Arthroscopic Fixation of Avulsion Fractures of the Lesser Tuberosity of the Humerus: Clinical Outcomes With a Mean 3.5-Year Follow-up.

BACKGROUND: Avulsion fracture of the lesser tuberosity (AFLT) of the humerus has traditionally been treated with open reduction internal fixation using screw fixation. The clinical outcomes of arthroscopic repair with suture anchors remains unknown. HYPOTHESIS: It was hypothesized that arthroscopic fixation with suture anchors would result in good clinical outcomes for the treatment of AFLT. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: This study included 15 patients who underwent arthroscopic repair of AFLT of the humerus between January 2014 and October 2017. All patients had

Why Mechanism Matters: A Literature Review of Simultaneous Ipsilateral Tibial Tuberosity Avulsion and Patella Fracture With Case Report.

Simultaneous ipsilateral tibial tuberosity avulsion and patella fractures are rare in adults. They are often associated with patients who have underlying bone disease and other medical co-morbidities. Here we describe a case where this injury was attributed to direct trauma and demonstrate our department's management for such an injury. In addition to our case report, we have performed a systematic literature review to identify other cases of the same injury. Only four other cases have been reported. Here we summarise and compare the management and outcome measures reported in each case. All p

Operative Treatment for a Painful Nonunion Avulsion Fracture of the Femoral Attachment of the Medial Collateral Ligament in a Teenager: A Case Report.

CASE: A 13-year-old female gymnast presented with medial knee pain despite several months of nonoperative management for a valgus hyperextension injury resulting in a bony avulsion fracture of the medial collateral ligament (MCL) origin at the medial femoral condyle. The MCL was repaired via open reduction and internal fixation (ORIF) with a single 4.0 mm cannulated screw and washer. CONCLUSIONS: ORIF is a successful treatment option for extra-articular bony MCL origin avulsion fractures that fail conservative treatment. Following surgical intervention and structured physical therapy, our pat

Diagnosis of isolated extensor carpi radialis longus (ECRL) tendon avulsion fracture using ultrasound: a paradigm shift.

Isolated avulsion fracture of the extensor carpi radialis longus (ECRL) tendon is a rare and poorly understood injury. We present a unique case of a 45-year-old male who fell on his flexed right hand. He presented with a subtle but extremely painful mass on the dorsum of his wrist. Ultrasound (U/S) imaging of the mass revealed an avulsed bone fragment attached to the ECRL tendon, confirming the clinical suspicion of an ECRL avulsion injury. Computed tomography and magnetic resonance imaging are well-documented imaging modalities to detect tendon avulsions. As demonstrated by this case report,

Combined avulsion fracture of the tibial tubercle and patellar tendon rupture in pediatric population: case series and review of literature.

Simultaneous occurrence of tibial tubercle fracture and patellar tendon avulsion with double-hit injury of the knee extensor mechanism is an extremely rare condition. However, they become more frequent due to increased participation in sports and high-energy recreational activities at younger age. It represents a frequently missed diagnosis; therefore, high index of suspicion is required for the diagnosis of such injuries to avoid delays in the treatment which would lead to possible suboptimal functional outcome. Only few case reports of such injuries were reported in the literature with limit

Arthroscopic suture bridge fixation technique with multiple crossover ties for posterior cruciate ligament tibial avulsion fracture.

PURPOSE: This study examined the clinical outcomes of a newly developed technique, arthroscopic suture bridge fixation with crossover ties of PCL tibial avulsion fracture using two tibial tunnels and a posterior trans-septal portal. METHODS: Records were reviewed of 18 patients (median age 33.5 years, range 13-55 years) with PCL tibial avulsion fractures treated with an arthroscopic suture bridge technique. Knee function before surgery and at last follow-up was evaluated by Lysholm and Tegner scores. A KT-2000 arthrometer was used to evaluate knee stability, and fracture union was assessed by

Arthroscopic fixation of an avulsion fracture of the tibia involving the posterior cruciate ligament: a modified technique in a series of 22 cases.

A total of 22 patients with a tibial avulsion fracture involving the insertion of the posterior cruciate ligament (PCL) with grade II or III posterior laxity were reduced and fixed arthroscopically using routine anterior and double posteromedial portals. A double-strand Ethibond suture was inserted into the joint and wrapped around the PCL from anterior to posterior to secure the ligament above the avulsed bony fragment. Two tibial bone tunnels were created using the PCL reconstruction guide, aiming at the medial and lateral borders of the tibial bed. The ends of the suture were pulled out thr

Repair of Acute Superficial Deltoid Complex Avulsion During Ankle Fracture Fixation in National Football League Players.

