Fracture-dislocations of the shoulder can present difficulties in treatment. Two cases are described to illustrate the complications that can occur when an inexperienced examiner attempts a closed reduction of a two-part anterior fracture-dislocation of the shoulder. Careful clinical and radiologic evaluation (A-P and transcapular or axillary lateral X-rays) followed by appropriate management can help to prevent these problems from occurring. If there is evidence that the fracture is displaced by manipulation, immediate open reduction and internal fixation are recommended.

Four patients with lumbosacral fracture-dislocation are presented. The common mechanism of injury was hyperflexion with compression. A rotational element may be implicated in single facet dislocation. Although lumbosacral fracture-dislocations can be managed conservatively, the best method of treatment is open reduction and bone grafting as soon after injury as possible. Only this will ensure complete correction of the deformity and prevent later deterioration.

Report of a case with dislocated fracture of the clavicle and dysplasia of the first rib with consecutive armplexus paresis and thrombosis of the axillar and brachial vein. In such cases operative treatment is advised.

The authors report 6 cases of fracture and dislocation of the lower lumbar vertebrae (L3, L4, L5) and note that they were usually lesions following violent trauma usually in hyperflexion. The following were treated 1 dislocation L3-L4, 2 dislocations L4-L5, 2 comminuted fractures of L4, 1 fracture of L5, and included in all cases intersomatic arthrodesis at one (five cases) level or two levels (one case) and one posterior internal fixation with Kempf's material. The simplicity of the follow up and the stability of the fixation, permit us to propose the method for all unstable lesions of the lo

Supracondylar fractures of the femor or humerus and fractures of the radius, ulna, tibia or fibula, plus crush injuries of the limbs, most likely should not be manipulated in the field because of the potential for traumatizing adjacent nerves, veins and arteries. An emergency splinting system that allows immobilization without repositioning has been developed. Furthermore, fractures in or about the elbow, wrist, knee, or ankle should not be subjected to reduction in the field, unless there are compelling reasons to justify the risk of producing significant additional trauma. Fractures of the trapezium occur infrequently, accounting for only 3.5 % of all carpal fractures. Trapezium fractures with concomitant Bennett's fracture-dislocations are exceedingly rare without a consensus on treatment recommendations. Here, we present a case of a simultaneous Bennett's fracture with a trapezium fracture in a pediatric patient. Our patient was treated with open reduction and internal fixation. Clinically, the patient was pain free with range of motion comparable to his uninjured side and without joint instability. A review of the literature reveals only twelve other cases re

The aim of this study was to compare outcomes of tibial plateau fracture dislocations (FD) with tibial plateau fractures alone. This study was an analysis of a series of tibial plateau fractures, in which FD was defined as a fracture of the tibial plateau with an associated loss of congruent joint reduction and stability of the knee, and classified by the Moore system. Patient data collected included demographics, injury information, and functional outcomes (short musculoskeletal function assessment [SMFA] score and Pain by the visual analog scale). Clinical outcomes at follow-up were recorded…

CASE: We report an unusual combination of injuries, terrible triad elbow fracture dislocation with ipsilateral avulsion of the triceps, and radial shaft fracture in a 23-year-old man. He was managed surgically and had good postoperative outcomes.
CONCLUSION: Evaluation of the triceps tendon avulsion injury is difficult in this context, such as problems evaluating the rotator cuff injuries after a shoulder fracture dislocation. Because of the diagnostic difficulty, a high index of suspicion is necessary.

An uncommon case of a closed dorsal fracture dislocation of the base of the fifth metacarpal bone associated with a flake fracture of the hamate is described. Under general anaesthesia, the dislocation was reduced by traction and direct pressure over the metacarpal base. To maintain the reduction, Kirschner wires were inserted percutaneously.

BACKGROUND: Despite extensive literature dedicated to determining the optimal treatment of isolated greater tuberosity (GT) fractures, there have been few studies to guide the management of GT fracture dislocations. The purpose of this review was to highlight the relevant literature pertaining to all aspects of GT fracture dislocation evaluation and treatment.
METHODS: A narrative review of the literature was performed.
RESULTS: During glenohumeral reduction, an iatrogenic humeral neck fracture may occur due to the presence of an occult neck fracture or forceful reduction attempts with inade…

Carpometacarpal (CMC) injuries can easily be missed in more than half of the cases. Early diagnosis is crucial for treatment. Although the clinical aspect can lead the treating physician to suspect that anything is going wrong, appropriate radiographs, especially in the lateral view, are crucial for the diagnosis. The most common CMC fracture dislocations affects the 4 th and 5 th joints. Treatment will depend on the type of injury and on the degree of joint involvement. Reduction and fixation are usually required. When only one ray is affected, usually the 5 th , closed reduction and fixation…

Elbow dislocations represent common injuries. A quarter of these injuries involve at least 1 fracture. The sequel of elbow fracture-dislocations can be fraught with complications, including recurrent instability, posttraumatic arthritis, elbow contracture, and poor functional results. The 3 main patterns of injury are valgus posterolateral rotatory instability, varus posteromedial rotatory instability, and transolecranon fracture-dislocation. This article discusses each pattern individually, including the anatomy, the typical injury pattern, and treatment strategies. It also discusses common c

Talus fractures are rare but often associated with complications and significant post-traumatic patient morbidity. They frequently occur at high-energy traumas such as traffic accidents. This is a case report of a young woman who after a fall from a roof, presented in an emergency department with a dislocated ankle which was initially misdiagnosed as a bimalleolar fracture and attempted repositioned as such before X-ray. Dislocated talus fractures, however, need urgent anatomic reduction after diagnostic imaging and often subsequent and acute osteosynthesis.
