercise and recovery. Skin temperatures on the cheek, ear and neck during exercise were significantly lower for the Shikoro-type condition…

Chronic pain among public safety personnel in Canada.

Background: Chronic pain is highly prevalent in the general population and may be even higher among public safety personnel (PSP; e.g., correctional officers, dispatchers, firefighters, paramedics, police). Comprehensive data on chronic pain among diverse Canadian PSP are relatively sparse. Aims: The current study was designed to provide initial estimates of chronic pain frequency and severity among Canadian PSP. Methods: Estimates of chronic pain frequency and severity (i.e., intensity and duration) at different bodily locations were derived from self-reported data collected through an online…

Partial rupture of the Achilles tendon during a simulated fire ground task: insights obtained from a case report for the prevention and reporting of musculoskeletal injury.

BACKGROUND: In this case report an incumbent firefighter partially ruptured his right Achilles tendon during a study of the physical demands of firefighting. METHODS: Kinematics and kinetics of the lower limbs and trunk were collected while the firefighter performed two simulated fire ground tasks. From this unexpected event, two insights were obtained that should be considered in all future injury prevention and reporting efforts. FINDINGS: (i) Consider the full anatomical linkage--the right ankle and knee kinematics leading up to the onset of injury trial were comparable to all preceding r

Unmasking the Hidden Danger: A Decade-Long Systematic Review of Case-Control Studies on Single Occupational Risks and Prostate Cancer.

The present systematic review addresses the influence of occupational exposures on prostate cancer risk. Eleven studies were analyzed for a range of occupational exposures, including but not limited to firefighting, physical activity, night shift work, chemical exposure, and solar ultraviolet radiation. The results of the review reveal that firefighters exposed to harmful substances, individuals engaged in physically strenuous work, and workers with chronic night shift routines showed an increased likelihood of developing prostate cancer. Moreover, the review identified an increased risk assoc…

The Only Way Is Up: Active Knee Exoskeleton Reduces Muscular Effort in Quadriceps During Weighted Stair Ascent.

Firefighters consistently rank stair ascent with gear, which can weigh over 35 kg, as their most demanding activity. Weighted stair climbing requires dynamic motions and large knee torques, which can cause exhaustion in the short term, and overuse injuries in the long term. An active knee exoskeleton could potentially alleviate the burden on the wearer by injecting positive energy at key phases of the gait cycle. Similar devices have reduced the metabolic cost for various locomotion activities in previous studies. However, no information is available on the effect of active knee exoskeletons o…

Development and validation of the trauma-specific emotional counting Stroop paradigm for fMRI study.

BACKGROUND: The emotional-counting Stroop (ecStroop) is a cognitive task to evaluate emotional information processing. This study aimed to develop a trauma-specific ecStroop protocol for firefighters and assess its validity as a functional magnetic resonance imaging (fMRI) activation paradigm. METHODS: To develop the ecStroop protocol, trauma-related words for firefighters were selected from previous studies, and general negative and neutral words were matched corresponding to the number of letters and syllables, parts of speech, and frequency in the Korean language. The negative emotional va…

Treating Panic Disorder Hypnotically.

A hypnosis protocol for treating panic disorder is provided. The implementation of this protocol is demonstrated through a case example involving the successful treatment of a 28-year-old firefighter presenting with a 4-month history of near-daily panic attacks. Core principles associated with this protocol include: (1) Elementary education about the physiology of panic; (2) A review of primary factors contributing to the evolution and manifestation of panic; (3) Encouragement of physical activity; (4) Utilization of hypnosis applications; and (5) Monitoring and measuring progress evidenced by

Occupational PAH exposures during prescribed pile burns.

Wildland firefighters are exposed to particulate matter and gases containing polycyclic aromatic hydrocarbons (PAHs), many of which are known carcinogens. Our objective was to evaluate the extent of firefighter exposure to particulate and PAHs during prescribed pile burns of mainly ponderosa pine slash and determine whether these exposures were correlated with changes in urinary 1-hydroxypyrene (1-HP), a PAH metabolite. Personal and area sampling for particulate and PAH exposures were conducted on the White Mountain Apache Tribe reservation, working with 21 Bureau of Indian Affairs/Fort Apache…

Direct versus indirect emotional consequences on the Iowa Gambling Task.

The Iowa Gambling Task has been widely used in the assessment of neurological patients with ventro-mesial frontal lesions. The Iowa Group has claimed that the Gambling Task is too complex for participants to follow using cognition alone, so that participants must rely on emotion-based learning systems (somatic markers). The present study investigates whether similar tasks can be performed without direct somatic markers. In a 'Firefighter' task closely matched to the classic Gambling Task, participants evaluate the performance of others--so that they experience reward and punishment indirectly.

Physiological Demands of Basic Fire Management Tasks in Members of the Canadian Armed Forces: A Pilot Study.

INTRODUCTION: Members of the Canadian Armed Forces (CAF) are required to meet the minimum standards of the Fitness for Operational Requirements of CAF Employment (FORCE) job-based simulation test (JBST) and must possess the capacity to perform other common essential tasks. One of those tasks is to perform basic fire management tasks during fire emergencies to mitigate damage and reduce the risk of injuries and/or death until professional firefighters arrive at the scene. To date however, the physiological demands of common firefighting tasks have mostly been performed on professional firefight…
