The novel seizure quality index for the antidepressant outcome prediction in electroconvulsive therapy: association with biomarkers in the cerebrospinal fluid.

For patients with depression treated with electroconvulsive therapy (ECT), the novel seizure quality index (SQI) can predict the risk of non-response (and non-remission)-as early as after the second ECT session-based the extent of several ictal parameters of the seizure. We aim to test several CSF markers on their ability to predict the degree of seizure quality, measured by the SQI to identify possible factors, that could explain some variability of the seizure quality. Baseline CSF levels of metabolites from the kynurenine pathway, markers of neurodegeneration (tau proteins, β-amyloids and n

A bedside ultrasound technique for fluid therapy monitoring in severe hypovolemia: Tissue Doppler imaging of the right ventricle.

Fluid therapy is one of the main issues for hemodynamic resuscitation. Tissue Doppler imaging (TDI) of the right ventricle (RV) with bedside ultrasound (BUS) technique is a new dynamic method to identify fluid responsiveness in patients with hypotension. Here, we present the case of a hypotensive patient monitored with TDI measurements of RV. A 75-year-old male patient was admitted to the emergency department (ED) with the complaint of diarrhea. He was in severe hypovolemia, with hypotension, tachycardia, and tachypnea. His laboratory results were normal. BUS was performed on the patient by th…

[Concentration of benzylpenicillin sodium salt in the cerebrospinal fluid of patients with late neurosyphilis as an indicator of efficacy of specific therapy].

A permeability of crystallized benzylpenicillin sodium salt (CBSS) through a blood-brain barrier (BBB) in patients with late neurosyphilis and experimental animals has been studied. Forty-six patients with vascular and late meningovascular neurosyphilis have been examined. A group of experimental animals included 50 rabbits. The injection of CBSS to patients and animals was followed by the measurement of antibiotic's concentration in the cerebrospinal fluid (CSF) using high performance liquid chromatography. The level of CSF penicillin concentration was estimated as an indicator of efficacy of

Possibilities of perfecting antimicrobial therapy. Dynamics of penicillin G concentrations in the cerebro-spinal fluid in purulent meningitis.

In 43 adult patients with purulent meningitis receiving daily doses of 1-4,000,000 IU penicillin G i.v. in bolus, determinations were made, 1-6 hrs after administration, in 153 samples of cerebrospinal fluid (CSF), to ascertain the concentrations of penicillin that have reached the CSF. After 1 hour maximum levels of penicillin were noted, exceeding by far the meningococcus and pneumococcus minimum inhibitory concentration (MIC). Generally, high penicillin concentrations persisted in the CSF, in most of the cases at least for the following 6 hours. The advantages of using penicillin G i.v. in

Fluid overload in patients with severe sepsis and septic shock treated with early goal-directed therapy is associated with increased acute need for fluid-related medical interventions and hospital death.

Early goal-directed therapy (EGDT) consists of early, aggressive fluid resuscitation and is known to improve survival in sepsis. It is unknown how often EGDT leads to subsequent fluid overload and whether post-EGDT fluid overload affects patients' outcomes. Our hypothesis was that patients with sepsis treated with EGDT were at risk for fluid overload and that fluid overload would be associated with adverse outcomes. We conducted a retrospective cohort of 405 consecutive patients admitted with severe sepsis and septic shock to the medical intensive care unit of a tertiary care academic hospital

Letter to the editor on: "Effect of different targets of goal-directed fluid therapy on intraoperative hypotension and fluid infusion in robot-assisted laparoscopic gynecological surgery: a randomized non-inferiority trial".

This study examined how different goal-directed fluid therapy types affected low blood pressure and fluid infusion during robot-assisted laparoscopic gynecological surgery. They used carotid corrected flow time (FTc) and tidal volume stimulation pulse pressure variation (VtPPV) to check the patient's volume status and responsiveness. The findings indicated that various fluid therapy targets significantly influence intraoperative hypotension and fluid requirements. However, the study exclusively employed unilateral carotid ultrasound assessments, potentially overlooking physiological or patholo…

Microbial spectrum and antimicrobial resistance are associated with outcomes after endoscopic ultrasound-guided therapy of pancreatic fluid collections.

