BACKGROUND: There is both increased interest and awareness in diabetes-associated Charcot foot arthropathy. The number of affected patients will likely increase as the incidence of both diabetes and morbid obesity increases. Many experts now favor surgical correction of the deformity rather than longitudinal management with accommodative bracing. In patients with open wounds and exposed bone and/or chronic osteomyelitis, it is controversial whether resolution of the bony infection should be achieved before attempting surgical correction of the acquired deformity.

The purpose of this study was to compare open to percutaneous tendo-achilles lengthening (TAL) as treatment for equinus deformity (ED) in children with cerebral palsy (CP). Eighteen ambulatory spastic CP children (28 feet) with isolated primary fixed ED were randomized to these two methods and prospectively followed up 7 to 18 months postoperatively (mean 11 months). The study found that the percutaneous TAL gave shorter operative time, shorter hospitalization period, better active dorsal and plantarflexion abilities, better parent satisfaction, and lower complication rate. It was concluded th

Nineteen ambulant patients (children and adolescents, 23 feet), with mild-to-moderate cavus and claw feet of neuromuscular etiology, underwent a modified Chuinard-Baskin operation, combining long toe extensor tendon transfer to the metatarsals with distal tenodesis onto the proximal phalanx. The technique aims to improve forefoot deformity and metatarsophalangeal dysfunction, and to enhance ankle dorsiflexion. Transcutaneous flexor tenotomies in several toes, and additional complementary surgery in the middle and hindfoot (plantar soft tissue releases, tendon lengthenings, and/or transfers and

Neuroarthropathy of the midfoot may lead to a structural deformity that predisposes the diabetic patient to skin breakdown and ulceration. In some cases, conservative management is not adequate, making surgical intervention necessary. The authors performed a retrospective study to look at those patients who required surgical intervention for a specific pattern of neuroarthropathy. Over a 2 1/2-year period, 32 feet (31 patients) underwent surgical procedures for treatment of nonhealing neuropathic ulcerations beneath the lateral column of Charcot feet. All feet underwent exostectomy with 17 und

Progressive collapsing foot deformity (PCFD) is a debilitating condition causing significant biomechanical impairments. Custom-made foot orthoses (FOs) provide a non-invasive treatment by supporting the medial longitudinal arch and improving foot and ankle biomechanics. While previous studies on PCFD have predominantly investigated the biomechanical effects of FOs during walking, their impact on more demanding dynamic tasks remains unclear. This study aimed to investigate the effects of custom-made FOs on lower limb kinematics and kinetics during step-up (SU), step-down (SD), and step-down on

BACKGROUND: While weightbearing computed tomography (WBCT) has been instrumental in analyzing total ankle arthroplasty (TAA) positioning, there is a notable gap in the literature regarding adaptive changes in the foot's medial column after TAA. This study aims to bridge this gap by comparing preoperative and postoperative alignments of the foot's medial column and analyzing if a correlation exists between TAA coronal alignment correction and medial column alignment adaptation.
METHODS: Sixty patients who underwent a lateral approach TAA for end-stage osteoarthritis (OA) between January 2021 a

AIM AND BACKGROUND: The introduction of the 'Vitruvian' Foot concept in 2019 established initial parameter measurements of the 'normal' foot quadrilateral (FQ) in the sagittal plane. This effort had been aimed at increasing precision when analysing alignment abnormalities in post-traumatic and congenital foot deformities, so as to more accurately guide corrective treatment planning with osteotomies. The current study enhances the prior, existing data with an expanded normal cohort. We also offer a case report that describes the successful application of the revised reference angles for our ana…

BACKGROUND: Progressive Collapsing Foot Deformity (PCFD) is a complex multiplanar deformity commonly treated with osteotomy-based reconstruction in flexible cases. Although weightbearing CT (WBCT) has improved assessment of PCFD, WBCT-based predictive planning models remain unexplored.
PURPOSE: To evaluate the accuracy of a WBCT-based predictive surgical planning model in estimating postoperative alignment following reconstruction for flexible PCFD.
STUDY DESIGN: Retrospective cohort study METHODS: Thirty-four patients with PCFD who underwent reconstruction with medial displacement calcaneal