BACKGROUND: Infolding and retraction of an avulsed deltoid complex after ankle fracture can be a source of persistent increased medial clear space, malreduction, and postoperative pain and medial instability. The purpose of this descriptive case series was to analyze the preliminary outcomes of acute superficial deltoid complex avulsion repair during ankle fracture fixation in a cohort of National Football League (NFL) players. We found that there is often complete avulsion of the superficial deltoid complex off the proximal aspect of the medial malleolus during high-energy ankle fractures in

Fixation failure in an isolated tibial eminence ACL traction avulsion fracture in a paratrooper: is there an association with vitamin D deficiency?

Tibial eminence avulsion fracture at the ACL footprint may be caused by high-energy forces such as a fall, in which the ACL ligament proves stronger than the forces that hold the bone together. For reasons of bone maturity however, tibial spine avulsion fractures where the ACL remains intact, typically occur in children but are rare in adults. This case demonstrates a rare type of adult tibial avulsion fracture with intact ACL and subsequent fragment fixation failure in which vitamin D deficiency may have been contributory. Because there is a high rate of inadequate vitamin D levels in patient

Infraglenoid tubercle avulsion and greater tuberosity fracture resulting from traumatic glenohumeral anterior dislocation: a case report.

Traumatic anterior shoulder dislocation can be associated with anteroinferior glenoid bone loss causing potential recurrent instability. We report on a 62-year-old man with a first-time traumatic anterior dislocation of the right shoulder, resulting in both an infraglenoid tubercle triceps avulsion fracture and a greater tuberosity fracture. After reduction, nonsurgical management was chosen. No inferior-oriented apprehension was noticed during follow-up, which might necessitate surgical treatment of the inferior glenoid rim. At latest follow-up, the patient had recovered his shoulder function

Arthroscopic physeal sparing all inside repair of the tibial avulsion fracture in the anterior cruciate ligament: technical note.

We describe a new and effective arthroscopic physeal sparing repair of the tibial eminence avulsion fracture using all an inside repair technique. The treatment of ACL avulsion is controversial, especially in skeletally immature patients, with concerns about physeal damage, and a more reliable way of fixation is still being pursued for small or comminuted fragments. Screw fixation and suture cerclage has some limitations, especially in the small fragment or skeletally immature patients. We fixed avulsion fragment using all inside repair between the distal portion of the ACL and transverse liga

Posterior avulsion fracture at adjacent vertebral body during cervical disc replacement with ProDisc-C: a case report.

STUDY DESIGN: A case report. OBJECTIVE: To report posterior avulsion fractures at the adjacent vertebral bodies that occurred during cervical arthroplasty with ProDisc-C, which was thought to be related to design of the prosthesis and lack of the prudence in surgical technique. SUMMARY OF BACKGROUND DATA: There is no previous report on the avulsion fracture of the posterior vertebral body during cervical arthroplasty. METHODS: A 32-year-old-man presented with painful numbness and weakness of the right upper extremity for 2 months. On magnetic resonance imaging and computer tomography scans,

Delayed recurrent hemarthrosis after staple fixation of tibial avulsion fracture of the posterior cruciate ligament: a case report.

This study reports a case of unusual vascular complication related to the staple fixation for the tibial avulsion fracture of the posterior cruciate ligament (PCL). The patient, who experienced recurrent hemarthrosis 12 months after staple fixation for the avulsion fracture of the PCL, was successfully managed by removing the staple and suturing the bleeding focus of the popliteal artery. Injury to the popliteal artery by the prominent staples could be the culprit causing the recurrent hemarthrosis. This type of delayed popliteal artery injury should be kept in mind in open reduction and inter

Tibial avulsion fracture of the posterior cruciate ligament: K-wire or screw fixation? A retrospective study of 26 patients.

Although the posterior cruciate ligament (PCL) is not frequently injured, a greater understanding of its role in stabilizing the knee joint, mechanism of injury and treatment has developed. Isolated avulsion injuries constitute only a subgroup of PCL injuries, but nevertheless several operative techniques have been described for the fixation of the avulsed bony fragment. In order to investigate whether K-wire or screw fixation yields better long-term results, we examined 26 patients at an average of 10.5 years after the initial operation. Clinical examination, activity level, radiographic eval

Avulsion fracture of the tibial eminence associated with severe medial ligamentous injury in an adolescent. A case report and literature review.

In the presence of open physes, tibial eminence fractures are usually isolated injuries. However, adults sustaining a fracture of the tibial eminence are more likely to have other associated injuries, most commonly a tear of the medial collateral ligament. A case of an adolescent with a tibial eminence fracture and a complete tear of the medial collateral ligament is reported. Review of the literature revealed two similar cases in adolescents. If medial instability is demonstrated on valgus stress of the knee in an adolescent with tibial eminence fracture, we believe stress roentgenograms of t