Infected pancreatic fluid collections (PFC) are among the most challenging entities in therapeutic endoscopy, requiring multiple interventions. In this multicenter study, we investigated whether microbiological profiles and antimicrobial resistance are associated with outcomes after endoscopic ultrasound-guided drainage. The primary endpoint was time to clinical success, defined as radiographic resolution of the PFC to < 30 mm, no need for additional intervention, and clinical improvement. Secondary outcomes included surgery-free survival, number of interventions, and ICU stay. Among 398 inclu

Effect of human urinary microenvironment and fluid flow on antibiotic and phage therapy efficacy against uropathogenic Escherichia coli.

Urinary tract infection (UTI) remains a major global health burden, with frequent recurrences and rising antimicrobial resistance compromising treatment efficacy. Conventional susceptibility assays often fail to predict clinical outcomes, highlighting the need for more physiologically relevant infection models. Here, we investigated how microenvironmental complexity influences uropathogenic Escherichia coli (UPEC) responses to antibiotics and bacteriophages using human urine, a three-dimensional human urothelial microtissue model (3D-UHU), and a novel mesofluidic platform (P-FLO) that applies

CSF Inflammation Markers Associated with Asymptomatic Viral Escape in Cerebrospinal Fluid of HIV-Positive Individuals on Antiretroviral Therapy.

HIV establishes a viral reservoir in the CNS despite viral suppression in the blood on antiretroviral therapy (ART). In a minority of people with HIV (PWH), HIV RNA is detectable in CSF when HIV RNA in plasma is undetectable or HIV RNA levels are higher in CSF compared with plasma, an event termed CSF viral escape that can occur with or without neurological symptoms. Asymptomatic CSF viral escape occurs in 3-20% of PWH on ART, yet associated biomarkers are unclear. To identify biomarkers associated with asymptomatic CSF viral escape, we performed a matched group study of PWH on ART with vs. wi

Refilling and preload dependence failed to predict cardiac index decrease during fluid removal with continuous renal replacement therapy.

BACKGROUND: Fluid removal can reduce the burden of fluid overload after initial resuscitation. According to the Frank-Starling model, iatrogenic hypovolemia should induce a decrease in cardiac index. We hypothesized that inadequate refilling detected by haemoconcentration during fluid removal or an increase in cardiac index (CI) during passive leg raising (PLR) could predict CI decrease during mechanical fluid removal with continuous renal replacement therapy (CRRT). METHODS: We conducted a single-centre prospective diagnostic accuracy study. The primary objective was to investigate the diagn

Hydromorphone and codeine concentrations in oral fluid specimens from patients receiving substitution therapy with Substitol™ (morphine sulfate).

This study aimed to determine whether hydromorphone and codeine can be detected in oral fluid specimens following administration of Substitol™, a slow-release formulation of morphine. This is of interest for those monitoring treatment compliance using drug testing. Oral fluid specimens collected for compliance assessment in routine clinical practice or as part of a clinical trial were subjected to quantitative analysis of hydromorphone, morphine, codeine, and 6-acetylmorphine using highly sensitive mass spectrometric methods. Oral fluid was collected using a Greiner Bio-One saliva collection s

Inflamed synovial fluid induces a homeostatic response in bone marrow mononuclear cells in vitro: Implications for joint therapy.

Synovial inflammation is a central feature of osteoarthritis (OA), elicited when local regulatory macrophages (M2-like) become overwhelmed, activating an inflammatory response (M1-like). Bone marrow mononuclear cells (BMNC) are a source of naïve macrophages capable of reducing joint inflammation and producing molecules essential for cartilage metabolism. This study investigated the response of BMNC to normal (SF) and inflamed synovial fluid (ISF). Equine BMNC cultured in autologous SF or ISF (n = 8 horses) developed into macrophage-rich cultures with phenotypes similar to cells native to norma