INTRODUCTION: Spastic foot deformity after central nervous system lesions is clinically heterogeneous and often combines equinus, varus, claw toes, pain, and gait limitation. Selective tibial neurotomy (STN) is a peripheral neurosurgical option for focal lower-limb spasticity, but data from low- and middle-income settings remain limited.
RESEARCH QUESTION: What are the long-term clinical and functional outcomes of STN for spastic foot deformity, and are these outcomes influenced by the extent of motor fascicle resection?
MATERIAL AND METHODS: We conducted a retrospective study of 31 patients

BACKGROUND: Long-distance running is popular but associated with a high risk of injuries, particularly toe-related injuries. Limited research has focused on preventive measures, prompting exploration into the efficacy of raised toe box running shoes.
PURPOSE: This study aimed to investigate the effect of running shoes with raised toe boxes on preventing toe injuries caused by distance running.
METHODS: A randomized crossover design involved 25 male marathon runners (height: 1.70 ± 0.02 m, weight: 62.6 + 4.5 kg) wearing both raised toe box (extended by 8 mm along the vertical axis and 3 mm al

BACKGROUND: The purpose of this study is to investigate the biomechanical effect of medial displacement calcaneal osteotomy (MDCO), subtalar joint fusion (SF), and medial ligament reconstruction (MLR: deltoid-spring ligament) in a severe flatfoot model. We hypothesized that (1) combination of MDCO and SF improves the tibiotalar and foot alignment in severe progressive collapsing foot deformity (PCFD) cadaver model. (2) However, if a residual valgus heel alignment remains after MCDO and SF, it can lead to increased medial ligament strain, foot malalignment, and tibiotalar valgus tilt, which wil

BACKGROUND: Although operative treatment of the flexible progressive collapsing foot deformity (PCFD) remains controversial, correction of residual forefoot varus and stabilization of the medial column are important components of reconstruction. A peroneus brevis (PB) to peroneus longus (PL) tendon transfer has been proposed to address these deformities. The aim of our study was to determine the effect of an isolated PB-to-PL transfer on medial column kinematics and plantar pressures in a simulated PCFD (sPCFD) cadaveric model.
METHODS: The stance phase of level walking was simulated in 10 mi

BACKGROUND: Triple arthrodesis is commonly used to correct rigid progressive collapsing foot deformity (PCFD). These patients often have associated first tarsometatarsal (TMT) instability on lateral weightbearing radiographs. It has not been well established if it is necessary to add first TMT arthrodesis to adequately correct the overall deformity. This study retrospectively examined pre- and postoperative radiographs of PCFD patients with first TMT instability that were managed by triple arthrodesis alone.
METHODS: All triple arthrodesis cases were searched for a single surgeon between 2013

BACKGROUND: Subluxation at the subtalar joint is one of the major radiographic features that characterize progressive collapsing foot deformity (PCFD). Although it is recognized that the cervical ligament plays an important function in maintaining the subtalar joint's stability, its role and involvement in PCFD is largely unknown. The purpose of this study was to assess the prevalence of cervical ligament insufficiency in patients with PCFD and to establish if the degree of its pathology changes with increasing axial plane deformity.
METHODS: This study retrospectively reviewed magnetic reson

BACKGROUND: Patients with longstanding progressive collapsing foot deformity often develop osteoarthritis of the ankle, midfoot, or hindfoot joints, which can be symptomatic or lead to fixed deformities that complicate treatment. The development of deformity is likely caused by ligament degeneration and tears. However, the effect of individual ligament tears on changes in joint contact mechanics has not been investigated.
METHODS: A validated finite element model of the foot was used to compare joint contact areas, forces, and pressures between the intact and collapsed foot, and to evaluate t

RECOMMENDATION: Forefoot varus is a physical and radiographic examination finding associated with the Progressive Collapsing Foot Deformity (PCFD). Varus position of the forefoot relative to the hindfoot is caused by medial midfoot collapse with apex plantar angulation of the medial column. Some surgeons use the term forefoot supination to describe this same deformity (see Introduction section with nomenclature). Correction of this deformity is important to restore the weightbearing tripod of the foot and help resist a recurrence of foot collapse. When the forefoot varus deformity is isolated
